529d2db852 fix(trekplan): drop TaskCreate/TaskUpdate from the tool lists - measured dead weight
Claude Code 2.1.233 stopped offering the task/todo tools on Opus 4.8, Sonnet 5, Fable 5
and newer unless CLAUDE_CODE_ENABLE_TODO_TOOLS=1 is set. /trekplan still listed
TaskCreate and TaskUpdate in allowed-tools. So did the two orchestrator reference docs,
which mirror the command but are never spawned. No prose in any command calls either tool.

Runtime effect, measured BEFORE removal on CC 2.1.274, claude-opus-5, with
CLAUDE_CODE_ENABLE_TODO_TOOLS unset:
- Probe: a throwaway project command whose frontmatter is
  `allowed-tools: Read, TaskCreate, TaskUpdate`, run headless
  (`claude -p --output-format stream-json --verbose`).
- The session had 151 tools. The only Task* names among them were Task, TaskOutput and
  TaskStop; neither TaskCreate nor TaskUpdate was there.
- The command loaded (listed in slash_commands) and completed with subtype=success,
  is_error=false and 0 tool calls. The model reported the tool as not available.
- `claude plugin validate` does not flag the unknown names either: only the one
  accepted warning appears, before and after.
Verdict: harmless dead weight, no error anywhere, so this is a correctness-of-docs fix
rather than a runtime fix.

End-state gate D-05: open -> closed (defects 5 -> 4 of 7). Suite 1059 (1057/0/2), run
on a clean export of the index.

f6c3535cea fix(trekplan): drop the "execute with team" path - TeamCreate/TeamDelete no longer exist
Claude Code 2.1.178 removed the TeamCreate and TeamDelete tools, so /trekplan's
"execute with team" handoff could never create a team: it always ended in its own
fallback (sequential execution). The path is removed and the fallback is now the only
behaviour. The handoff no longer offers "execute with team"; if a user asks for it
anyway, /trekplan says there is no team path and executes sequentially. Parallel
implementation stays where it already was: --decompose plus /trekexecute's
multi-session waves. No new orchestration replaces it.

- commands/trekplan.md: TeamCreate/TeamDelete removed from allowed-tools; the offered
  option and the team section are gone.
- agents/planning-orchestrator.md (reference doc): the offered option is gone.
- docs/cc-upgrade-2.1.181-decision-matrix.md still names the team path: a historical
  record, left as written.

End-state gate D-01: open -> closed (defects 7 -> 6 of 7). Suite 1059 (1057/0/2), run
on a clean export of the index.

f8d9d7fef9 chore(voyage): release v5.6.1 — one-line descriptions for reference/dormant agents (~700 tok trim)
Trim the always-loaded token cost of the agent listing Claude Code injects
into every session. The three *-orchestrator reference docs
(planning/research/review) and the dormant synthesis-agent carried
multi-paragraph description: frontmatter (full rationale + CC-2.1.172
history + a usage example) despite never being spawnable from the live
/trek* pipeline; their description is now a single line, with the full
rationale already living — and remaining — in each file's body. ~700
tokens trimmed, no behavior change.

The three orchestrators retain the self-declaration "reference document,
not a spawnable capability" (doc-consistency pin); synthesis-agent retains
its DORMANT / not-wired flag + docs/T1-synthesis-poc-results.md pointer.
Bump plugin.json, package.json, package-lock.json, and the README badge to
5.6.1; prepend CHANGELOG v5.6.1 entry + README What's-new note. Surfaced
via config-audit always-loaded token-audit dogfooding.

Additive — no breaking change, no runtime behavior change. Canonical
node --test: 739 pass, 0 fail (2 skipped); version-consistency + agent
inventory + frontmatter pins green.

2df0cbb372 docs(voyage): S33 — reconcile agent inventory (21 spawnable + 3 reference docs) + record D1–D3 considered-and-kept
Balance-backlog S33 (DOC, non-breaking). The D1–D3 forks resolved conservatively
(balance-backlog-plan.md), collapsing the model + observability work into a
documentation record. Three deliverables, all doc-only, no code/frontmatter change:

- V35 (doc half): the "24 agents" headline is reconciled to its honest split —
  21 spawnable (one dormant: synthesis-agent, Δ≈0) + 3 orchestrator reference
  docs (planning-/research-/review-orchestrator document the inline /trek*
  workflow; not spawned). Each orchestrator header now declares itself a
  "reference document, not a spawnable capability". README + CLAUDE.md state the
  split; counts in the new pins are DERIVED from agents/ so they survive reword.
- D2 (V32 rationale): docs/observability.md gains a "Why direct export rather
  than a native collector" section — direct export keeps the path / SSRF /
  field-allowlist guards in audited in-process code (the S21 hardening) instead
  of re-hosting a collector; textfile mode remains the collector escape hatch.
- D3 (kept-opus): docs/voyage-vs-cc-balance-analysis.md §10 decision record —
  opus on V09 (glue), V35 (dormant), V11 (retrieval), V16 (mechanical), V08
  (researchers) was reconsidered for sonnet and KEPT (pin 40d8742 firm).

No agent frontmatter changed — tests/lib/agent-frontmatter.test.mjs is the
structural model source-of-truth and is untouched (diff is description-only:
model: opus + tools lists unchanged, no Agent tool granted). No Handover-1
change; no exporter/gemini-bridge removal. Non-breaking, no version bump.

tests/lib/doc-consistency.test.mjs: +5 S33 pins (inventory split derived from
agents/; synthesis-agent dormant; orchestrator relabel; observability D2
rationale; analysis-doc D1–D3 record). Tests 739 (737 pass / 2 skip / 0 fail),
bar `node --test`; `claude plugin validate` green (1 accepted warning).

b4edc12bec fix(voyage): S23 — make /trekplan Phase 9 dedup executable (defect #1)
Phase 9's dedup hand-off was broken on two layers, both surfaced by the S22
dogfood: (a) plan-critic + scope-guardian (Read/Glob/Grep, no Write) were told
to write /tmp/*-out.json the dedup helper reads — they cannot; (b) even with
Write, their Output format emits markdown, not the helper's
{agent,findings:[{file,line,rule_key,text}]} schema. readJsonOrNull then
swallowed the absent files -> a silent empty merge that discarded every finding.

Fix (operator-chosen A'): keep the reviewers read-only; make the hand-off run.
- plan-review-dedup.mjs gains a --stdin mode reading {plan_critic,scope_guardian};
  malformed stdin exits non-zero so a broken hand-off surfaces loudly instead of
  collapsing into a silent empty merge. File mode + its tests are untouched.
- plan-critic.md + scope-guardian.md now emit a trailing machine-readable `json`
  findings block (the inline hand-off; no Write tool needed).
- trekplan.md + planning-orchestrator.md Phase 9 rewritten in lockstep: extract
  both blocks, pipe via heredoc into --stdin. No temp files, portable, no
  pathguard dependency.

TDD: malformed-stdin test failed first (CLI ignored stdin -> exit 0 = the bug),
green after impl. New S23 doc-pin asserts both docs use --stdin (not the dead
/tmp paths) and both agents declare the json block. Suite 724 (722/2/0); live
HEAD baseline was 720, not the stale 705 STATE carried forward.

04063fe60f docs(voyage): S20-fix — correct stale "Use Sonnet for all sub-agents" in 3 orchestrator docs
Follow-up to e7a58b0 (S20). Operator directive: errors found during a
task must always be fixed, not flagged out-of-scope. The three
orchestrator reference docs (review/research/planning) carried a
pre-pinning "Cost: Use Sonnet for all sub-agents" rule that contradicts
the operator-pinned all-opus reality (40d8742 — all 24 named agents are
model: opus, guarded by agent-frontmatter.test.mjs). The authoritative
command (trekresearch.md) was already correct.

Corrected all three to mirror trekresearch.md: sub-agents use their
pinned model: frontmatter (currently opus); a phase_signals model signal
or the active --profile (e.g. economy) overrides per-phase. Legitimate
sonnet uses (economy profile, low-effort path) are per-phase overrides,
not a blanket default — left untouched. The two "(Sonnet, Explore)"
ad-hoc follow-up/deep-dive hints are outside the 24-agent pinning and
align with the global "Sonnet for retrieval" model strategy — left as-is.

TDD: anti-false-claim doc-consistency pin written failing-first (3 hits)
in doc-consistency.test.mjs, then the prose fixed. Suite 713->714
(712 pass / 2 skip / 0 fail); census doc-pins 69->70, total 670->671.

eb5a7324a4 fix(voyage): S17 — downgrade NW2 bake-off verdict + drop orchestrator Agent-grants
Survivor #3 (bake-off data un-archived): per-run JSON a1..b3.json was never
committed (git diff-filter=A empty), so the medians/jaccard ladder cannot be
re-derived or audited. Commit-path would require fabricated data (forbidden) →
downgraded "POSITIVE" to "opt-in-defensible (single un-archived run)" in
docs/T2-bakeoff-results.md (verdict line + §5 header + new §Reproducibility
caveat) and the v5.5.0 CHANGELOG entry. New doc-consistency pin fails if the
doc re-asserts bare POSITIVE or drops the un-archived disclosure.

Survivor #5 (latent dispatch risk): planning/research/review orchestrators are
inline reference docs no command invokes, yet shipped tools:["Agent",...]. The
harness cannot spawn sub-agents with Agent, so the grant was pure latent risk.
Dropped "Agent" from all three frontmatters; flipped the agent-frontmatter
invariant (its canonical home) from "must include Agent" to "must NOT include
Agent". Removed the duplicate orchestrator pin from doc-consistency to avoid a
double guard (S19 anti-bloat).

Tests 690 → 691 (689 pass / 2 skip / 0 fail). claude plugin validate passes
(1 accepted CLAUDE.md-at-root warning).

3f77b68727 docs(voyage): correct now-false harness-constraint claims (W0/CC-01)
8 sites across 3 orchestrator agents + trekbrief/trekplan/trekresearch asserted 'the harness does not expose the Agent tool to sub-agents' as present fact -- the rationale for the v2.4.0 inline migration. CC 2.1.172 (verified) lets sub-agents spawn sub-agents up to 5 levels deep, so the claim is false. Replaced each with verified history (pre-2.1.172) + current fact + forward pointer to the decision matrix (W1/CC-26). Decision-neutral: states fact without pre-empting the orchestration redesign. CHANGELOG history left untouched. Tests 578/0/2; claude plugin validate passes.

40d8742712 chore(voyage): pin all sub-agents to Opus permanently (operator request)
Flip model: sonnet → model: opus across 20 agent files, 4 prose references
in commands (trekplan, trekresearch), trekendsession command frontmatter,
and CLAUDE.md tables. Aligns CLAUDE.md premium-profile row to actual
premium.yaml content (all-opus, which has been the case since v4.1.0 but
the doc was drift). Companion to VOYAGE_PROFILE=premium env-var (set in
~/.zshenv same day) — env-var governs orchestrator phase model; this
commit governs sub-agent models which are frontmatter-pinned and not
reachable by the profile resolver.

npm test: 516 pass, 0 fail, 2 skipped (unchanged from baseline).

Operator rationale: complete Opus coverage across all Voyage activity,
including the 20 sub-agents that the profile system does not control
(architecture-mapper, task-finder, plan-critic, scope-guardian,
brief-reviewer, code-correctness-reviewer, brief-conformance-reviewer,
review-coordinator, session-decomposer, plus the 6 researcher agents,
plus the 5 codebase-analysis agents).

Cost implication: sub-agent runs ~5x more expensive vs sonnet. Accepted.

3683ae0070 chore(voyage): release v5.0.0 — remove bespoke playground + /trekrevise + Handover 8; render produced artifacts to HTML + link, annotate via /playground
The v4.2/v4.3 bespoke playground SPA (~388 KB), the /trekrevise command,
Handover 8 (annotation → revision), the supporting lib/ modules
(anchor-parser, annotation-digest, markdown-write, revision-guard), the
Playwright e2e suite, and the @playwright/test / @axe-core/playwright
devDeps are removed. A browser walkthrough found the playground borderline
unusable, and it duplicated the official /playground plugin's
document-critique / diff-review templates.

In their place: scripts/render-artifact.mjs — a small, zero-dependency
renderer that turns a brief/plan/review .md into a self-contained,
design-system-styled, zero-network .html (frontmatter folded into a
<details> block). /trekbrief, /trekplan, and /trekreview call it on their
last step and print the file:// link; to annotate, run /playground
(document-critique) on the .md and paste the generated prompt back.

Resolves the v4.3.1-deferred findings as moot (their target files are
deleted). npm test green: 509 tests, 507 pass, 0 fail, 2 skipped.

1afa7e2c72 docs(ultraplan-local): complete external-architect doc rydding [skip-docs]
D2 of post-v3.4.1 stabilisering. Removes 14 plugin-name references from
agents/, commands/, and docs/ tracked files (CLAUDE.md/README.md/SECURITY.md
were ryddet in v3.4.1 commit 52cfc64).

The external architect plugin was moved out of the public marketplace
2026-05-04 due to ToS concerns around future skill sources. References in
prose are now stale or misleading for public users. The architecture/overview.md
filesystem slot remains available for any compatible producer — discovery
is plugin-agnostic via lib/validators/architecture-discovery.mjs (drift-WARN,
never drift-FAIL).

Files:
- agents/planning-orchestrator.md (1 ref generalized)
- commands/ultraplan-local.md (2 refs generalized; missed by prompt inventory)
- docs/HANDOVER-CONTRACTS.md (4 refs generalized; Handover 3 + stability summary)
- docs/architect-bridge-test.md (deleted; was a public-only bridge checklist)
- docs/subagent-delegation-audit.md (5 refs/rows removed; intervention #5 dropped, recommendation adjusted)

CHANGELOG.md retains historical references (20 occurrences) intentionally.

Verification:
- grep tracked non-CHANGELOG md: 0 references remaining
- npm test: 361/361 pass (baseline preserved)

3a365561cb feat(ultraplan-local): parallelize Phase 9 review with inline dedup
Strengthen single-message reinforcement for plan-critic + scope-guardian
parallel dispatch in commands/ultraplan-local.md Phase 9 and mirror in
agents/planning-orchestrator.md Phase 6. Reviewers now write structured JSON
to /tmp/{plan-critic,scope-guardian}-out.json which is merged via the
lib/review/plan-review-dedup.mjs CLI shim from S8.

The merged set lets us revise the plan once for duplicate findings instead
of twice. Source: research/05 R1 + R2.

Pin in tests/lib/doc-consistency.test.mjs locks both files against
single-message + dedup-helper regressions.

02e2fe9c9d feat(graceful-handoff): 2.0 — migrate to skills/ with disable-model-invocation [skip-docs]
Step 1 of v2.0 plan. Hard cut from commands/ to skills/ per Anthropic
recommendation for new plugins. Frontmatter sets disable-model-invocation:
true and pins model: claude-sonnet-4-6. Docs (README, CLAUDE.md, root
README) deferred to Step 9 per plan.

1e81334bd9 feat(ultraplan-local)!: v2.4.0 — orchestrator agents as inline reference
Redefine research-orchestrator, planning-orchestrator, and
architect-orchestrator from "background executor" to "inline
reference documentation". The agent files remain as the canonical
workflow descriptions, but the /ultra* commands now execute the
phases directly in the main command context instead of spawning
these agents as sub-agents.

The /ultra* command markdowns are now the de-facto orchestrators.
Splitting work into a separate sub-agent was incompatible with the
harness's treatment of the Agent tool (not exposed to sub-agents).

BREAKING CHANGE: These agents are no longer invoked. Any external
integration that spawned them directly should now invoke the
corresponding /ultra* command instead.

Extract interview from /ultraplan-local into new /ultrabrief-local command.
/ultraplan-local now requires --brief or --project (breaking). All pipeline
artifacts land in one project directory: .claude/projects/{date}-{slug}/
with brief.md, research/, plan.md, sessions/, progress.json.

Breaking changes:
- /ultraplan-local requires --brief <path> or --project <dir>
- /ultraplan-local --spec removed (convert specs to briefs per MIGRATION.md)
- Interview phase moved to /ultrabrief-local
- spec-reviewer renamed to brief-reviewer with 5th dimension (Research Plan validity)

- /ultrabrief-local command (interactive interview → brief.md with research plan)
- templates/ultrabrief-template.md (task brief format with intent + research plan)

2e84787c53 feat(ultraplan-local): v1.8.0 — close Opus 4.7 schema-drift gap
Opus 4.7 reads agent instructions more literally than 4.6. The v1.7
planning-orchestrator described the Step+Manifest schema via prose +
procedural rules, which 4.6 inferred correctly but 4.7 sometimes
rendered as narrative "Fase N" prose — producing plans ultraexecute
Phase 2 rejected. First observed 2026-04-17 during llm-security v6.2.0
planning.

v1.8.0 closes the gap:

- planning-orchestrator Phase 5 embeds a literal copyable Step+Manifest
  example (JWT middleware) replacing "read the template" prose
- Explicit forbidden-format clause: ## Fase N, ### Phase N, ### Stage N,
  and any non-"### Step N:" heading are denied
- Phase 5.5 schema self-check: grep-verify canonical Step count matches
  Manifest count and narrative heading count is zero, before handing to
  plan-critic
- ultraexecute-local --validate mode: schema-only check that parses
  steps + manifests, reports READY/FAIL with actionable error hints,
  no security scan, no execution. Fast sanity check between
  /ultraplan-local and full execution.

Static verification: 17/17 PASS.

9369acc3b8 feat(ultraplan-local): v1.7.0 — self-verifying plan chain
Wave 1 of a 6-session parallel build revealed three failure modes:
(1) hallucinated completion (status=completed after 2/5 steps, last
tool call was an arbitrary file review), (2) fail-late bash (3/6
sessions had push blocked inside sub-agent sandbox after all work
was done), (3) no objective verification (plans were prose).

v1.7 closes all three by making the plan an executable contract.

Per-step YAML manifest (expected_paths, commit_message_pattern,
bash_syntax_check, forbidden_paths, must_contain) is the objective
completion predicate. Plan-critic dimension 10 (Manifest quality)
is a hard gate. Session decomposer propagates manifests verbatim
and emits an obligatory Step 0 pre-flight (git push --dry-run,
exit 77 sentinel) in every session spec.

ultraexecute-local gets Phase 7.5 (independent manifest audit from
git log + filesystem, ignoring agent bookkeeping) and Phase 7.6
(bounded recovery dispatch, recovery_depth ≤ 2). Hard Rule 17
forbids marking a step passed without manifest verification. Hard
Rule 18 forbids ending on an arbitrary tool call before reporting.

Division of labor is made explicit:
- /ultraresearch-local gathers context (no build decisions)
- /ultraplan-local produces an executable contract (manifests,
  plan-critic gate)
- /ultraexecute-local executes disciplined (does NOT compensate
  for weak plans — escalates)

Code complete. Docs partial (Arbeidsdeling table + manifest section
added to plugin + marketplace READMEs). Verification tests
(10-sequence) pending — see REMEMBER.md.

Backward compat: v1.6 plans without plan_version marker get
legacy mode with synthesized manifests and legacy_plan: true in
progress file. Plan-critic emits advisory, not block.

4679f4556d feat(ultraplan-local): v1.6.0 — /ultraresearch-local deep research command
Add /ultraresearch-local for structured research combining local codebase
analysis with external knowledge via parallel agent swarms. Produces research
briefs with triangulation, confidence ratings, and source quality assessment.

New command: /ultraresearch-local with modes --quick, --local, --external, --fg.
New agents: research-orchestrator (opus), docs-researcher, community-researcher,
security-researcher, contrarian-researcher, gemini-bridge (all sonnet).
New template: research-brief-template.md.

Integration: --research flag in /ultraplan-local accepts pre-built research
briefs (up to 3), enriches the interview and exploration phases. Planning
orchestrator cross-references brief findings during synthesis.

Design principle: Context Engineering — right information to right agent at
right time. Research briefs are structured artifacts in the pipeline:
ultraresearch → brief → ultraplan --research → plan → ultraexecute.
