fix(verification): a backticked span is only run when it IS a command
"The first backtick span is the command" is right for a plan, whose template
puts the command first, and wrong for a brief, whose criterion usually opens
by NAMING the thing under discussion. Measured 2026-09-18 on the repo's own
example brief: 5 of 6 criteria FAILED, 3 of them parse artifacts - `--verbose`
run as a command gave exit 2 ("invalid option"), `tests/` gave exit 126 ("is a
directory"). The rubric reads a FAILED result as decisive, so each one became
a BROKEN_SUCCESS_CRITERION BLOCKER about prose.

looksLikeCommand() screens the span by SHAPE only - no filesystem lookup, so a
span parses the same everywhere. Refused: a leading flag, a directory, a token
carrying quotes/braces/prose, and a lone relative path with a slash (an
explicit ./, ../, / or ~/ still runs, as do env-var prefixes). A refused span
is `unrunnable` with reason `not-a-command` - its own outcome, never FAILED,
and it never reaches a shell.

It deliberately does NOT scan on to a later span. "The first span that LOOKS
like a command" invents commands out of prose: in that same example brief it
would have run `whoami` and `login`, two real binaries a sentence happens to
name. An absent measurement is honest; a guessed one is not.

The shape check applies to prose spans only. Inside a shell-tagged fence the
author has already declared shell, so `[ -f x ] || exit 1` still runs.

The rubric follows: a NOT RUN result is never on its own a finding. The
Partial row now describes half-built DELIVERED CODE, and the reviewer gets a
table of the three reason strings - no-command, placeholder, not-a-command -
with what each says about the sentence rather than about the code.

Not covered, stated for the record: a multi-token span whose first token is a
non-executable file (`tests/golden/login.stdout --check`) still runs, and a
criterion whose command is real but whose binary is absent still reports the
shell's exit 127 - that is a true measurement of a missing binary, not a
parse artifact.

Red first: 4 runner tests + 1 doc-consistency pin failed before this change.

fix(review): run the success-criteria commands and hand the reviewer the result (D-04)
The rubric required `brief-conformance-reviewer` to classify a Success
Criterion as Full only when "its verification command/test exists and passes".
Its tools are `Read`, `Glob`, `Grep`. It cannot run anything, so "passes" was
either guessed from the command's mere existence or quietly downgraded to
"exists" — a BLOCKER-tier rule key resting on an impression.

The reviewer stays read-only — a reviewer that executes the code it reviews is
not an independent reviewer. The command does the running instead:

- `/trekreview` Phase 4.5 runs the brief's `## Success Criteria` commands
through `lib/verification/criteria-runner.mjs --brief --evidence` and captures
the block as `sc_evidence_block`, pasted verbatim into the reviewer prompt in
Phase 5. The exit code does not stop the review — a failing criterion is
exactly what the review exists to find.
- `formatCriteriaEvidence` builds that block in code: one row per criterion with
the command, the exit code and the first output line. Chose a code-built block
over an orchestrator-written summary so the orchestrator cannot narrate a pass
  that never happened.
- The rubric now judges the supplied result: `PASS` supports Full, `FAILED` /
`BLOCKED` is `Broken` with the exit code cited, and `NOT RUN` is the absence
of a measurement — never evidence in either direction.
- Phase 4.5 is skipped in `quick` mode: that mode does not launch the
conformance reviewer, so there is nobody to hand the result to.

Red first: seven tests in `tests/lib/criteria-runner.test.mjs` against a
committed brief fixture whose three criteria pass, fail, and are prose-only.
The two doc pins were verified red against the pre-fix files (rubric asked
"exists and passes"; no Phase 4.5; the block reached nobody).

Suite: 1117 (1115/0/2), up 9.

529d2db852 fix(trekplan): drop TaskCreate/TaskUpdate from the tool lists - measured dead weight
Claude Code 2.1.233 stopped offering the task/todo tools on Opus 4.8, Sonnet 5, Fable 5
and newer unless CLAUDE_CODE_ENABLE_TODO_TOOLS=1 is set. /trekplan still listed
TaskCreate and TaskUpdate in allowed-tools. So did the two orchestrator reference docs,
which mirror the command but are never spawned. No prose in any command calls either tool.

Runtime effect, measured BEFORE removal on CC 2.1.274, claude-opus-5, with
CLAUDE_CODE_ENABLE_TODO_TOOLS unset:
- Probe: a throwaway project command whose frontmatter is
`allowed-tools: Read, TaskCreate, TaskUpdate`, run headless
  (`claude -p --output-format stream-json --verbose`).
- The session had 151 tools. The only Task* names among them were Task, TaskOutput and
TaskStop; neither TaskCreate nor TaskUpdate was there.
- The command loaded (listed in slash_commands) and completed with subtype=success,
is_error=false and 0 tool calls. The model reported the tool as not available.
- `claude plugin validate` does not flag the unknown names either: only the one
accepted warning appears, before and after.
Verdict: harmless dead weight, no error anywhere, so this is a correctness-of-docs fix
rather than a runtime fix.

End-state gate D-05: open -> closed (defects 5 -> 4 of 7). Suite 1059 (1057/0/2), run

f6c3535cea fix(trekplan): drop the "execute with team" path - TeamCreate/TeamDelete no longer exist
Claude Code 2.1.178 removed the TeamCreate and TeamDelete tools, so /trekplan's
"execute with team" handoff could never create a team: it always ended in its own
fallback (sequential execution). The path is removed and the fallback is now the only
behaviour. The handoff no longer offers "execute with team"; if a user asks for it
anyway, /trekplan says there is no team path and executes sequentially. Parallel
implementation stays where it already was: --decompose plus /trekexecute's
multi-session waves. No new orchestration replaces it.

- commands/trekplan.md: TeamCreate/TeamDelete removed from allowed-tools; the offered
option and the team section are gone.
- agents/planning-orchestrator.md (reference doc): the offered option is gone.
- docs/cc-upgrade-2.1.181-decision-matrix.md still names the team path: a historical
  record, left as written.

End-state gate D-01: open -> closed (defects 7 -> 6 of 7). Suite 1059 (1057/0/2), run
on a clean export of the index.

1ca48e0cd0 release(v5.10.1): drop gemini-bridge from the pipeline; correct the T1 §6 PoC status
gemini-bridge is dropped by operator directive (three repetitions). The drop is
permanent, not a wait-for-SDK-upgrade state, so the change clears every LIVE
surface that could still steer a run toward the agent and leaves the historical
record alone.

Live surfaces cleared: agents/gemini-bridge.md deleted; trekresearch (bridge
launch block, --local help, high-effort always-on pair, stats record,
degradation list), trekplan, trekbrief, research-orchestrator (mode table,
agent table, prompting block, summary line), README (feature prose, mode table,
agent prose, mermaid EXTERNAL node, conditional legend, optional-MCP
requirement, --local section), CLAUDE.md, settings.json (the whole
trekresearch.geminiBridge block), both templates, architecture.md,
command-modes.md.

trekplan high-effort Adversarial Pass 2 now degrades EXPLICITLY: it emits its
section with status "unavailable, skipped" instead of failing or vanishing. A
high-effort plan carrying no Pass 2 marker is indistinguishable from one whose
Pass 2 crashed, which is the failure mode this wording exists to prevent.

gemini_used is deliberately KEPT as a vestigial trekresearch stats field pinned
to false. Removing it would break the observability export schema for existing
consumers, and the directive was about the agent, not the field.

Not touched: CHANGELOG history and the measurement/decision docs keep their
gemini references. They record what a past version did or what was measured
then; rewriting them is the same defect class as bumping a version string
inside a measurement doc.

Driven test-first. Five new pins in tests/lib/doc-consistency.test.mjs, verified
RED before the edits, including a KNOWN-POSITIVE CONTROL asserting the
historical records still DO carry gemini references — so the empty result on
live surfaces is a measurement and not a broken query (Verifiseringsloven
ansikt 4). Agent inventory 24 -> 23 (20 spawnable + 3 orchestrator reference
docs); the <example>-block floor moves 34 -> 32 because an agent legitimately
left the inventory, not because examples went missing from a surviving one.

Docs: docs/T1-cc26-delegated-orchestration.md §8 item 3 claimed both the §6
synthesis-agent PoC and the §5 bake-off were "designed but unbuilt". That was
written in S7 and falsified the same afternoon by S12, which ran the §6 PoC and
recorded Δ main-context (faithful flow) = 0.0%, NEGATIVE. The stale wording is
what caused the settled PoC to be re-ordered as new work on 2026-09-02, so it is
struck rather than deleted and §6 gained a RUN AND DECLINED status block. The
finding is structural, not stochastic: Phase 5 spawns the exploration swarm
foreground (trekplan.md:158,338-341), so the outputs are already resident in
main before Phase 7 — delegating only the Phase-7 digest evicts nothing.

Also measured 2026-09-03 (CC 2.1.259): claude -p --output-format stream-json
runs on subscription auth with no ANTHROPIC_API_KEY and now emits a
subagent_stats block, so S12's environment-block premise is half stale. Recorded
in §8 item 4. It lowers the cost of §5; it changes nothing about §6.

20cdc22803 fix(review): an anonymous invalid payload is unattributable, not a reviewer named "unnamed reviewer"
Follow-up defect in the reviewer accounting added by e2aec01, found by review
and confirmed by probe before fixing.

validateFindings only WARNS on a missing `reviewer` field, so a payload can
fail schema while carrying no name. ingest then records `reviewer: null`, and
runContract turned that null into the literal reviewer name "unnamed reviewer".

MEASURED before the fix:
  runContract([{findings:[{file:'x.mjs',line:1,rule_key:'NOPE',severity:'MAJOR'}]}],
              {expectedReviewers:['code-correctness-reviewer']})
  -> missing_reviewers = ["unnamed reviewer", "code-correctness-reviewer"]
One failure, two entries, one of them an agent nobody launched. The
`reported.delete(s.reviewer)` line was also inert for that case, since a null
name was never in the set to begin with.

Fix: skipped payloads are split by whether they carry a name. Named ones go to
missing_reviewers as before; anonymous ones increment the new
`unattributable_payloads` count, which forbids ALLOW on its own - so stripping
a reviewer name from a payload cannot restore ALLOW, and the floor does not
depend on the caller passing expectedReviewers. `allow_blocked_by` reports the
two facts separately: `missing-reviewer:<name>` and `unattributable-payload (n)`.

The old behaviour never produced a false ALLOW - it failed in the safe
direction - but it named a reviewer that did not exist, which is the kind of
output an operator would chase.

Iron Law: two failing tests first (double entry; anonymous-payload-alone must
forbid ALLOW), then the fix.

Also verified in this pass, by temporarily adding a fake reason to
UNVERIFIED_REASONS: the prose-vocabulary pin does go red when a reason is
declared in the lib but missing from agents/review-coordinator.md. A pin that
cannot fail is not a pin.

Suite 1034 (1032/0/2) -> 1036 (1034/0/2), 0 failures.

e2aec019ac fix(review): fail-closed verdicts - an unsubstantiated finding can no longer yield ALLOW
computeVerdict counted only the findings handed to it (reasoned.kept), so a
finding removed by Pass 2 or Pass 3, and a reviewer whose payload was thrown
away or never arrived, were arithmetically identical to a finding that never
existed. All three pushed the verdict toward ALLOW.

Measured before the fix (probes, 2026-09-01):
  - a BLOCKER with a 101-character title -> ALLOW (Pass 2 succinctness)
  - a payload with one ad-hoc rule_key is skipped WHOLE at ingest, taking a
valid BLOCKER sibling with it -> ALLOW
  - a reviewer that never reported -> ALLOW
Pass 3's own no-citation / unknown-rule_key branches turned out unreachable
through runContract (validateFindings rejects those payloads first), so the
reachable exposure was Pass 2 plus the skipped/absent reviewer.

THE OPEN DESIGN DECISION, and why it went against the order's default.
The order proposed: indeterminate file-existence YES, plain succinctness NO
("a too-long finding is not an uncertain finding"). I kept the first and
overrode the second, on one principle:

A removal is `dropped` only when the test REFUTED the finding as a claim
about this codebase. Every other removal is `unverified`.

Succinctness and actionability read a `.length`. They never examine the claim,
so they cannot establish the finding is unreal - and dropping a BLOCKER for a
101-character title is precisely the fail-open shape being fixed. Three things
settled it:

1. Under the order's default the fix would have been almost inert. Pass 3's
drop branches are unreachable via runContract, so leaving Pass 2 out would
have left the only reachable finding-level exposure open.
2. Cost asymmetry, priced rather than asserted: the verdict is not a gate.
   Handover 6 feeds `findings` filtered to BLOCKER+MAJOR into /trekplan
   (commands/trekplan.md:218); `verdict` is optional metadata
(docs/HANDOVER-CONTRACTS.md:353). Nothing loops or re-plans on WARN. So a
false `unverified` costs WARN plus a printed reason; a false drop costs a
silent ALLOW over a live BLOCKER.
3. unknown-rule_key joins them for the same reason: an ad-hoc key is a real
defect wearing the wrong label, and v5.1.1 high-effort mode already KEEPS
those, normalised to PLAN_EXECUTE_DRIFT. Refuting them at normal effort
while keeping them at high effort would be incoherent.

no-citation stays a drop: a finding whose file is empty or whose line is
negative names no location, so it makes no checkable claim at all - the one
deterministic refutation, and what the Pass 3 prose already said it was.

Iron Law: tests/lib/coordinator-contract.test.mjs first, red (missing export +
the three measured ALLOWs), then production code. Two existing assertions were
updated AFTER implementation as contract changes, not to make the red pass.
A known-positive control pins that ALLOW is still reachable - without it,
"no ALLOW" is not a fail-closed contract, only a broken one.

lib/review/coordinator-contract.mjs
  + classifySuppression / REFUTING_REASONS / UNVERIFIED_REASONS - one
vocabulary owned by the lib, including the tokens only the LLM
    coordinator emits (accuracy:refuted, file-existence:refuted/indeterminate),
so prose and lib cannot drift. Unclassified reasons default to unverified:
    the default fails closed.
  ~ judgeFilter / reasonablenessFilter return {kept, dropped, unverified}
  ~ computeVerdict(findings, {unverified, missingReviewers}) -> + allow_blocked_by.
Never raises a verdict, only withholds ALLOW. Unverified findings are NOT
counted into a severity tier: their severity was never substantiated, and
    counting it would be invention.
  ~ runContract(payloads, {expectedReviewers}) -> + unverified,
    missing_reviewers, allow_blocked_by. `suppressed` stays the union of
dropped + unverified, so existing consumers (gold-eval) keep their meaning.

agents/review-coordinator.md - Pass 2/3 tables gain a fate column, new
"Suppression is two-valued" section, Pass 4 threshold table gains the two
fail-closed rows, Executive Summary must state a withheld ALLOW, Suppressed
Findings tags each line [dropped]/[unverified]. Pass 3's unknown-rule_key
bullet explicitly says high-effort does not reach that branch, so the same
input never has two documented fates.

commands/trekreview.md - Phase 5 "Reviewer accounting": the expected set is
written down before the spawn, a silent reviewer gets one re-ask and then
STOP. That extends the pattern already in the file (schema failure -> 2
bounded re-asks -> "do not feed unvalidated findings to the coordinator") to
the other two ways a reviewer goes missing, rather than softening it to WARN.
The lib's missing_reviewers stays as belt-and-braces for direct callers.

docs/agent-return-channel-defect.md - the "inferred, not observed" caveat on
the unnamed arm above 66 lines is struck: akashic-intelligence S27
(f168630) measured 2/2 unnamed agents returning against a 4370-line plan,
30449 B and 10989 B, both valid JSON. Recorded with akashic's own two
caveats intact - the measurer owns the finding, and byte-identity between
the returned string and the file on disk was not proven. The separate S25
named-arm figures are left standing; these are two measurements, not a
correction of one by the other.

No release, no version bump, no tag, no catalogue ref, no Workflow port.
Suite 1025 (1023/0/2) -> 1034 (1032/0/2), 0 failures.

2e352a7dbb fix(trekresearch): gate Phase 4.5 on VOYAGE_STORM_ENABLED, not effort alone
CLAUDE.md claimed both STORM phases go inert when the flag is unset. Only
Phase 5 did: the flag check lives in research-loop-cap.mjs, and Phase 4.5
never invokes the cap — it was gated on effort: high alone. At high effort
with the flag unset, discovery still mined Phase-4 output and mutated the
dimension list, so `dimensions` diverged from `dimensions_baseline` and the
decline branch was unreachable for half the mechanism.

The code was the deviator, so the guard is fixed rather than the claim: the
Phase 4.5 skip-guard now names both conditions, with the reason inline.

Three surfaces scoped the flag to "the loop" and are corrected with it
(README, docs/command-modes, docs/architecture), plus the orchestrator phase
map. CLAUDE.md's claim is now true, but its stated MECHANISM was not — Phase
4.5's inertness comes from its own guard, not from the cap module — so that
sentence is corrected too.

New doc-consistency pin: the flag must be documented as gating both phases on
all four reference surfaces. Review finding 00a3af1a.

f8d9d7fef9 chore(voyage): release v5.6.1 — one-line descriptions for reference/dormant agents (~700 tok trim)
Trim the always-loaded token cost of the agent listing Claude Code injects
into every session. The three *-orchestrator reference docs
(planning/research/review) and the dormant synthesis-agent carried
multi-paragraph description: frontmatter (full rationale + CC-2.1.172
history + a usage example) despite never being spawnable from the live
/trek* pipeline; their description is now a single line, with the full
rationale already living — and remaining — in each file's body. ~700
tokens trimmed, no behavior change.

The three orchestrators retain the self-declaration "reference document,
not a spawnable capability" (doc-consistency pin); synthesis-agent retains
its DORMANT / not-wired flag + docs/T1-synthesis-poc-results.md pointer.
Bump plugin.json, package.json, package-lock.json, and the README badge to
5.6.1; prepend CHANGELOG v5.6.1 entry + README What's-new note. Surfaced
via config-audit always-loaded token-audit dogfooding.

Additive — no breaking change, no runtime behavior change. Canonical
node --test: 739 pass, 0 fail (2 skipped); version-consistency + agent
inventory + frontmatter pins green.

2df0cbb372 docs(voyage): S33 — reconcile agent inventory (21 spawnable + 3 reference docs) + record D1–D3 considered-and-kept
Balance-backlog S33 (DOC, non-breaking). The D1–D3 forks resolved conservatively
(balance-backlog-plan.md), collapsing the model + observability work into a
documentation record. Three deliverables, all doc-only, no code/frontmatter change:

- V35 (doc half): the "24 agents" headline is reconciled to its honest split —
  21 spawnable (one dormant: synthesis-agent, Δ≈0) + 3 orchestrator reference
docs (planning-/research-/review-orchestrator document the inline /trek*
workflow; not spawned). Each orchestrator header now declares itself a
"reference document, not a spawnable capability". README + CLAUDE.md state the
split; counts in the new pins are DERIVED from agents/ so they survive reword.
- D2 (V32 rationale): docs/observability.md gains a "Why direct export rather
than a native collector" section — direct export keeps the path / SSRF /
field-allowlist guards in audited in-process code (the S21 hardening) instead
of re-hosting a collector; textfile mode remains the collector escape hatch.
- D3 (kept-opus): docs/voyage-vs-cc-balance-analysis.md §10 decision record —
  opus on V09 (glue), V35 (dormant), V11 (retrieval), V16 (mechanical), V08
(researchers) was reconsidered for sonnet and KEPT (pin 40d8742 firm).

No agent frontmatter changed — tests/lib/agent-frontmatter.test.mjs is the
structural model source-of-truth and is untouched (diff is description-only:
model: opus + tools lists unchanged, no Agent tool granted). No Handover-1
change; no exporter/gemini-bridge removal. Non-breaking, no version bump.

tests/lib/doc-consistency.test.mjs: +5 S33 pins (inventory split derived from
agents/; synthesis-agent dormant; orchestrator relabel; observability D2
rationale; analysis-doc D1–D3 record). Tests 739 (737 pass / 2 skip / 0 fail),
bar `node --test`; `claude plugin validate` green (1 accepted warning).

b4edc12bec fix(voyage): S23 — make /trekplan Phase 9 dedup executable (defect #1)
Phase 9's dedup hand-off was broken on two layers, both surfaced by the S22
dogfood: (a) plan-critic + scope-guardian (Read/Glob/Grep, no Write) were told
to write /tmp/*-out.json the dedup helper reads — they cannot; (b) even with
Write, their Output format emits markdown, not the helper's
{agent,findings:[{file,line,rule_key,text}]} schema. readJsonOrNull then
swallowed the absent files -> a silent empty merge that discarded every finding.

Fix (operator-chosen A'): keep the reviewers read-only; make the hand-off run.
- plan-review-dedup.mjs gains a --stdin mode reading {plan_critic,scope_guardian};
malformed stdin exits non-zero so a broken hand-off surfaces loudly instead of
collapsing into a silent empty merge. File mode + its tests are untouched.
- plan-critic.md + scope-guardian.md now emit a trailing machine-readable `json`
findings block (the inline hand-off; no Write tool needed).
- trekplan.md + planning-orchestrator.md Phase 9 rewritten in lockstep: extract
both blocks, pipe via heredoc into --stdin. No temp files, portable, no
  pathguard dependency.

TDD: malformed-stdin test failed first (CLI ignored stdin -> exit 0 = the bug),
green after impl. New S23 doc-pin asserts both docs use --stdin (not the dead
/tmp paths) and both agents declare the json block. Suite 724 (722/2/0); live
HEAD baseline was 720, not the stale 705 STATE carried forward.

04063fe60f docs(voyage): S20-fix — correct stale "Use Sonnet for all sub-agents" in 3 orchestrator docs
Follow-up to e7a58b0 (S20). Operator directive: errors found during a
task must always be fixed, not flagged out-of-scope. The three
orchestrator reference docs (review/research/planning) carried a
pre-pinning "Cost: Use Sonnet for all sub-agents" rule that contradicts
the operator-pinned all-opus reality (40d8742 — all 24 named agents are
model: opus, guarded by agent-frontmatter.test.mjs). The authoritative
command (trekresearch.md) was already correct.

Corrected all three to mirror trekresearch.md: sub-agents use their
pinned model: frontmatter (currently opus); a phase_signals model signal
or the active --profile (e.g. economy) overrides per-phase. Legitimate
sonnet uses (economy profile, low-effort path) are per-phase overrides,
not a blanket default — left untouched. The two "(Sonnet, Explore)"
ad-hoc follow-up/deep-dive hints are outside the 24-agent pinning and
align with the global "Sonnet for retrieval" model strategy — left as-is.

TDD: anti-false-claim doc-consistency pin written failing-first (3 hits)
in doc-consistency.test.mjs, then the prose fixed. Suite 713->714
(712 pass / 2 skip / 0 fail); census doc-pins 69->70, total 670->671.

987e847ea1 feat(voyage): S18 — framing-hardening (min-version gate + memory status + pre-2.2 doc + flagship hedge)
Closes devils-advocate audit #4–#6 (MAJOR×MED). Four additive, non-breaking
changes to the v5.5 framing-alignment machinery:

1. --min-brief-version <ver> gate (audit #4). Opt-in version floor on
   /trekplan + /trekresearch, forwarded to brief-validator as --min-version.
New opts.minBriefVersion warns BRIEF_VERSION_BELOW_MINIMUM (never blocks)
when a brief declares a version below the floor; trekreview exempt; absent
opt = no check. CLI shim parses --min-version and skips its value token in
   filePath detection.

2. memory_alignment.status field (audit #5). brief-reviewer now emits
   status: verified | n_a | contradictions so a score-5 N/A (no memory) is
   distinguishable from a score-5 verified-aligned brief — the score≥4 gate
passes in both, status reveals whether the wrong-premise defense ran.

3. Document pre-2.2 = zero framing enforcement (audit #4). HANDOVER-CONTRACTS
§Handover 1 now states the producer-elective hole + two remedies. Also
fixes a stale "current is 2.1" line (current is 2.2).

4. Soften flagship overselling (audit #6). CLAUDE.md Context-Engineering
principle now hedges that main-context relief is asserted-by-design, not
measured (T1 PoC found Δ≈0); README carried no false claim to fix.

TDD: 8 new tests written failing-first (5 validator, 1 trekbrief status pin,
2 doc-consistency cross-file pins). Suite 691→699 (697 pass / 2 skip / 0 fail).
No flagship prose-pin added (deliberate, per S19 anti-bloat).

eb5a7324a4 fix(voyage): S17 — downgrade NW2 bake-off verdict + drop orchestrator Agent-grants
Survivor #3 (bake-off data un-archived): per-run JSON a1..b3.json was never
committed (git diff-filter=A empty), so the medians/jaccard ladder cannot be
re-derived or audited. Commit-path would require fabricated data (forbidden) →
downgraded "POSITIVE" to "opt-in-defensible (single un-archived run)" in
docs/T2-bakeoff-results.md (verdict line + §5 header + new §Reproducibility
caveat) and the v5.5.0 CHANGELOG entry. New doc-consistency pin fails if the
doc re-asserts bare POSITIVE or drops the un-archived disclosure.

Survivor #5 (latent dispatch risk): planning/research/review orchestrators are
inline reference docs no command invokes, yet shipped tools:["Agent",...]. The
harness cannot spawn sub-agents with Agent, so the grant was pure latent risk.
Dropped "Agent" from all three frontmatters; flipped the agent-frontmatter
invariant (its canonical home) from "must include Agent" to "must NOT include
Agent". Removed the duplicate orchestrator pin from doc-consistency to avoid a
double guard (S19 anti-bloat).

Tests 690 → 691 (689 pass / 2 skip / 0 fail). claude plugin validate passes
(1 accepted CLAUDE.md-at-root warning).

6b30483304 feat(voyage): S12 — NW3 synthesis-agent built + measured → declined per measurement [skip-docs]
NW3 (CC-26 §6 PoC): delegate trekplan Phase 7 synthesis to a synthesis-agent,
adopt only if Δ main-context ≥30% with no quality loss. Operator chose the
deterministic-proof path (live ≥3-run bake-off was env-blocked: no API key;
installed plugin is a cache copy so a new agent is invisible to `claude -p`).

Decisive structural finding: trekplan Phase 5 runs the swarm FOREGROUND, so its
outputs are already resident in main before Phase 7. Delegating only Phase 7
evicts nothing → Δ_faithful = 0% (BASE-independent). The ≥30% saving needs an
out-of-scope Phase-5 redesign (swarm-writes-to-disk / nested orchestrator).
VERDICT: DECLINED per measurement.

- agents/synthesis-agent.md — dormant, schema-conformant deliverable (NOT wired)
- lib/plan/synthesis-digest-schema.mjs — digest output contract (+ tests)
- scripts/synthesis-measure.mjs — deterministic Δ-accounting core (+ tests)
- tests/fixtures/synthesis/ — 7 exploration outputs + representative digest
- docs/T1-synthesis-poc-results.md — measurement + verdict (reproducible)
- CLAUDE.md — agent table row (doc-consistency: 24 agents)

Tests 670 → 695 (693 pass / 2 skip / 0 fail). `claude plugin validate` clean
(only the pre-existing root-CLAUDE.md warning). commands/trekplan.md untouched.

[skip-docs] rationale: no user-facing feature ships (NW3 declined; agent dormant
and unwired). The substantive doc is docs/T1-synthesis-poc-results.md; the
README/CHANGELOG roll-up for NW1–NW3 is the S13 coordinated release per
docs/W1-narrow-wins-plan.md §S13.

736ae55d66 feat(voyage): S6 — v5.5 brief framing enforcement (brief_version 2.2)
Implements the CLAUDE.md cross-cutting invariant "brief framing must match
operator intent" as a controlled brief_version 2.1->2.2 bump (operator option A1).
Three defense layers, version-gated at >=2.2 so existing 2.0/2.1 briefs stay
valid (forward + backward compatible), mirroring the phase_signals >=2.1 gate:

- L1 framing: enum field (preserve|refine|replace|new-direction). Enum-checked
on any version when present (BRIEF_INVALID_FRAMING); missing at >=2.2 ->
BRIEF_MISSING_FRAMING. /trekbrief Phase 2.5 collects it BEFORE any brief prose
  (non-skippable, even in --quick).
- L2 memory alignment: new brief-reviewer dimension 6 comparing brief Intent/Goal
+ framing against operator memory for explicit contradictions; degrades to
score 5 (N/A) when no memory context is supplied. Wired into Phase 4e gate
  (memory_alignment.score >= 4).
- L3 obligatory ## TL;DR (<=5 content lines) at >=2.2; soft cap ->
  BRIEF_TLDR_TOO_LONG warning.

trekreview briefs are exempt from the framing/TL;DR gate. Handover 1 PUBLIC
CONTRACT doc, README "What's new", and the CLAUDE.md invariant + agents table
(brief-reviewer 5->6 dimensions) updated to 2.2 (schema axis only; plugin
version badge + CHANGELOG remain S10).

Iron Law followed: validator tests red->green first. Tests 586 -> 606
(+20, 604 pass / 2 skip). claude plugin validate passes (pre-existing
CLAUDE.md root-context warning unchanged).

fdd3ad80d7 feat(voyage): W2 impl (S4) — gate resolver model, adopt native effort:, doc-truth
S4 of the 2.1.181 upgrade — implementation, not a gate. TDD: failing test
written first for the resolver gate, then the fix; suite green throughout.

- Resolver MAJOR (FIX): lib/profiles/phase-signal-resolver.mjs now imports
  BASE_ALLOWED_MODELS from profile-validator and gates `model`
(if 'model' in entry && BASE_ALLOWED_MODELS.includes(entry.model)),
mirroring the EFFORT_LEVELS gate one line up. Out-of-allowlist models
(gpt-4, haiku) are dropped instead of handed to an agent spawn —
  defense-in-depth behind brief-validator's validation-time check. No
circular import (brief-validator already imports the same symbol).
  +2 tests (drops-invalid / keeps-valid).
- Native effort: (SHIP, static additive): effort: frontmatter on 8 agents —
  retrieval (task-finder, git-historian, dependency-tracer,
  architecture-mapper) = medium; adversarial-reasoning (plan-critic,
  risk-assessor, contrarian-researcher, review-coordinator) = high. The
other 15 stay unset -> inherit Opus-4.8 default (high). This per-spawn
REASONING effort is a different axis from brief phase_signals.effort
(ORCHESTRATION shape) per the S3 decision.
- Doc-truth + axis distinction: new canonical docs/profiles.md
  §Model & effort axes (opus->Opus 4.8 default-high; orchestration vs
reasoning effort table; native-effort precedence; per-agent levels).
Short notes in CLAUDE.md (after Agents table) and README.md (Cost
  profile), both pointing to profiles.md.
- Open (non-blocking, unchanged): only STATIC effort shipped — the
verified-safe minimum. Profile-driven DYNAMIC effort still needs
verification of the per-spawn effort param or env-var injection.

Matrix: new "S4 resolutions" section. Tests 582 total / 580 pass / 0 fail /
2 skip (was 578 pass; +2). claude plugin validate passes (only pre-existing
root-CLAUDE.md warning).

3f77b68727 docs(voyage): correct now-false harness-constraint claims (W0/CC-01)
8 sites across 3 orchestrator agents + trekbrief/trekplan/trekresearch asserted 'the harness does not expose the Agent tool to sub-agents' as present fact -- the rationale for the v2.4.0 inline migration. CC 2.1.172 (verified) lets sub-agents spawn sub-agents up to 5 levels deep, so the claim is false. Replaced each with verified history (pre-2.1.172) + current fact + forward pointer to the decision matrix (W1/CC-26). Decision-neutral: states fact without pre-empting the orchestration redesign. 098ba15a5b feat(voyage): define high-effort behavior + amend brief Non-Goal/SC1 + coordinator normalization (Decision B)
Wave 6 / Step 10 — autonomy-gated. Operator confirmed: gemini-bridge
substitution for plan-critic doubling AND SC1 amendment to
resolver-invariant encoding (decisions.local.json recorded).

- commands/trekplan.md: gemini-bridge plan-review Pass 2 on
post-revision plan in high-effort mode (replaces fragile
  plan-critic doubling per risk-assessor).
- commands/trekresearch.md: full swarm + contrarian-researcher +
  gemini-bridge always-on.
- commands/trekreview.md: skip Pass 3 reasonableness + invoke
  coordinator normalization rule.
- commands/trekexecute.md: gates_mode = closed (strict manifest-audit,
main-merge pauses); flag override still wins.
- agents/review-coordinator.md: Pass 3 high-effort normalization —
substitute unknown rule_key with PLAN_EXECUTE_DRIFT, preserve
  original in original_rule_key.
- .claude/projects/2026-05-13-trekflow-solo-lane/brief.md (gitignored,
not committed): Non-Goal amendment locks low/high tiers; SC1
  amendment authorizes resolver-invariant interpretation.
- tests/lib/doc-consistency.test.mjs: +4 pins for the
  "### High-effort behavior (v5.1.1)" heading per command.

Tests: 578 pass, 0 fail, 2 skipped (+4 from 574).

Closes #7 (operator-gated decisions captured + coordinator
normalization landed).

40d8742712 chore(voyage): pin all sub-agents to Opus permanently (operator request)
Flip model: sonnet → model: opus across 20 agent files, 4 prose references
in commands (trekplan, trekresearch), trekendsession command frontmatter,
and CLAUDE.md tables. Aligns CLAUDE.md premium-profile row to actual
premium.yaml content (all-opus, which has been the case since v4.1.0 but
the doc was drift). Companion to VOYAGE_PROFILE=premium env-var (set in
~/.zshenv same day) — env-var governs orchestrator phase model; this
commit governs sub-agent models which are frontmatter-pinned and not
reachable by the profile resolver.

npm test: 516 pass, 0 fail, 2 skipped (unchanged from baseline).

Operator rationale: complete Opus coverage across all Voyage activity,
including the 20 sub-agents that the profile system does not control
(architecture-mapper, task-finder, plan-critic, scope-guardian,
brief-reviewer, code-correctness-reviewer, brief-conformance-reviewer,
review-coordinator, session-decomposer, plus the 6 researcher agents,
plus the 5 codebase-analysis agents).

Cost implication: sub-agent runs ~5x more expensive vs sonnet. Accepted.

3683ae0070 chore(voyage): release v5.0.0 — remove bespoke playground + /trekrevise + Handover 8; render produced artifacts to HTML + link, annotate via /playground
The v4.2/v4.3 bespoke playground SPA (~388 KB), the /trekrevise command,
Handover 8 (annotation → revision), the supporting lib/ modules
(anchor-parser, annotation-digest, markdown-write, revision-guard), the
Playwright e2e suite, and the @playwright/test / @axe-core/playwright
devDeps are removed. A browser walkthrough found the playground borderline
unusable, and it duplicated the official /playground plugin's
document-critique / diff-review templates.

In their place: scripts/render-artifact.mjs — a small, zero-dependency
renderer that turns a brief/plan/review .md into a self-contained,
design-system-styled, zero-network .html (frontmatter folded into a
<details> block). /trekbrief, /trekplan, and /trekreview call it on their
last step and print the file:// link; to annotate, run /playground
(document-critique) on the .md and paste the generated prompt back.

Resolves the v4.3.1-deferred findings as moot (their target files are
deleted). npm test green: 509 tests, 507 pass, 0 fail, 2 skipped.

1afa7e2c72 docs(ultraplan-local): complete external-architect doc rydding [skip-docs]
D2 of post-v3.4.1 stabilisering. Removes 14 plugin-name references from
agents/, commands/, and docs/ tracked files (CLAUDE.md/README.md/SECURITY.md
were ryddet in v3.4.1 commit 52cfc64).

The external architect plugin was moved out of the public marketplace
2026-05-04 due to ToS concerns around future skill sources. References in
prose are now stale or misleading for public users. The architecture/overview.md
filesystem slot remains available for any compatible producer — discovery
is plugin-agnostic via lib/validators/architecture-discovery.mjs (drift-WARN,
never drift-FAIL).

Files:
- agents/planning-orchestrator.md (1 ref generalized)
- commands/ultraplan-local.md (2 refs generalized; missed by prompt inventory)
- docs/HANDOVER-CONTRACTS.md (4 refs generalized; Handover 3 + stability summary)
- docs/architect-bridge-test.md (deleted; was a public-only bridge checklist)
- docs/subagent-delegation-audit.md (5 refs/rows removed; intervention #5 dropped, recommendation adjusted)

CHANGELOG.md retains historical references (20 occurrences) intentionally.

3c2bda87db refactor(marketplace): split cc-architect from ultraplan-local into its own plugin
Extract `/ultra-cc-architect-local` and `/ultra-skill-author-local` plus all 7
supporting agents, the `cc-architect-catalog` skill (13 files), the
`ngram-overlap.mjs` IP-hygiene script, and the skill-factory test fixtures
from `ultraplan-local` v2.4.0 into a new `ultra-cc-architect` plugin v0.1.0.

Why: ultraplan-local had drifted into containing two distinct domains — a
universal planning pipeline (brief → research → plan → execute) and a
Claude-Code-specific architecture phase. Keeping them together forced users
to inherit an unfinished CC-feature catalog (~11 seeds) when they only
wanted the planning pipeline, and locked the catalog and the pipeline into
the same release cadence. The architect was already optional and decoupled
at the code level — only one filesystem touchpoint remained
(auto-discovery of `architecture/overview.md`), which already handles
absence gracefully.

Plugin manifests:
- ultraplan-local: 2.4.0 → 3.0.0 (description + keywords updated)
- ultra-cc-architect: new at 0.1.0 (pre-release; catalog is thin, Fase 2/3
of skill-factory unbuilt, decision-layer empty, fallback list still
  needed)

What stays in ultraplan-local: brief/research/plan/execute commands, all
19 planning agents, security hooks, plan auto-discovery of
`architecture/overview.md` (filesystem-level contract, not code-level).

What moved (28 files via git mv, R100 — full history preserved):
- 2 commands, 8 agents, 1 skill catalog (13 files), 2 scripts, 8 fixtures

Documentation updates: plugin CLAUDE.md and README.md for both plugins,
root README.md (added ultra-cc-architect section, updated ultraplan-local
section), root CLAUDE.md (added ultra-cc-architect to repo-struktur),
marketplace.json (registered ultra-cc-architect), ultraplan-local
CHANGELOG.md (v3.0.0 entry with migration guidance).

Test verification: ngram-overlap.test.mjs passes 23/23 from new location.

Memory updated: feedback_no_architect_until_v3.md now points at the new
plugin and reframes the threshold around catalog maturity rather than an
ultraplan-local milestone.

e4e0ca5828 Revert "feat(ultraplan-local): M1 — profile recommendation flow in ultrabrief"
This reverts commit 55ad63fcd8.

Kjell Tore Guttormsen
55ad63fcd8 feat(ultraplan-local): M1 — profile recommendation flow in ultrabrief
Adds the profile recommendation step to /ultrabrief-local Phase 4. The
brief stays universal (same questions, same template); the new step is
purely a processing-decision layer that records which profile downstream
commands should apply.

What lands:
- agents/profile-recommender.md — new sonnet agent that scores available
profiles against the finalized brief (keyword + NFR-signal matching,
axis bumps, hallucination gate that forbids inventing profile names).
Emits a fenced JSON block with ranked entries.
- templates/ultrabrief-template.md — frontmatter gains
  recommended_profile, profile_match, profile_rationale (default values
applied when only `default` is available — true at M1).
- commands/ultrabrief-local.md — Phase 4 gains Step 4h with explicit
branches: short-circuit when only `default` exists; AskUserQuestion
confirmation when top score ≥ 0.7; explicit fallback message when below
threshold; manual selection sub-question on user override. Persists the
three frontmatter fields to brief.md after user confirmation. JSON
parser failure falls back to `default` with `profile_match: fallback`
rather than blocking — silent fallback is the worst outcome, but a
  *visible* fallback is acceptable.
- scripts/profile-loader.mjs — adds selectRecommendation(ranked, opts) +
RECOMMENDATION_THRESHOLD=0.7 export. Single source of truth for the
threshold logic so the command spec and the helper agree.
- scripts/profile-loader.test.mjs — 10 new tests for selectRecommendation
  (default-only, empty/malformed input, above/below threshold, custom
threshold, max-by-score, missing fields). Total now 36/36.
- README.md / CLAUDE.md / marketplace landing — docs reflect M0 + M1
  shipped, M2 + M3 still pending.

In practice nothing changes for users at M1 because only `default` is
available — Step 4h takes the short-circuit path and writes
`profile_match: default-only`. M2 ships the additional profiles that
make the recommender meaningful.

1e81334bd9 feat(ultraplan-local)!: v2.4.0 — orchestrator agents as inline reference
Redefine research-orchestrator, planning-orchestrator, and
architect-orchestrator from "background executor" to "inline
reference documentation". The agent files remain as the canonical
workflow descriptions, but the /ultra* commands now execute the
phases directly in the main command context instead of spawning
these agents as sub-agents.

The /ultra* command markdowns are now the de-facto orchestrators.
Splitting work into a separate sub-agent was incompatible with the
harness's treatment of the Agent tool (not exposed to sub-agents).

BREAKING CHANGE: These agents are no longer invoked. Any external
integration that spawned them directly should now invoke the
corresponding /ultra* command instead.

d59dd84377 fix(ultraplan-local): v2.3.2 — skill-drafter slug-collision hint
skill-drafter now reads {catalog_root}/<slug>.md before writing its
draft and prepends a warning block to its confirmation output when
an existing skill would be overwritten during manual `mv` promotion.
The draft is still written to .drafts/<slug>.md — the check is a
hint, not a block.

Closes v2.3.0 dogfood finding (post_dogfood_findings[0]): the
drafter produced .drafts/hooks-pattern.md when an approved
hooks-pattern.md seed already existed, giving no signal that `mv`
during promotion would silently overwrite the seed. v2.3.1
introduced the qualified-slug mechanism to resolve such collisions;
v2.3.2 surfaces them at the right moment — before promotion.

Changes:
- agents/skill-drafter.md — new Step 2 between slug computation and
  source reading. Reads {catalog_root}/<slug>.md, inspects
review_status, derives a kebab-case qualifier from the concept
handle (or source basename fallback). Subsequent steps renumbered
3→7. Output format gains Collision: field and optional warning
  block. New Hard Rule.
- tests/fixtures/skill-drafter/slug-collision-expected.md — reference
fixture documenting expected confirmation shape across four
scenarios (no collision, approved collision, soft pending
collision, collision with no good qualifier). Skill-drafter is
prompt-driven; fixture anchors shape for human verification and
  downstream parsers.
- CHANGELOG [2.3.2], plugin.json 2.3.1→2.3.2, README badge, plugin
  CLAUDE.md slug-convention Collision-hint bullet, marketplace root
README summary, marketplace root CLAUDE.md plugin table.

Non-breaking. No frontmatter/drafts-layout/tool-scope/regex changes.
Existing pipelines see one extra field and an optional warning —
both purely additive.

9288d34f50 feat(ultraplan-local): v2.3.1 — qualified slug convention for cc-architect-catalog
Resolves v2.3.0 dogfood collision: skill-factory produced a
specialized hooks-pattern.md draft that would have overwritten the
generic seed. Qualified slugs let one feature host multiple named
patterns at different abstraction levels.

Slug convention: <cc_feature>[-<qualifier>]-<layer>.md. Unqualified =
canonical baseline. Qualified = sub-pattern (e.g., hooks-observability-
pattern.md) that does not displace the baseline.

Changes:
- SKILL.md: slug convention section, coverage-table qualified column,
matcher logic for N patterns per feature, modification rules cover
  qualified-vs-canonical choice and collision handling.
- feature-matcher: catalog map is cc_feature -> {layer -> [skills]};
selection rules (baseline by default, qualified when justified,
  multi-skill when non-overlapping); supporting_skill accepts list.
- gap-identifier: adds pattern_count[cc_feature] to coverage audit.
- architecture-critic: supporting-skill verification — every cited
skill name must exist in the catalog (blocker severity).
- First qualified skill: hooks-observability-pattern.md (promoted from
  .drafts/, source ai-psychosis/README.md, ngram-overlap 0.01).
- Version bump 2.3.0 -> 2.3.1 across plugin.json, badges, table, root
  CLAUDE.md, CHANGELOG.

Non-breaking: existing unqualified slugs keep working, no cc_feature
taxonomy changes, hallucination gate unchanged.

028f4b0224 feat(ultraplan-local): add skill-author-orchestrator
Opus orchestrator for /ultra-skill-author-local. Sequential 5-phase pipeline:
validate source → concept-extractor → skill-drafter → ip-hygiene-checker
→ completion summary.

No retry logic, no parallelism, no automation (per brief Non-Goals). Each
phase consumes the previous phase's output. --quick mode skips IP-hygiene
with a BIG WARNING for testing the drafting pipeline in isolation.

e0ab9ee448 feat(ultraplan-local): add ip-hygiene-checker agent
Sonnet worker for /ultra-skill-author-local. Runs scripts/ngram-overlap.mjs
on draft + source, parses verdict, takes action:

- accepted/needs-review: stamp ngram_overlap_score in frontmatter (Edit)
- rejected: rm draft (no preservation per brief Success Criteria 4)

Tool scope: Bash (node + rm in .drafts/), Read, Edit. Score is containment
rounded to 2 decimals to match success-criteria regex.

f4de2735f8 feat(ultraplan-local): add skill-drafter agent
Sonnet worker for /ultra-skill-author-local. Consumes concept-extractor JSON
plus source file, writes a 150-600 word draft to .drafts/<slug>.md with the
9-field frontmatter contract (review_status=pending, ngram_overlap_score=null).

Imperative voice, progressive disclosure, no verbatim copy from source.
Aborts with too-technical-to-paraphrase if the subject can't be rephrased.

Sonnet worker for /ultra-skill-author-local. Reads ONE local source file
and emits structured concept JSON with cc_feature/layer/concept fields.

Enforces two gates:
- Hallucination gate: cc_feature MUST be one of 8 canonical values
- Gap-class gate: class C (decision) and D (outside CC) → out_of_scope

07f02e77db feat(ultraplan-local): v2.2.0 — /ultra-cc-architect-local
New optional command between /ultraresearch-local and /ultraplan-local that
matches brief+research against Claude Code features (hooks, subagents, skills,
output-styles, MCP, plan-mode, worktrees, background-agents) and produces an
architecture note with brief-anchored rationale plus explicit gaps.

Added:
- commands/ultra-cc-architect-local.md (--project, --fg, --quick, --no-gaps)
- agents/architect-orchestrator.md (opus) — 6-phase background orchestrator
- agents/feature-matcher.md (sonnet) — fallback-ranked feature proposals
- agents/gap-identifier.md (sonnet) — 4 gap classes with issue-ready drafts
- agents/architecture-critic.md (sonnet) — hallucination gate as BLOCKER
- skills/cc-architect-catalog/ — SKILL.md + 10 seed entries (reference/pattern)

Changed (non-breaking):
- commands/ultraplan-local.md — auto-discovers architecture/overview.md
- agents/planning-orchestrator.md — cross-references cc_features_proposed
- plugin.json — 2.1.0 → 2.2.0, description, cc-architecture keyword
- CHANGELOG, README, CLAUDE.md (plugin + marketplace root)

Pipeline becomes brief → research → architect → plan → execute. Architect is
optional; existing project dirs keep working unchanged.

6ca03e24ce feat(ultraplan-local): v2.1.0 — dynamic quality-gated interview
Replace hardcoded Q1-Q8 in /ultrabrief-local with a section-driven
completeness loop (Phase 3) and a draft/review/revise loop with
brief-reviewer as stop-gate (Phase 4). Quality drives the interview,
not a question counter.

brief-reviewer now emits a machine-readable JSON block with per-dimension
scores (1-5) and detail arrays alongside the existing prose report;
planning-orchestrator continues to consume the prose verdict unchanged.

Phase 4 gate: all dimensions >= 4 AND research_plan = 5. On fail, a
targeted follow-up is generated from the weakest dimension's detail
field and the draft is re-reviewed. Max 3 review iterations bound cost;
exhaustion writes brief.md with brief_quality: partial and an explicit
Brief Quality section. Force-stop surfaces per-dimension findings before
the user chooses continue or partial.

Not breaking. /ultrabrief-local [--quick] <task> interface unchanged.
--quick now means compact start with escalation, not a max-N cap.

973138239d feat(ultraplan-local)!: v2.0.0 — brief-driven four-command pipeline
Extract interview from /ultraplan-local into new /ultrabrief-local command.
/ultraplan-local now requires --brief or --project (breaking). All pipeline
artifacts land in one project directory: .claude/projects/{date}-{slug}/
with brief.md, research/, plan.md, sessions/, progress.json.

Breaking changes:
- /ultraplan-local requires --brief <path> or --project <dir>
- /ultraplan-local --spec removed (convert specs to briefs per MIGRATION.md)
- Interview phase moved to /ultrabrief-local
- spec-reviewer renamed to brief-reviewer with 5th dimension (Research Plan validity)

Added:
- /ultrabrief-local command (interactive interview → brief.md with research plan)
- templates/ultrabrief-template.md (task brief format with intent + research plan)
- brief-reviewer agent (5-dimension brief quality review)
- --project <dir> flag on /ultraresearch-local, /ultraplan-local, /ultraexecute-local
- MIGRATION.md (v1 → v2 upgrade guide)

Changed:
- planning-orchestrator accepts Brief file: input (was Spec file:)
- planning-orchestrator Phase 1b uses brief-reviewer
- README + CLAUDE.md rewritten for four-command pipeline and task/research brief terminology
- CHANGELOG.md [2.0.0] entry with rationale
- Marketplace root README + CLAUDE.md updated to v2.0.0
