Round 6 (v6.2.0..c3641ff, WARN: 11 MAJOR, 3 MINOR; 0 BLOCKER). The round-5
fixes hold (code-correctness-reviewer); three of the new MAJOR were side
effects of running checks with no shell. Each fixed behind a test that
failed first on c3641ff.
- git checks stay in the repo too: no path that climbs out with .., starts
at / or reads <rev>:../x, and no --no-index (both reviewers).
- A trailing `# comment` on a shell-fence line is dropped: with no shell,
its words made an honest check NOT RUN, among them the first check of the
shipped example plan.
- A glob reaches `node --test` only on Node 21 and later, which expands it
itself; on an older Node it is refused with that reason (engines >=18).
- With no shell, exit 126/127 is the test program's own: red proof reads it
as a red (the control run still tells an environment fault apart), the
done line and the test lock as a failure.
- A runner that could not start (not installed) is NOT RUN in the criteria
runner and not-run (exit 2) in regression-check, as red-proof and
done-line already said; never a red suite.
- Tests-only: a backticked `Makefile` alone in its entry is a path, and red
proof confirms the plan's tests-only reading against the step's own diff
(the altitude finding: the diff is a fact, the Files: prose is not).
- release-check stops a suite that never ends (15 minutes) and reads it as
not measured (exit 2).
- The two lines this release added to pre-bash-executor.mjs are ASCII.
- MINOR: git and the other runners share one plain-word pattern.
Not in this round: MINOR f00a2eb8 (shared norm/ticked-path and isMain
helpers) and MINOR on plan-review-dedup's repeated tokenizing.

Round 5 (v6.2.0..d37e538, WARN: 11 MAJOR, 2 MINOR). The patching did not
converge (the same two places every round), so the operator chose a
structural fix on 2026-10-11. Each change behind a test that failed first.
- A check runs with no shell. criteria-runner's runCommand starts the runner
with the very words the screen checked (spawnSync(words[0], words.slice(1)),
without NODE_TEST_CONTEXT); Phase 7 criteria, Regression: (its exec copy is
gone, MINOR 0e98d5c7), proof of red, the done line and the test lock all
run through it. No quote, glob, tilde or variable can rewrite a checked
word any more. The screen stays and says why: a word that means something
else in a shell (quote, backslash, $, brace, leading ~) is refused; a glob
reaches only node --test, which expands its own patterns (sh read
`tests/.?/` as `tests/../`); no path climbs out with .. or starts at /;
a word starting with @ is refused (pytest reads @file as more arguments).
- The K6 plan-file sweep has v6.2.0's own Files: reader again (516dffa):
backticked paths, and plain words only in a field without them. The
shared reader from round 3 planned directories a description named and
dropped paths v6.2.0 planned. fieldPaths stays for the tests-only check,
with a path test on spans (`parse()`, `p.parse()` are no paths), plain
descriptions ignored when the field has backticked paths, dotfiles,
backslash paths and glued spans read.
- Skill frontmatter reads a value as YAML does: trailing space is nothing,
and a comment ends a plain value, a flow list or a block indicator
(`model: opus # pinned`). A comment that cuts a plain description, or is
the whole value, is still a hazard.
- release-check reads the diff with --no-renames: a test moved into an
allowed place after the review shows as removed.
- The example plan's step 2 heading no longer says "must FAIL initially";
steps 1-2 keep the tests-only warning (operator 2026-10-09, 73262b6).
Open: MINOR c1fef1ad (one shared isMain helper) is not in this round.
Measured: 0 of 463 Files: fields with backticked paths (520 fields in 182
plans across the marketplace repos) carry another path-like word.
Suite 2185 (2183/0/2).

Round 4 (v6.2.0..41ea8fc, WARN: 9 MAJOR, 3 MINOR); the nine MAJOR fixed,
each behind a test that failed first (operator 2026-10-10). Three were
regressions from round 3's shared Files: reader.
- fieldPaths keeps a backticked path with punctuation after it and one
inside a note, a directory (docs/) and a glob (src/*.mjs) in plain text,
and app/(auth)/x.ts whole; a bare word counts only alone and capitalised
(Makefile), so "format" or "v6.3.0" in a description is no path. The K6
sweep plans what v6.2.0 planned for `a.mjs`: and `b.mjs`.
- skill frontmatter: a quoted value may carry a YAML comment after it.
- criteria runner: no word with a leading ~ (sh expands it), and nothing
after `make test` (another target would run).
- release-check: scope_sha_end must be a commit sha (a ref name passed
coverage whatever was reviewed), and README lines changed after the review
may differ only in the badge's version. The round-3 leading-zero example
is 7 characters now, the shortest sha accepted.
The three MINOR (one word screen for git, the exec copy in regression-check,
a shared isMain helper) are not in this round.
Suite 2175 (2173/0/2).

Round 3 (v6.2.0..96cb977, BLOCK: 1 BLOCKER, 8 MAJOR, 2 MINOR), each fixed
behind a test that failed first.
- Every allowlisted runner, not only git, takes plain words and a short list
of options that only read: an option that writes a file or loads code
(--test-reporter-destination, --outputFile, --junitxml, --basetemp,
--import, --config, -p, -o, -exec, a reporter path) is on no list; after
`npm test --` only paths follow; `make test` takes no VAR=value. A built-in
node test reporter (tap, spec, dot, junit, lcov) stays allowed.
- /trekexecute: a restore the guard refuses (destructive git, in the mode
that shows no prompts) or the operator declines is never recorded as
reverted: simplify = "revert-refused" and the run stops before review; a
refused step revert is reported with its paths.
- stepTestsOnly: a fixture document under tests/ is a test file, "e.g." is
not a path, ./ is dropped; the plan-file sweep reads Files: with the same
reader (fieldPaths); the validator hands its parsed fields over.
- plan-validator --steps-from asks for Verify: on a 1.8 plan too.
- skill frontmatter: a value starting with # is a hazard; a quote after an
escaped backslash closes the value. Routing reads kind tags with hasOwn.
- release-check reads scope_sha_end as written (an all-digit sha keeps its
leading zero), and Voyage's own artifacts under .claude/projects/ and
.claude/voyage/ committed after the review do not count as unreviewed.
Suite 2170 (2168/0/2).

Round 2 (v6.2.0..0d044f6, BLOCK: 2 BLOCKER, 11 MAJOR, 1 MINOR), each fixed
behind a test that failed first.
- The git runner checks only words /bin/sh passes through unchanged (no
quote, backslash, $, glob or brace) and refuses --output and its
abbreviations: round 1's --output refusal was bypassed by quoting.
- fieldPaths: backticked paths are kept as written, parentheses and all; in
plain text every path-like word counts, and a parenthesised note does not.
- plan-validator --steps-from reads the plan's ## Verification for an
appended End-to-end step, and warns instead of failing under --soft.
- regression-check prints progress_status, the value progress.json takes;
/trekexecute copies it.
- release-check: a plugin in a subdirectory is checked in its own directory,
the suite is not run when a cheaper check already makes the answer exit 2,
and a bad argument or a crash is exit 2.
- Skill routing looks a skill up among skills only, and builds the corpus
vectors once.
- /trekexecute Phase 7.2 says plainly that remediation steps in the review
template's own form stop with a re-plan; making them runnable is a later
order (operator 2026-10-10).

The review of v6.2.0..f669b87 (BLOCK: 1 BLOCKER, 12 MAJOR, 1 MINOR), each
fixed behind a test that failed first, or, for the two missing tests, a test
shown to fail on a stub of the branch it covers.
- Review steps appended by /trekexecute Phase 7.2 are held to the step rules
on any plan version: plan-validator --steps-from <N> (on a plan below 1.8 a
template review step validated and was executed).
- The guard refuses where no one can be asked also when its rules will not
load; the wave merge path names that refusal instead of MERGE_REFUSED.
Tests for the classifier-error refusal and post-approval-log's own check.
- release-check: the version files may change only in their version after
the review (JSON compared without it), README diff lines are read inside
hunks only, an unreadable diff is exit 2, and npm test runs without
NODE_TEST_CONTEXT.
- What counts as a step of only tests: a note in parentheses is not a path,
and a data file under specs/ is not a test. End-to-end: rides on the last
step that builds behaviour (validator hint, trekplan, template).
- A removed line is cited as <rev>:<path>:<line> (findings-schema hint,
trekreview re-ask).
- skill-lint counts a trailing newline as no line; regression-check takes
commandEnv from lib/util/command-env.mjs; CLAUDE.md names four mode-read
exemptions.
- /security-review, older than v6.2.0: the criteria runner refuses git
--output, which writes a file.

1. A step of only tests can never be proven red. One reading,
stepTestsOnly (lib/parsers/plan-schema.mjs, test paths from the new
lib/util/test-path.mjs): the plan validator warns PLAN_STEP_TESTS_ONLY,
red-proof --plan/--step adds tests_only to a not-red-at-base, and
/trekexecute and the step helper send such a step straight up to the
plan instead of into fix rounds that cannot succeed. /trekplan, the plan
template and plan-critic (dimension 8, still 12) say the test belongs in
the step that builds the behaviour.
2. Timing tests under load: the hook helper's kill limit is 30 s and per
call overridable; SC #14 passes on the fastest of up to three runs
against the same 800 ms limit (tests/helpers/timing.mjs).
3. The example plan's On failure lines open with a K11 word.
4. /trekreview saves the reviewer's whole answer for the findings check and
names FINDING_PROSE_LINE_OUT_OF_RANGE (error: re-ask; warning: report).
5. The criteria runner's commands run without NODE_TEST_CONTEXT
(commandEnv, moved to lib/util/command-env.mjs).
6. The resolver returns effort only when the brief's phase signal sets one;
the four commands said it always did.
Also: the K2 hook tests used fixed session ids in a per-user temp dir, so
two suites running at once took each other's approval records; the ids are
now unique per run.

The minor points (B8-B12, C13, C14, C16, C17) of the second voyage-lekerepo
trial (2026-10-08), each verified against the code and started with a test
that failed first.
- K10: optional step field `Regression:` - one command the criteria runner
allows, unpiped; plan-validator refuses one that can never run
(PLAN_STEP_REGRESSION_NOT_RUNNABLE). Any plan_version, no bump.
- K11: `On failure:` opens with revert | retry | skip | escalate, or the
validator warns (PLAN_STEP_ON_FAILURE_UNKNOWN); onFailureAction in
plan-schema.
- B8 (operator decision 2026-10-08): minors open after the last critic round
are listed numbered in the Phase 10 summary; the ones the operator picks are
fixed and get one more round (--extra-round when the 3 are spent).
- B11: plan-review dedup merges two reviewers' findings on the same file:line
with the same content (trial pair: Jaccard 0.44, overlap 0.83); the Jaccard
threshold for every other pair is unchanged.
- B12: /trekbrief Step 4c says the reviewer runs in the background and is
waited for, not "foreground, blocking".
- C13: the Phase 4 size count includes .mjs, .cjs, .mts, .cts.
- C14: --evidence shows the first output line on PASS rows.
- C16: findings-schema checks path:line citations in finding prose (error) and
in the prose around the block (warning) against the file.
- C17: git-historian gets the brief, never proposes changing a frozen file,
and names the remote from `git remote -v`.

Wave 1, order 3 (P3 plan mode as the start, P2 design as its own part, P9
plan side). Contracts K3-K6 for the later merges.
- plan_version 1.8 (1.7 plans stay valid): Test first on every step, one
End-to-end ("run the thing") step, optional Interface in / Interface out /
Review focus fields, a ceremony class in the header (absent =
architectural), and for architectural plans a ## Design part: module map
without cycles, architecture rules that name their enforcer and the step
that proves them, a requirements check asked as keep / change.
- K3 lib/validators/plan-gate.mjs: the critic verdict is recorded INSIDE the
plan (Claude Code's plan approval hands a hook only the plan file's text),
bound to the hash taken before the reviewers read it; 0 go / 1 stop / 2
could not run; at most 3 critic rounds, then stop and ask.
- K5 lib/verification/arch-rules.mjs: runs the target repo's own
dependency-cruiser or import-linter (installs nothing; 2 = not measured,
never green); --prove --rule shows pass -> fail on a planted violation of
that rule -> pass. Verified against dependency-cruiser 18.5.0 and
import-linter 2.15; madge 8.0.0 left out (missed a planted .mjs cycle).
- K6 lib/verification/plan-scope-sweep.mjs: changed files no step names.
- /trekplan Phases 8-10: design part, bounded critic loop, the gate before
"execute", a plan-mode path through ExitPlanMode (not yet run end to end
inside plan mode). plan-critic 10 -> 12 dimensions; scope-guardian checks
the ceremony.
- Fixed on the way: plan-validator and plan-review-dedup used the
file://argv[1] main guard (no-op under a symlink or a space); CRLF plans
failed every manifest; the dedup dropped finding severity.

New case plan-halts-without-intent-approval (expectation committed before
its first run): a 2.1 brief WITH phase_signals and no intent marker.
Graders: intent-approval.mjs --check ran; the trace carries the gate's own
JSON code for BRIEF_INTENT_NOT_APPROVED; no Agent; no plan.md.
review-requires-project: the arg parser now has a CLI that checks the
required flag and prints 'Error: --project <dir> is required.' + usage
(exit 1, code ARG_REQUIRED_MISSING). trekreview.md runs it with
$ARGUMENTS (it passed "$@", which the Bash tool never has, so the parser
never ran — 0 Bash calls in the PM's case-5 traces) and relays its stderr
instead of composing the line. The grader project-required (reply regex,
unstable on backticks) is replaced by parser-ran + names-missing-project
(trace, the parser's JSON code), the same form as names-rule.
Graders: no-error-code is bound to the validator's JSON output form and
covers every REVIEW_* code (WRONG_TYPE and VERSION_FORMAT were missing);
says-pass/says-fail reject a preceding NOT and a longer word; every case
with no-write also gets no-bash-write (redirect, tee, touch, cp, mv, rm,
sed -i in the Bash command). Checked on the PM's 10 recorded traces: no
false positive; known-positive/negative still split.
Red 348aa95 6/7 → green 7/7. Suite 1231: 1229 pass / 0 fail / 2 skip.

Veikart steg 1, økt 1 av 2. The three holes measured in the grounds:
(1) ## Intent / ## Goal were only checked for existence, (2) no approval
marker existed, (3) brief-approved was emitted only on the auto path.
- lib/validators/intent-approval.mjs: --check / --stamp. The marker is
intent_approved_hash (sha256 over normalized ## Intent + ## Goal) +
intent_approved_at. Editing either section after approval → STALE.
- /trekplan Phase 1 (Read the brief, both --brief and --project): runs
--check and HALTS on BRIEF_INTENT_NOT_APPROVED / _STALE / _INVALID with
the remedy spelled out; an unrunnable check halts too.
- /trekbrief Phase 4h (before the Phase 5 fork, asked even in --quick):
shows Intent + Goal verbatim, AskUserQuestion Approve / Revise / Leave;
only "Approve" runs --stamp. --stamp emits brief-approved, so the manual
(default) path records it; the auto path's own emission is removed.
New mode /trekbrief --approve <project-dir> = Phase 4h alone.
- README, CLAUDE.md, command-modes, HANDOVER-CONTRACTS §Handover 1,
jsonl-schemas (trekbrief-stats gains intent_approved).
Valgt ingen brief_version-bump fordi skjemaendringen er rent additiv (to
valgfrie felt) og kravet sitter i /trekplan — enhver produsents brief kan
godkjennes via /trekbrief --approve uten produsentendring. Valgt eget
What the marker does NOT prove (module header, command prose, contract):
same user, same machine — any session can stamp. A trace, not a signature.
Suite 1183 → 1201 (1199/0/2). Mutants M1–M5 (stale check off, check
always valid, gate line removed, stamp emits nothing, no normalization)
each fell ≥ 1 test. yardstick unchanged: RED, 1 of 3 countable.

The plan template (templates/plan-template.md:21) emits plan_version ONLY in
the prose blockquote metadata line:
> Generated by trekplan v{version} on {YYYY-MM-DD} — `plan_version: 1.7`
It carries no frontmatter plan_version (the frontmatter block is trekreview-only
and holds source_findings). But PLAN_VERSION_REGEX in lib/parsers/plan-schema.mjs
was `^`-anchored (/m), matching ONLY line-start (frontmatter). The backtick-
wrapped prose form never matched → extractPlanVersion returned null → every real
generated plan got a spurious PLAN_NO_VERSION warning from plan-validator.
Not caught earlier because no test ran the parser against the actual template;
the synthetic fixtures all carry frontmatter plan_version and parse fine.
Internal source contradiction surfaced: the parser comment documents intent
"frontmatter OR doc body", planning-orchestrator.md:228 + the template place it
in the prose line, while HANDOVER-CONTRACTS.md calls it a frontmatter field.
Operator-chosen fix (parser, not template): relax the regex to
`/(?:^|`)plan_version:.../m` so it matches line-start (frontmatter) OR the
backtick-prefixed prose form. Honors the parser's own documented contract,
fixes all existing + future plans, changes no plan output, touches one code
file — vs the template-fix which would alter every plan's shape, contradict
planning-orchestrator.md, and force a 3-file doc reconciliation (scope creep).
TDD (red first): added prose-form + canonical-template regression pins in
plan-schema.test.mjs and a no-PLAN_NO_VERSION pin in plan-validator.test.mjs;
all 3 red on the `^`-anchored regex, green after the relax. The template pin
ties the parser directly to the real generated artifact.
S22 defect closed in docs/S22-happy-path-dogfood.md (§Pipeline defects + log).
Tests: 728 (726 pass / 2 skip / 0 fail; live `node --test`, = 725 baseline +3).
plugin validate passes (1 accepted CLAUDE.md-at-root warning).

Make the bake-off-validated Workflow substrate (Arm B) reachable behind an
opt-in --workflow flag for /trekreview. Default Phase 5-6 path stays prose
to preserve the lower portability floor; --workflow raises the consumer
floor to Claude Code 2.1.154+ (the Workflow tool).
- arg-parser: --workflow added to trekreview boolean flags
- commands/trekreview.md: flag row + Phase 5 substrate-routing gate +
new section 'Phase 5-6 via the Workflow substrate' (invocation contract,
S10 gotchas, bake-off citation, auto/bypass residual as Known limitation)
- docs/command-modes.md: --workflow row in /trekreview table
- routes to existing scripts/trekreview-armB.workflow.mjs (byte-identical to
the S10 part-B POSITIVE build); integration is pure routing, no script change
TDD: 8 new tests (arg-parser flag recognition + combine; command/doc prose
pins for route, opt-in posture, 2.1.154+ floor, bake-off evidence).
Suite 662 -> 670 (668 pass / 2 skip / 0 fail). plugin validate clean modulo
known root-CLAUDE.md warning. Resolves W1-narrow-wins-plan.md S11.
The v4.2/v4.3 bespoke playground SPA (~388 KB), the /trekrevise command,
Handover 8 (annotation → revision), the supporting lib/ modules
(anchor-parser, annotation-digest, markdown-write, revision-guard), the
Playwright e2e suite, and the @playwright/test / @axe-core/playwright
devDeps are removed. A browser walkthrough found the playground borderline
unusable, and it duplicated the official /playground plugin's
document-critique / diff-review templates.
In their place: scripts/render-artifact.mjs — a small, zero-dependency
renderer that turns a brief/plan/review .md into a self-contained,
design-system-styled, zero-network .html (frontmatter folded into a
<details> block). /trekbrief, /trekplan, and /trekreview call it on their
last step and print the file:// link; to annotate, run /playground
(document-critique) on the .md and paste the generated prompt back.
Resolves the v4.3.1-deferred findings as moot (their target files are
deleted). npm test green: 509 tests, 507 pass, 0 fail, 2 skipped.

Pure module computing deterministic 16-char SHA-256 prefix for annotation set.
Canonicalization: sort by id, fixed field order (id|target_artifact|target_anchor|intent|comment|timestamp), \n-join, sha256, take first 16 hex.
Brief SC4 specifies sha256-prefix; research-05 said sha1 — brief wins per Hard Rule "Brief-driven".
6 tests pass: empty digest, order-independence, intent-sensitivity, format invariant, golden value, undefined-vs-empty equivalence.
Step 3 av v4.1-execute (Wave 1, Session 1).
Legg ny eksportert const OPTIONAL_STRING_KEYS = ['profile_used'] parallel
til eksisterende OPTIONAL_KEYS. Utvid parseManifest med ny dispatch-loop
etter OPTIONAL_BOOLEAN_KEYS. Returnerer MANIFEST_OPTIONAL_TYPE hvis
profile_used finnes men ikke er string.
Forskjell fra OPTIONAL_BOOLEAN_KEYS: absence == not-present (NOT defaulted
til false, unlike boolean). Downstream-konsumenter kan dermed skille mellom
unset og empty-string.
Tester (5 nye, baseline 372 → 377):
- OPTIONAL_STRING_KEYS export drift-pin
- profile_used: economy parses successfully (SC #10 forward-compat)
- profile_used: numeric rejected
- absence: field NOT in parsed (string-key semantics)
- profile_used + skip_commit_check + memory_write co-existence

Step 2 av v4.1-execute (Wave 1, Session 1).
Legg --profile i valued-arrayen for alle 6 voyage-kommandoer (trekbrief,
trekresearch, trekplan, trekexecute, trekreview, trekcontinue). Mønster
identisk med eksisterende --project/--brief valued-handling. Ingen endring
til parseArgs-logikk — utvider kun schema.
Tester (11 nye, baseline 361 → 372):
- 6 happy-path-tests (én per kommando)
- ARG_MISSING_VALUE for --profile uten verdi
- --profile + --quick kombo
- --profile + --gates edge-case (--gates parses inline, ikke i FLAG_SCHEMA)
- --profile + --project kombo
- trekcontinue --profile (validerer at tomt valued[] nå er utvidet)

The ultra-cc-architect plugin was removed from the marketplace; voyage's
architecture-discovery contract still pointed at it by name. Replaced
verbatim references with plugin-agnostic phrasing ("upstream architect
producer") in code comments and user-facing warning messages.
CHANGELOG entries and config-audit v5.0.0 snapshots intentionally
preserved as historical records.
Step 1 of v3.4.1 hot-fix plan (project 2026-05-04-v3.3.1-ultracontinue-fixes).
Adds ultracontinue entry to FLAG_SCHEMA covering boolean flags --help,
--cleanup, --confirm, --dry-run with no valued flags. The -h short form
is intentionally not aliased: it appears as positional[0] === '-h' and
the command prose dispatches usage on either condition.
7 new tests in tests/lib/arg-parser.test.mjs verify empty args, --help,
-h positional, --cleanup, --cleanup --confirm, project-dir positional,
and .md positional (parser-level accept; command-level reject).
