feat(trekexecute): couple the executor to the review gate, the plan approval and the step checks
Wave 2, order 6. /trekexecute now:
- refuses a plan whose logged approval does not cover its current sha256
  (K2, approval-log verify --kind plan), before step 1;
- runs the step check (K9) and the step's optional Regression: (K10, new
lib/verification/regression-check.mjs through the criteria runner) at step
  end, before the checkpoint; exit 2 is "not measured", never green;
- runs the architecture rules (K5) and the plan-file sweep (K6) after the last
  step; a violation or a file outside the plan turns completed into partial;
- reviews by itself in the closing run (Phase 7.2): run the thing, /simplify,
/trekreview, verdict-gate; a BLOCK adds the review's remediation steps after
the plan validator accepts them and the operator re-approves the plan, capped
  at TREKEXECUTE_MAX_REVIEW_ROUNDS (3);
- makes the main-merge gate a lock on the verdict: BLOCK or undecided keeps
the merge closed; when open, the executor merges and the guard's question is
the one question (operator decision 2026-10-08). The --gates pause before
the wave merge-back is gone for the same reason.

The guard refuses a merge into main and a push in the permission mode that
shows no prompts, since no one can be asked (operator decision 2026-10-09; two
mode-read exemptions approved by the operator).

progress.json gains optional coupling fields (Handover 5); the validator
rejects the runner's exitCode in plan_verification.failed_criteria (C15).
Pointer lines name the skills test-first and debugging (S2).

feat(review): the review is a gate - verdict in the file, CC engine, project memory
Ordre 5 (wave 1, P11 + P5), review_version 1.1.

- K7: `verdict: ALLOW | WARN | BLOCK` is required in review.md (strict
error, soft warning for 1.0 reviews). New CLI lib/review/verdict-gate.mjs:
  0 = ALLOW/WARN, 1 = BLOCK, 2 = could not decide (never green); a verdict
greener than the file's own BLOCKERs or JSON block is exit 2. Realpath
main guard (the old file:// guard exits 0 under a symlink - measured).
- No finding is struck for length: succinctness trims only MINOR/SUGGESTION
text (catalogue tier), keeps BLOCKER/MAJOR whole, counts trims; the
coordinator lists what it declined to judge.
- Claude Code's /code-review and /security-review run as the engine via the
  Skill tool (never --fix/--comment/--post/ultra); lib/review/engine-findings.mjs
turns their findings into a reviewer payload; two catalogue keys
  (CODE_REVIEW_DEFECT, SECURITY_REVIEW_VULNERABILITY), catalogue 12 -> 14.
An engine that did not run forbids ALLOW.
- Every BLOCKER is written as a numbered `### Step R<n>` with its source
  finding (strict error REVIEW_MISSING_REMEDIATION_STEP).
- P5: lib/review/recurrence.mjs - a finding class in two review.md files
(current + git history, no new store) becomes one proposal, routed by the
catalogue's memory_route; written only between Voyage markers in
  .claude/rules/voyage-review-lessons.md under a size budget, CLAUDE.md gets
a pointer only; the operator approves every line (Phase 8.7).
- Workflow substrate: catalogue enum in step; a missing reviewer or engine
forbids ALLOW there too (it returned ALLOW on zero findings before).
- Fixtures review-run-A/B said WARN over a BLOCKER; now BLOCK.

Borrowed and credited in code: Superpowers (declined-to-judge), ai-native-sdlc
(cap only nits; mistake/fix/check), Spec Kit (managed block; converge steps),
Pocock (retro routing), Kiro (verdict file + gate; evidence per lesson),
Cherny (engine -> brief control order; review -> memory; budget).

feat(brief): ceremony by size, one question round, logged and committed approvals
P1 (order 2, wave 1). /trekbrief puts the task in a size class
(ceremony: small|bounded|architectural, K1; heaviest in doubt, said
aloud, overridable, only moved up) that sets the question budget and the
review cap, and asks one round of questions: facts looked up, never
asked; recommendation first; ranked by impact x uncertainty with a
visible Deferred list; stop when the next three answers can be predicted.
"høres bra ut", "gjør som du vil" and an empty answer are not approval.

K2: lib/util/approval-log.mjs, an append-only hash-chained log in the
target repo (.claude/voyage/approvals.jsonl) bound to the approved
content's sha256; append/verify (0/1/2), answer, digest. Committed by
lib/util/artifact-commit.mjs: the log always, the brief and plan only
with artifact_store: git (operator decision 2026-10-06). /trekplan
Phase 1 halts on a broken, diverged, missing or mismatched log entry;
Phase 11 logs and commits an explicit plan approval before any handoff.

Both new brief fields are optional: no brief_version bump. The
main-module guard in intent-approval.mjs and brief-validator.mjs now
compares real paths. Borrowings credited in the prose and module headers.

fix(docs): the order fields stand on all five surfaces (green)
/trekbrief Phase 7 says the writer adds order_id / order_claimed_at /
order_reason (never typed by the model) and that the yardstick joins use on
order_id; the fixture row lists them in `fields`, EXPORT_DENYLIST names them,
and its stale line_ref (657-672) now reads 961-976; the allowlist header
names them as dropped; docs/architecture.md Stats explains them.

/trekbrief --approve now runs Phase 7 after a successful stamp, with fixed
values (mode "approve", 0 turns, brief_quality/research_topics/auto_research
from the brief's frontmatter, auto_result "manual", intent_approved true).
Valgt fordi PM 04.10 (point 3): fix the code, not the criterion - the
approval must leave the trekbrief record. Measured hole: okf-wiki v1-k has
brief-approved 04.10 17:12Z and trekplan 17:29Z, but only a 03.10 trekbrief
record with intent_approved false. CLAUDE.md, README.md and
docs/command-modes.md say so. The manual path is unchanged.

npm test 1309 -> 1362 (1360 pass / 0 fail / 2 skip).

docs: no user doc promises a launched session, a launch script or a fourth security layer
README claimed four security layers including scoped per-child tool
access (gone with the launched children), an auto-dispatched Phase 7.6
recovery session and a launch template; architecture.md still pointed at
launch.sh. They now describe what 6.0.0 does: session specs plus
orders.md, a recovery order for a session the user opens, three layers.
operations.md gets a status note instead of a rewrite (decision record).

fix(intent): M1 + M2 — the parser fails closed; --approve reaches any brief file
M1: headings inside fenced code blocks and HTML comments are text, not
headings, and ## Intent / ## Goal must each appear exactly once. A second
section, an example section in a code block or comment, or a '## ' line in
a code block inside Intent no longer carries the old approval: check gives
INVALID (duplicate) or STALE, stamp refuses a duplicate. A plain brief
hashes exactly as before (pinned), so no stamped brief goes stale.
Re-run of the PM's 29 probes: vi-a, vi-b, vii-a, vii-b now stop; the rest
unchanged. The PM's p7c probe was a no-op (its replace string is not in the
base brief); vii-c is covered by its own test, red on 66e1fa1.

M2: /trekbrief --approve takes <project-dir | brief-file>. New
intent-approval.mjs --resolve decides: a directory → <dir>/brief.md and
/trekplan --project; a file → itself and /trekplan --brief. Phase 4h and the
stamp line use {BRIEF_PATH}; /trekplan's halt table names
/trekbrief --approve {brief_path}, which works on both brief modes, and so
does the gate's hint. HANDOVER-CONTRACTS' "open to every producer's brief"
is now true. Chose fixing the path over rewording the contract because two
real briefs (docs/*-brief.md) had no approval path at all.

Red ff760ed 9/29 → green 29/29. Suite 1219: 1217 pass / 0 fail / 2 skip.

feat(intent): /trekplan halts without an approved intent; the approval is stamped on both paths
Veikart steg 1, økt 1 av 2. The three holes measured in the grounds:
(1) ## Intent / ## Goal were only checked for existence, (2) no approval
marker existed, (3) brief-approved was emitted only on the auto path.

- lib/validators/intent-approval.mjs: --check / --stamp. The marker is
  intent_approved_hash (sha256 over normalized ## Intent + ## Goal) +
intent_approved_at. Editing either section after approval → STALE.
- /trekplan Phase 1 (Read the brief, both --brief and --project): runs
--check and HALTS on BRIEF_INTENT_NOT_APPROVED / _STALE / _INVALID with
the remedy spelled out; an unrunnable check halts too.
- /trekbrief Phase 4h (before the Phase 5 fork, asked even in --quick):
  shows Intent + Goal verbatim, AskUserQuestion Approve / Revise / Leave;
only "Approve" runs --stamp. --stamp emits brief-approved, so the manual
(default) path records it; the auto path's own emission is removed.
  New mode /trekbrief --approve <project-dir> = Phase 4h alone.
- README, CLAUDE.md, command-modes, HANDOVER-CONTRACTS §Handover 1,
  jsonl-schemas (trekbrief-stats gains intent_approved).

Valgt ingen brief_version-bump fordi skjemaendringen er rent additiv (to
valgfrie felt) og kravet sitter i /trekplan — enhver produsents brief kan
godkjennes via /trekbrief --approve uten produsentendring. Valgt eget

What the marker does NOT prove (module header, command prose, contract):
same user, same machine — any session can stamp. A trace, not a signature.

Suite 1183 → 1201 (1199/0/2). Mutants M1–M5 (stale check off, check
always valid, gate line removed, stamp emits nothing, no normalization)
each fell ≥ 1 test. yardstick unchanged: RED, 1 of 3 countable.

1ca48e0cd0 release(v5.10.1): drop gemini-bridge from the pipeline; correct the T1 §6 PoC status
gemini-bridge is dropped by operator directive (three repetitions). The drop is
permanent, not a wait-for-SDK-upgrade state, so the change clears every LIVE
surface that could still steer a run toward the agent and leaves the historical
record alone.

Live surfaces cleared: agents/gemini-bridge.md deleted; trekresearch (bridge
launch block, --local help, high-effort always-on pair, stats record,
degradation list), trekplan, trekbrief, research-orchestrator (mode table,
agent table, prompting block, summary line), README (feature prose, mode table,
agent prose, mermaid EXTERNAL node, conditional legend, optional-MCP
requirement, --local section), CLAUDE.md, settings.json (the whole
trekresearch.geminiBridge block), both templates, architecture.md,
command-modes.md.

trekplan high-effort Adversarial Pass 2 now degrades EXPLICITLY: it emits its
section with status "unavailable, skipped" instead of failing or vanishing. A
high-effort plan carrying no Pass 2 marker is indistinguishable from one whose
Pass 2 crashed, which is the failure mode this wording exists to prevent.

gemini_used is deliberately KEPT as a vestigial trekresearch stats field pinned
to false. Removing it would break the observability export schema for existing
consumers, and the directive was about the agent, not the field.

Not touched: CHANGELOG history and the measurement/decision docs keep their
gemini references. They record what a past version did or what was measured
then; rewriting them is the same defect class as bumping a version string
inside a measurement doc.

Driven test-first. Five new pins in tests/lib/doc-consistency.test.mjs, verified
RED before the edits, including a KNOWN-POSITIVE CONTROL asserting the
historical records still DO carry gemini references — so the empty result on
live surfaces is a measurement and not a broken query (Verifiseringsloven
ansikt 4). Agent inventory 24 -> 23 (20 spawnable + 3 orchestrator reference
docs); the <example>-block floor moves 34 -> 32 because an agent legitimately
left the inventory, not because examples went missing from a surviving one.

Docs: docs/T1-cc26-delegated-orchestration.md §8 item 3 claimed both the §6
synthesis-agent PoC and the §5 bake-off were "designed but unbuilt". That was
written in S7 and falsified the same afternoon by S12, which ran the §6 PoC and
recorded Δ main-context (faithful flow) = 0.0%, NEGATIVE. The stale wording is
what caused the settled PoC to be re-ordered as new work on 2026-09-02, so it is
struck rather than deleted and §6 gained a RUN AND DECLINED status block. The
finding is structural, not stochastic: Phase 5 spawns the exploration swarm
foreground (trekplan.md:158,338-341), so the outputs are already resident in
main before Phase 7 — delegating only the Phase-7 digest evicts nothing.

Also measured 2026-09-03 (CC 2.1.259): claude -p --output-format stream-json
runs on subscription auth with no ANTHROPIC_API_KEY and now emits a
subagent_stats block, so S12's environment-block premise is half stale. Recorded
in §8 item 4. It lowers the cost of §5; it changes nothing about §6.

Suite 1041 (1039/0/2), up from 1036 by exactly the five tests added.

4e5523fb0a docs(trekresearch): deep-research engine is operator-only from CC 2.1.218; document the real window
Claude Code 2.1.218 changed /deep-research to start only when invoked
manually; from there the Skill tool refuses a model invocation outright
with `disable-model-invocation` (measured in a real `--engine deep-research`
run 2026-09-01 18:32Z - the SC3 fallback to swarm held). The engine was
built against 2.1.196 and the prose still promised a path Claude Code has
removed ("requires Claude Code 2.1.154+").

The flag is KEPT as an additive opt-in that never hard-fails. Prose only:
no lib/ change, no adapter-contract change, no default change, no version
bump.

- commands/trekresearch.md: flag bullet states the closed window
  `2.1.154 <= CC < 2.1.218`; the pre-gate gained an UPPER ceiling (still a
numeric comparison, not a string one) that skips straight to the fallback
with reason `disable-model-invocation`; step 3 names the Skill-tool
refusal as a known failure manifestation; step 4 lists the reason tokens.
- docs/command-modes.md + README.md: same window, one sentence each, on the
  reference row itself.
- CHANGELOG.md: Unreleased/Docs entry.

Tests first (Iron Law): tests/lib/doc-consistency.test.mjs gained a pin
requiring 2.1.218 + disable-model-invocation on all three surfaces, on both
reference rows, and in both load-bearing regions of the command (flag
bullet + pre-gate). Seen red on all three surfaces before the prose change.
tests/commands/trekresearch-engine.test.mjs unchanged and green.

New baseline: 1024 tests, 1022 pass / 0 fail / 2 skip (was 1023: 1021/0/2).

def6c05384 fix(research-loop-cap): floor the turn cap before the guard, not after
A fractional TREKRESEARCH_MAX_CONV_TURNS below 1 cleared the `n <= 0`
guard on its raw value and only then floored, so '0.5' and '0.9' became
0 and the budget became 0 x MAX_TOTAL_DIMENSIONS = 0: every turn denied,
the loop silently dead rather than bounded.

README.md:229 and docs/architecture.md:15 both promise that invalid
values fall back to 3. docs/command-modes.md:42 enumerated "empty,
non-numeric, zero, or negative" and happened to sidestep the case; the
enumeration is now exhaustive about it.

Measured before: '0.5' -> 0, '0.9' -> 0, '2.7' -> 2, '' / 'abc' / '-2'
/ '0' -> 3. Measured after: '0.5' -> 3, '0.9' -> 3, '2.7' -> 2, and
'Infinity' -> 3 (it is not a cap either).

A cap of 0 is not a narrower cap, it is an off switch. The tests pin
both directions: the fraction falls back, and allowTurn cannot report a
budget of 0 under it.

2e352a7dbb fix(trekresearch): gate Phase 4.5 on VOYAGE_STORM_ENABLED, not effort alone
CLAUDE.md claimed both STORM phases go inert when the flag is unset. Only
Phase 5 did: the flag check lives in research-loop-cap.mjs, and Phase 4.5
never invokes the cap — it was gated on effort: high alone. At high effort
with the flag unset, discovery still mined Phase-4 output and mutated the
dimension list, so `dimensions` diverged from `dimensions_baseline` and the
decline branch was unreachable for half the mechanism.

The code was the deviator, so the guard is fixed rather than the claim: the
Phase 4.5 skip-guard now names both conditions, with the reason inline.

Three surfaces scoped the flag to "the loop" and are corrected with it
(README, docs/command-modes, docs/architecture), plus the orchestrator phase
map. CLAUDE.md's claim is now true, but its stated MECHANISM was not — Phase
4.5's inertness comes from its own guard, not from the cap module — so that
sentence is corrected too.

New doc-consistency pin: the flag must be documented as gating both phases on
all four reference surfaces. Review finding 00a3af1a.

9dc50a2047 refactor(voyage): S31 — V15 trim plan-export to headless-only (decompose alias)
Drop the pr/issue/markdown variants from `/trekplan --export`. Claude Code
reformats a plan into a PR body, issue comment, or stripped markdown ad-hoc
on request, so a dedicated export path added maintenance without value.
Keep `--export headless` as a backwards-compatible alias for `--decompose`
and relabel it as the decomposition entry it actually is.

- commands/trekplan.md: Phase 1 parse rejects non-headless formats and sets
mode = decompose for headless; delete the Export phase; rename Phase 1.6 →
Phase 1.5 (Decompose); update the mode enum, argument-hint, and usage block.
- docs/command-modes.md, README.md: export row relabeled as a --decompose alias.
- tests/commands/trekplan.test.mjs: +2 V15 pins (variants gone, headless kept).

Non-breaking (plan D-register). Tests 731 (729 pass / 2 skip / 0 fail), bar

fe97f6172c docs(voyage): S24 — autonomy-gate truth-pass + guard pin (S21b doc/code drift)
operations.md:15 + command-modes.md described an autonomy surface that does
not exist in code. Truth-pass against lib/util/autonomy-gate.mjs (the source
of truth) closes three false claims found while auditing the autonomy surface
in S21 (devils-advocate-results.md S21b forward-pointer):

F1 (MAJOR) — fabricated state machine. operations.md claimed
      `idle → approved → executing → merge-pending → main-merged`; only `idle`
      exists. Real states: idle → gates_on|auto_running → paused_for_gate →
      completed (events: start/phase_boundary/resume/finish).
F2 (MINOR) — event-emit.mjs does NOT "record each transition"; it emits 3
      named lifecycle events (brief-approved, main-merge-gate, user_input) and
is decoupled from the pure, no-I/O autonomy-gate.
  F3 — `--gates {open|closed|adaptive}` is false: the CLI shim + all 4 command
      docs take a BOOLEAN `--gates {true|false}`. open/closed/adaptive is a
DERIVED gates_mode policy, /trekexecute-only, mapped from the brief effort
      signal (low→open, standard→adaptive, high→closed; trekexecute.md:1562/74/75).

Operator-chosen fix (S24): boolean-true representation + a gates_mode policy
note, applied to BOTH operations.md and command-modes.md (4 rows) — same
false-claim class, fixed in one pass (fix-errors-found-in-scope).

TDD: doc-pin in doc-consistency.test.mjs imports STATES from autonomy-gate.mjs,
forbids the fabricated names + the flag-enum, requires the real states. Red
first (failed on "merge-pending"), green after the truth-pass.

Tests: 725 (723 pass / 2 skip / 0 fail). plugin validate passes (1 accepted

987e847ea1 feat(voyage): S18 — framing-hardening (min-version gate + memory status + pre-2.2 doc + flagship hedge)
Closes devils-advocate audit #4–#6 (MAJOR×MED). Four additive, non-breaking
changes to the v5.5 framing-alignment machinery:

1. --min-brief-version <ver> gate (audit #4). Opt-in version floor on
   /trekplan + /trekresearch, forwarded to brief-validator as --min-version.
New opts.minBriefVersion warns BRIEF_VERSION_BELOW_MINIMUM (never blocks)
when a brief declares a version below the floor; trekreview exempt; absent
opt = no check. CLI shim parses --min-version and skips its value token in
   filePath detection.

2. memory_alignment.status field (audit #5). brief-reviewer now emits
   status: verified | n_a | contradictions so a score-5 N/A (no memory) is
   distinguishable from a score-5 verified-aligned brief — the score≥4 gate
passes in both, status reveals whether the wrong-premise defense ran.

3. Document pre-2.2 = zero framing enforcement (audit #4). HANDOVER-CONTRACTS
§Handover 1 now states the producer-elective hole + two remedies. Also
fixes a stale "current is 2.1" line (current is 2.2).

4. Soften flagship overselling (audit #6). CLAUDE.md Context-Engineering
principle now hedges that main-context relief is asserted-by-design, not
measured (T1 PoC found Δ≈0); README carried no false claim to fix.

TDD: 8 new tests written failing-first (5 validator, 1 trekbrief status pin,
2 doc-consistency cross-file pins). Suite 691→699 (697 pass / 2 skip / 0 fail).
No flagship prose-pin added (deliberate, per S19 anti-bloat).

9fb536e2d8 feat(voyage): S11 — NW2 part B, integrate opt-in --workflow flag
Make the bake-off-validated Workflow substrate (Arm B) reachable behind an
opt-in --workflow flag for /trekreview. Default Phase 5-6 path stays prose
to preserve the lower portability floor; --workflow raises the consumer
floor to Claude Code 2.1.154+ (the Workflow tool).

- arg-parser: --workflow added to trekreview boolean flags
- commands/trekreview.md: flag row + Phase 5 substrate-routing gate +
new section 'Phase 5-6 via the Workflow substrate' (invocation contract,
S10 gotchas, bake-off citation, auto/bypass residual as Known limitation)
- docs/command-modes.md: --workflow row in /trekreview table
- routes to existing scripts/trekreview-armB.workflow.mjs (byte-identical to
the S10 part-B POSITIVE build); integration is pure routing, no script change
