A new skill, voyage:reviewing-adversarially (contract S4): no praise, no
finding without a source (file:line or a quoted line), no downgrade without
a reason, summary-only said and not judged, a Declined to Judge list, claims
checked against the diff rather than the report, axes kept separate.
Red baseline first: six review runs by helpers without the skill (two
invented fixtures) had no separate Declined to Judge list in 6 of 6, an
unsourced finding in 6 of 6, praise in 2 of 6 and a verdict on something
unmeasured in 4 of 6. With the skill: 6 of 6, 0, 0 and 0.
- brief-conformance-reviewer, code-correctness-reviewer, review-coordinator
and scope-guardian preload it through skills:; the 21-line "Interaction
Awareness - MANDATORY OVERRIDE" block copied into three of them is gone.
Each agent keeps its own role.
- The two reviewers and scope-guardian end with their own Declined to Judge
list; /trekreview names the skill at spawn and passes the reviewers'
lines to the coordinator, which copies them into review.md as prose
(the JSON declined_to_judge array and the verdict are unchanged).
- Eval cases 3 / 2 / 1; pins in tests/lib/skills-review-prose-pins.test.mjs.
- tests/fixtures/jsonl-schemas.md: the trekreview-stats line_ref moves with
Phase 8 (636-686).

An engine's brief-conflict mark could lower a reviewer agent's MAJOR: ingest kept an engine field a reviewer wrote on its own finding, and on a severity tie the survivor depended on payload order (engine first gave ALLOW, reviewer first WARN). ingest now takes engine from the payload source; Pass 1 and the workflow dedup keep the reviewer-agent finding on a tie. Prose (coordinator Pass 1, Handover 6) follows. Decision 13, operator 12.10.

Round 8 (0f400d3..29b4ac9, BLOCK: 1 BLOCKER, 12 MAJOR, 1 MINOR). Each fix
began with a test that failed on 29b4ac9 (13 tests, all red there); suite
2221 (2219/0/2).
- BLOCKER: a backticked check whose trailing comment holds a quote, $,
backtick or backslash is refused, not dropped. The comment was never
screened, while the executor hands the Verify span as written to red
proof, the test lock and the done line inside --cmd "...", where a $(...)
runs and a quote ends the argument. trekplan's text and the reviewer's
NOT RUN table say so.
- A span that is only a comment is not-a-command, not a placeholder. The
dropComment text says the allowlist still refuses a word with a # in it.
- The criteria runner, comparison-gate and end-state-gate use isMain: they
did nothing and exited 0 under a symlinked path. The test now refuses any
process.argv[1] comparison outside lib/util/is-main.mjs.
- Files: in a field with backticked paths, a plain word counts only before
the entry's description (a dash, or a colon after the path); a name with
no slash only first in its entry or after and/or/+/&. "mocks
fs.readFileSync", "runs on Node.js 22.x" or "no change to package.json"
no longer make a tests-only step look like it builds something.
Lowercase makefile, rakefile and justfile are build files. Measured on the
520 Files: fields in 48 plans of the marketplace and the test repo:
3 fields read differently (two notes no longer read as files, one
duplicate name dropped), tests-only changed in 0.
- The test lock compares --cmd without its trailing comment, and the test
lock and the done line read a stop with stoppedBy, the one reading.
red-proof drops its second reading, spawnError.
- release-check reads a relative --review in --repo or, when only that one
exists, from the current directory as 6.3.0 did; two different files are
exit 2, never guessed. The runbook says so.
- One trailing-json reader: a closing fence may be longer than three
backticks, in the block reader and the verdict gate alike, and the review
validator and the synthesis digest use it, so three backticks inside a
JSON string no longer hide a BLOCKER from the remediation-step check.

Round 7 (516dffa..45aa9e1, WARN: 12 MAJOR, 0 BLOCKER) and the fence defect
found while writing its review.md. Each fix began with a test that failed on
0f400d3 (v6.3.0); suite 2208 (2206/0/2), 14 tests more.
- Tests-only (operator 2026-10-11): the plan's Files: alone decides, and the
text says so (red-proof, trekexecute Sub-step B, step-implementer,
HANDOVER-CONTRACTS). red-proof no longer reads the step's diff, so a change
already in the tree when the step began no longer counts against it, and a
plan placed outside .claude/ reads the same. The round-6 test that had the
diff overrule the plan is rewritten to the new rule.
- The Files: reader keeps a plain file beside a backticked path
(", Dockerfile", "the CLI entry src/cli.mjs", "and Makefile"). Measured on
the 520 Files: fields in 182 plans: no field reads differently.
- Red proof reports a command its timeout killed as killed after the timeout,
not "could not start"; the release check tells a timeout from output past
its buffer and from another signal. One reading of a stop
(criteria-runner stoppedBy) for red proof, the criteria runner and the
release check.
- A trailing `# comment` is dropped in a backticked check too, and in a
command passed as written, so the words screened are the words run.
- The release check reads a relative --review in --repo.
- The 15-minute timeout is defined once (criteria-runner.mjs).
- The findings check reads each cited file once per validation.
- One isMain helper (lib/util/is-main.mjs) for every CLI: the 31 copies of
the realpath guard and the 20 modules still on the file:// guard, which
exited 0 having done nothing under a symlink or a path with a space.
- The Bash guard writes its no-one-to-ask refusal once, on a direct condition.
- The findings check ends a ```json block only at a fence on its own line, so
three backticks inside a JSON string no longer end it (also in 6.2.0).
- Brief-conformance notes: a test for test-lock green on exit 127 (it
passed at once; it fails when 127 is read as could-not-run); the prose now
names the git revision `@~1` as the one leading-@ exception.
Not changed: the example plan's steps 1-2 keep their tests-only warning
(operator decision 2026-10-09; the r5 test records it). Brief note 4 (a
Regression: whose runner is not installed goes to the fix rounds) is a
routing question for the operator.

Round 6 (v6.2.0..c3641ff, WARN: 11 MAJOR, 3 MINOR; 0 BLOCKER). The round-5
fixes hold (code-correctness-reviewer); three of the new MAJOR were side
effects of running checks with no shell. Each fixed behind a test that
failed first on c3641ff.
- git checks stay in the repo too: no path that climbs out with .., starts
at / or reads <rev>:../x, and no --no-index (both reviewers).
- A trailing `# comment` on a shell-fence line is dropped: with no shell,
its words made an honest check NOT RUN, among them the first check of the
shipped example plan.
- A glob reaches `node --test` only on Node 21 and later, which expands it
itself; on an older Node it is refused with that reason (engines >=18).
- With no shell, exit 126/127 is the test program's own: red proof reads it
as a red (the control run still tells an environment fault apart), the
done line and the test lock as a failure.
- A runner that could not start (not installed) is NOT RUN in the criteria
runner and not-run (exit 2) in regression-check, as red-proof and
done-line already said; never a red suite.
- Tests-only: a backticked `Makefile` alone in its entry is a path, and red
proof confirms the plan's tests-only reading against the step's own diff
(the altitude finding: the diff is a fact, the Files: prose is not).
- release-check stops a suite that never ends (15 minutes) and reads it as
not measured (exit 2).
- The two lines this release added to pre-bash-executor.mjs are ASCII.
- MINOR: git and the other runners share one plain-word pattern.
Not in this round: MINOR f00a2eb8 (shared norm/ticked-path and isMain
helpers) and MINOR on plan-review-dedup's repeated tokenizing.
Suite 2194 (2192/0/2).

Round 5 (v6.2.0..d37e538, WARN: 11 MAJOR, 2 MINOR). The patching did not
converge (the same two places every round), so the operator chose a
structural fix on 2026-10-11. Each change behind a test that failed first.
- A check runs with no shell. criteria-runner's runCommand starts the runner
with the very words the screen checked (spawnSync(words[0], words.slice(1)),
without NODE_TEST_CONTEXT); Phase 7 criteria, Regression: (its exec copy is
gone, MINOR 0e98d5c7), proof of red, the done line and the test lock all
run through it. No quote, glob, tilde or variable can rewrite a checked
word any more. The screen stays and says why: a word that means something
else in a shell (quote, backslash, $, brace, leading ~) is refused; a glob
reaches only node --test, which expands its own patterns (sh read
`tests/.?/` as `tests/../`); no path climbs out with .. or starts at /;
a word starting with @ is refused (pytest reads @file as more arguments).
- The K6 plan-file sweep has v6.2.0's own Files: reader again (516dffa):
backticked paths, and plain words only in a field without them. The
shared reader from round 3 planned directories a description named and
dropped paths v6.2.0 planned. fieldPaths stays for the tests-only check,
with a path test on spans (`parse()`, `p.parse()` are no paths), plain
descriptions ignored when the field has backticked paths, dotfiles,
backslash paths and glued spans read.
- Skill frontmatter reads a value as YAML does: trailing space is nothing,
and a comment ends a plain value, a flow list or a block indicator
(`model: opus # pinned`). A comment that cuts a plain description, or is
the whole value, is still a hazard.
- release-check reads the diff with --no-renames: a test moved into an
allowed place after the review shows as removed.
- The example plan's step 2 heading no longer says "must FAIL initially";
steps 1-2 keep the tests-only warning (operator 2026-10-09, 73262b6).
Open: MINOR c1fef1ad (one shared isMain helper) is not in this round.
Measured: 0 of 463 Files: fields with backticked paths (520 fields in 182
plans across the marketplace repos) carry another path-like word.
Suite 2185 (2183/0/2).

1. A step of only tests can never be proven red. One reading,
stepTestsOnly (lib/parsers/plan-schema.mjs, test paths from the new
lib/util/test-path.mjs): the plan validator warns PLAN_STEP_TESTS_ONLY,
red-proof --plan/--step adds tests_only to a not-red-at-base, and
/trekexecute and the step helper send such a step straight up to the
plan instead of into fix rounds that cannot succeed. /trekplan, the plan
template and plan-critic (dimension 8, still 12) say the test belongs in
the step that builds the behaviour.
2. Timing tests under load: the hook helper's kill limit is 30 s and per
call overridable; SC #14 passes on the fastest of up to three runs
against the same 800 ms limit (tests/helpers/timing.mjs).
3. The example plan's On failure lines open with a K11 word.
4. /trekreview saves the reviewer's whole answer for the findings check and
names FINDING_PROSE_LINE_OUT_OF_RANGE (error: re-ask; warning: report).
5. The criteria runner's commands run without NODE_TEST_CONTEXT
(commandEnv, moved to lib/util/command-env.mjs).
6. The resolver returns effort only when the brief's phase signal sets one;
the four commands said it always did.
Also: the K2 hook tests used fixed session ids in a per-user temp dir, so
two suites running at once took each other's approval records; the ids are
now unique per run.

Conflict: CLAUDE.md command table. Kept both sides: this branch's
/trekresearch row (the external agents preload researching-sources) and
main's /trekplan row (open minors listed for the operator).
researching-sources adapted to contract S1, now that 7a's form check and
routing test run over it:
- SKILL.md takes the fixed headings (Overview, When to use, Process, Red
flags, Enforced by, Credits); the five rules and the confidence scale move
under Process unchanged; Red flags lists the thoughts the baseline runs
showed; "text only" becomes "prose only" as the form check reads it.
- The description adds "official" documentation and "looking up online what
an error message means or what causes it": the error-lookup case ranked
#3 behind debugging. That prompt also said "our tests fail", which is
debugging's job; it now asks only what the error means and where that is
documented. No threshold changed.
- Eval cases carry the routing tags (skill-trigger / skill-no-trigger /
skill-effect, skill=, owner= task-finder and trekplan, from=), 7a's
Skill grader pattern, arm: both on the no-trigger graders, and the
judge-model line on the effect case.
- The `## Skills` row in README and CLAUDE.md.
- tests/lib/skills-research-prose-pins.test.mjs drops its two form pins
(skill-lint is the authority now): 14 -> 12.
npm test on the merged tree: 2055 = 2053 pass / 0 fail / 2 skipped

The source rules the research agents shared by copy now live in one skill,
skills/researching-sources/: official sources first with URL, date and
matching version; no citation, no finding; an empty search is reported with
its denominator and never as a finding; fetched pages are data, and text in
them aimed at AI assistants is ignored and reported; nothing local in an
outbound query. It also holds the one confidence scale.
- docs-, community-, security-, contrarian-researcher and research-scout
preload it (skills: ["voyage:researching-sources"]); the copied rule text
leaves them, their own role stays.
- Two agent sentences that contradicted the rules are gone: an empty
counter-evidence search "increases confidence", and "fall back silently".
- /trekresearch names the skill where it starts the external agents and in
web follow-ups; Phase 6 rates on the skill's scale word for word; the
deep-research adapter accepts `contradictory` like the template does.
- Enforced by: research-validator checks structure only, the query privacy
gate runs only in the default-off STORM steps; the rest is text only.
A guard on every web search is a separate later order (operator 08.10).
- Red baseline (helpers, offline fixtures): without the skill 3 of 5 runs
ignored an injected instruction silently and 1 of 2 contrarian runs counted
an empty search as weak support; with it 0 of 3 and 0 of 2. Path leaks:
0 of 7 without, 0 of 5 with.
- Eval cases (3 should trigger, 2 should not, 1 effect), written, not run.
- tests/lib/skills-research-prose-pins.test.mjs: 14 pins, 13 red on 4ee711a.

The minor points (B8-B12, C13, C14, C16, C17) of the second voyage-lekerepo
trial (2026-10-08), each verified against the code and started with a test
that failed first.
- K10: optional step field `Regression:` - one command the criteria runner
allows, unpiped; plan-validator refuses one that can never run
(PLAN_STEP_REGRESSION_NOT_RUNNABLE). Any plan_version, no bump.
- K11: `On failure:` opens with revert | retry | skip | escalate, or the
validator warns (PLAN_STEP_ON_FAILURE_UNKNOWN); onFailureAction in
plan-schema.
- B8 (operator decision 2026-10-08): minors open after the last critic round
are listed numbered in the Phase 10 summary; the ones the operator picks are
fixed and get one more round (--extra-round when the 3 are spent).
- B11: plan-review dedup merges two reviewers' findings on the same file:line
with the same content (trial pair: Jaccard 0.44, overlap 0.83); the Jaccard
threshold for every other pair is unchanged.
- B12: /trekbrief Step 4c says the reviewer runs in the background and is
waited for, not "foreground, blocking".
- C13: the Phase 4 size count includes .mjs, .cjs, .mts, .cts.
- C14: --evidence shows the first output line on PASS rows.
- C16: findings-schema checks path:line citations in finding prose (error) and
in the prose around the block (warning) against the file.
- C17: git-historian gets the brief, never proposes changing a frozen file,
and names the remote from `git remote -v`.

The workshop (ordre 7a, contracts S1 and S2): a form check
(lib/skills/skill-lint.mjs) that follows Claude Code's own skill fields,
and a model-free routing test (lib/skills/routing.mjs) over every skill,
the 7 commands and the 21 spawnable agents, reading the eval cases
themselves (evals/skill-*, 3 fire / 2 quiet / 1 effect, tagged with the
event they came from). Thresholds pinned, never lowered.
The two skills were written against failures measured first in helpers
without them: red only as an import crash, the whole suite run from a
hand-made file list instead of the repo's own command, and an
unreproduced fix shipped under authority pressure. step-implementer
preloads both through its skills: line; no tool list change.

- The pre-compact hook read only a checkpoint that started with `git commit`,
so every plan in the new `git add … && git commit` form rebuilt nothing, and
an empty match also skipped the session-state refresh. It now finds the
commit anywhere in the span and always refreshes; first test for the hook.
- A sub-bullet that carries its own command stays a criterion (folding it
left the command unrun); the shallowest bullet sets the base indent.
- Only a MINOR/SUGGESTION /code-review finding outside the range is released
from forbidding ALLOW: a MAJOR one may be a break cited at an unchanged
caller.
- `reuse` is an improvement category; the session-spec template stages too;
a negated bash 3.2 constraint, an env prefix and `bash -c` are handled.
- Stale comment, Declined-to-Judge wording and rule-key counts brought in line.
Tests fail on 7c11c2a first (9 of 24).

voyage-lekerepo trial 2 (2026-10-08), report section A; the operator decided
they are fixed before the next release:
- A1: a /code-review finding whose category is simplification, efficiency,
style or readability enters as CODE_REVIEW_IMPROVEMENT (MINOR), not MAJOR.
- A2: recommended_action is the engine's own fix when transcribed, otherwise
a test-first step; never the failure scenario dressed as a remedy.
- A3: a /code-review finding outside the exact range it was given is declined
and listed, but no longer holds ALLOW back; /security-review's still does.
- A4: the manifest must_contain recipe uses grep -E -e ... -- so a pattern
like --sep is not read as an option.
- A5: indented sub-points belong to their criterion (counted), and a
criterion with sub-points and no command warns BRIEF_SC_SUBPOINTS.
- A6: a brief requiring bash 3.2 whose criterion runs plain bash warns
BRIEF_SC_BASH_NOT_PINNED; /trekbrief proposes /bin/bash.
- A7: the plan template's and /trekplan's checkpoints stage the step's files.
Tests fail on 5f9faee first (tests/lib/trial2-fixes.test.mjs, 11 of 14).

Independent read-only review of c56f852..2d29966 (2026-10-08), each defect
reproduced by the reviewer; each new test fails on that code.
- test-lock green ran ANY allowlisted command: `green --cmd "git status"`
marked a red step green and the stop guard let it stop. The lock now
stores the step's test command (`lock --cmd`); `green` runs only that
one (a different --cmd, or a lock without one, exits 2). trekexecute D1
calls `green` with no command, which also removes the quoting hazard.
- plan-gate --accept-open worked at round 1 and passed the gate; it now
refuses while rounds are left.
- findings-schema --repo-root swallowed a following flag, or took a root
that does not exist, and silently skipped the line check: exit 2.
- brief-review-stamp hashed only the body, so a changed `framing` or
`research_topics` stayed "current"; it now covers the reviewed
frontmatter too (only the post-review stamps are left out).
- red-proof red_kind read an assertion message naming SyntaxError as a
load error; the signatures are now specific.

voyage-lekerepo trial (2026-10-06) point 6, operator decision 2026-10-08
(«samme skala, og du spør»): the critics rated a non-discriminating test
minor in round 3, there was no way to fix it after the last round, it was
built, and /trekreview called the same weakness MAJOR (MISSING_TEST).
- plan-critic, scope-guardian: rate on the review catalogue's tiers; a test
that cannot fail on a wrong implementation is major, never minor.
- plan-gate: an open MAJOR stops (PLAN_GATE_OPEN_MAJOR: revise and run the
next round; PLAN_GATE_OPEN_MAJOR_FINAL after the last round: ask the
operator). --accept-open records "build as is" after their yes, bound to
the round and the plan hash; never for a blocker.
- trekplan Phase 9: REVISE after the last round asks fix now (one extra
round) or build as is, instead of carrying the majors silently.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
voyage-lekerepo end-to-end trial (2026-10-06), report points 1, 5, 11:
- engine-findings: recommended_action is "Fix <file:line> so this no longer
happens: <failure scenario>", not "Resolve: <title>" (8 of 8 engine
findings were set aside as actionability:empty); the coordinator never
sets an engine finding aside for Actionability.
- findings-schema --repo-root: a cited line past the end of an existing
file is FINDING_LINE_OUT_OF_RANGE, so Phase 5's bounded re-ask fixes it
before the coordinator sees it. The reviewers' instruction said "the
actual line number from the diff"; it now says the file's own line,
never the patch's.
- fill-finding-ids.mjs: the coordinator (no shell) writes @@ID_<n>@@ and
trekreview Phase 7 computes them over (file, line, rule_key); an
unresolved placeholder or a wrong 40-hex id fails. The 4-field id with
`title` the docs named is gone; the code always used three.

voyage-lekerepo end-to-end trial (2026-10-06), report points 2, 3, 4, 9:
11 of 13 brief criteria were NOT RUN at review with no warning in /trekbrief;
a plan `git grep` criterion passed validator, gate and three critic rounds,
then felled the run in Phase 7; the End-to-end field and Phase 7's text
search never met; a prose Verify left the test command undefined.
- criteria-runner: runnableCheck() screens a declared check without running
it (same allowlist; an npm run script is not looked up at write time);
/bin/bash tests/<script> allowed; the plan report carries end_to_end,
verified when a passed criterion runs a step's End-to-end command (the
older text form still counts).
- plan-validator 1.8: PLAN_VERIFICATION_NOT_RUNNABLE,
PLAN_STEP_VERIFY_NOT_RUNNABLE, PLAN_END_TO_END_NOT_RUNNABLE,
PLAN_END_TO_END_NOT_VERIFIED (the gate inherits them).
- brief-validator: BRIEF_SC_NOT_RUNNABLE warning; /trekbrief offers a
tests/ script; brief-reviewer no longer proposes unrunnable commands.
- trekexecute: Phase 7 copies end_to_end from the runner; Verify's test
command is its first backticked span.
Measured on the trial's own brief and plan: 11 warnings (the 11 NOT RUN),
and the plan stops on V5 git grep and the unlisted End-to-end command.

An independent review of abc3083 found 15 defects, the two worst verified
in scratch repos. Each fix started with a test that failed.
- Proof of red could be "proven" by the environment: a start point
unpacked with git archive had no node_modules and no .git, so any test
failed there. The start point is now a git clone --shared checked out at
start_sha, gitignored top-level entries are linked in, and a control run
(start point + the change) must pass, or the result is not-run.
- The Test first file was never committed (the template lists it apart
from Files:) and leaked into the next step's diff. The step's scope is
now Files: + test files, staged at the checkpoint.
- New files outside Files: were invisible to every check. Every untracked
file is registered with --intent-to-add; a revert unregisters them.
- The floor guard paired renames (foo.test.js -> .bak read as clean) and
missed skipIf and node's t.skip/t.todo; prose files no longer count as
stubs.
- Phase 7.5 audited from the last step's start_sha; it now uses the run's
session_start_sha.
- The done line is written after the checkpoint and checked with --commit.
- fix_rounds never passes 5; attempts are never reset; re-control keeps
the spec/quality verdicts and earlier floor-guard rulings; the closed
stop list maps every existing stop; failed vs stopped made consistent;
the controller's effort is said, not assumed.
Tests: 1470 (1468 pass, 0 fail, 2 skipped).

Wave 1, order 3 (P3 plan mode as the start, P2 design as its own part, P9
plan side). Contracts K3-K6 for the later merges.
- plan_version 1.8 (1.7 plans stay valid): Test first on every step, one
End-to-end ("run the thing") step, optional Interface in / Interface out /
Review focus fields, a ceremony class in the header (absent =
architectural), and for architectural plans a ## Design part: module map
without cycles, architecture rules that name their enforcer and the step
that proves them, a requirements check asked as keep / change.
- K3 lib/validators/plan-gate.mjs: the critic verdict is recorded INSIDE the
plan (Claude Code's plan approval hands a hook only the plan file's text),
bound to the hash taken before the reviewers read it; 0 go / 1 stop / 2
could not run; at most 3 critic rounds, then stop and ask.
- K5 lib/verification/arch-rules.mjs: runs the target repo's own
dependency-cruiser or import-linter (installs nothing; 2 = not measured,
never green); --prove --rule shows pass -> fail on a planted violation of
that rule -> pass. Verified against dependency-cruiser 18.5.0 and
import-linter 2.15; madge 8.0.0 left out (missed a planted .mjs cycle).
- K6 lib/verification/plan-scope-sweep.mjs: changed files no step names.
- /trekplan Phases 8-10: design part, bounded critic loop, the gate before
"execute", a plan-mode path through ExitPlanMode (not yet run end to end
inside plan mode). plan-critic 10 -> 12 dimensions; scope-guardian checks
the ceremony.
- Fixed on the way: plan-validator and plan-review-dedup used the
file://argv[1] main guard (no-op under a symlink or a space); CRLF plans
failed every manifest; the dedup dropped finding severity.

Operator decision 2026-10-06. Before, a WARN that existed only because a
reviewer or engine never reported (e.g. /security-review without an origin
remote) mapped to verdict-gate exit 0, so a review nobody performed in full
was green. Now `missing-reviewer:` / `unattributable-payload` in the JSON
block's allow_blocked_by is exit 2 (could not decide, never green); a WARN
for a MAJOR or a set-aside finding still passes with its reasons. A 1.1
review must carry allow_blocked_by.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
An adversarial read of d88245c reproduced 15 problems; all fixed here, each
with a test that fails on the old code.
- verdict-gate: a greener verdict must be backed by the LAST fence being a
`json` block of the right shape, with no BLOCKER in the JSON (any case),
in `## Findings (BLOCKER)`, or a differing finding-id list; two verdict
lines and more than one file argument are exit 2. ({}, null, lowercase
"blocker", a trailing ```jsonc and a later json fence all gave exit 0.)
- engine-findings: a transcribed "narrower" key can no longer lower an
engine tier; absolute paths under --repo-root match the scope; "12" lines
parse; an empty scope is an error; out-of-scope engine findings are
unverified (`engine:out-of-scope`) and forbid ALLOW.
- recurrence: the catch-all engine keys and keys normalised from unknown
ones are never proposed; the same unfixed finding in two iterations is not
a recurrence; proposals are validated before they are offered; \r/U+2028
and damaged markers are refused; writes go through a symlinked CLAUDE.md.
Prose points --repo at the session directory, not the git toplevel.
- Workflow substrate: engines are counted from what arrived, and the
verdict's floor is recomputed from the coordinator's findings; now tested
by running the script with stubbed Workflow globals. Two literal NUL bytes
written as \u0000 (git saw the file as binary).
- Trimming keeps Pass 1's "Also flagged by" provenance; `## Declined to
Judge` and remediation steps are required from review_version 1.1 only
(N-1); the zero-diff review now carries the JSON block the gate needs.

/trekexecute no longer builds steps itself. Each plan step is built by one
fresh helper (new agent step-implementer: opus, effort high, no Agent tool)
and judged by one controller (a general helper on opus) that reads the
diff from a recorded start_sha and gives two verdicts, spec and quality.
Fix rounds go up, maximum 5: three fixes by a fresh helper, then back to
the plan, then to the operator. No cheaper-model ladder.
Proof is a command, never the model's word:
- lib/verification/red-proof.mjs: the step's test must fail at the start
point and pass at the result (exit 0/1/2; 2 is never green). Reproduces
the 02243c6 red-first measurement (6 of 55) through the shim fixture.
- lib/verification/test-integrity.mjs: the floor guard on the diff
(removed asserts, skipped/deleted tests, muted checks, stubs, loosened
thresholds). Over the last 150 commits: 149 clean, 1 real finding.
- lib/verification/done-line.mjs: DONE step N only when the test command
exits 0.
Test first is required from plan_version 1.8. The test lock (K8) is used
through its CLI and reported as not-locked while it is missing. A run
stops only on a closed list of four classes; every other call goes into a
decision log with its cost if wrong. Without a passing run-the-thing
criterion the result is not verified end to end, never green.
Handover 5 gains optional per-step start_sha, report_path, red_proof,
test_lock, fix_rounds (0-5) and run-level decisions and end_to_end; old
progress files still validate and resume.
No isolation on the helper: measured, a worktree-isolated subagent
branches from the default branch (02f320f), not the executor's HEAD
(2c89a6b), so it would miss the earlier steps.

Ordre 5 (wave 1, P11 + P5), review_version 1.1.
- K7: `verdict: ALLOW | WARN | BLOCK` is required in review.md (strict
error, soft warning for 1.0 reviews). New CLI lib/review/verdict-gate.mjs:
0 = ALLOW/WARN, 1 = BLOCK, 2 = could not decide (never green); a verdict
greener than the file's own BLOCKERs or JSON block is exit 2. Realpath
main guard (the old file:// guard exits 0 under a symlink - measured).
- No finding is struck for length: succinctness trims only MINOR/SUGGESTION
text (catalogue tier), keeps BLOCKER/MAJOR whole, counts trims; the
coordinator lists what it declined to judge.
- Claude Code's /code-review and /security-review run as the engine via the
Skill tool (never --fix/--comment/--post/ultra); lib/review/engine-findings.mjs
turns their findings into a reviewer payload; two catalogue keys
(CODE_REVIEW_DEFECT, SECURITY_REVIEW_VULNERABILITY), catalogue 12 -> 14.
An engine that did not run forbids ALLOW.
- Every BLOCKER is written as a numbered `### Step R<n>` with its source
finding (strict error REVIEW_MISSING_REMEDIATION_STEP).
- P5: lib/review/recurrence.mjs - a finding class in two review.md files
(current + git history, no new store) becomes one proposal, routed by the
catalogue's memory_route; written only between Voyage markers in
.claude/rules/voyage-review-lessons.md under a size budget, CLAUDE.md gets
a pointer only; the operator approves every line (Phase 8.7).
- Workflow substrate: catalogue enum in step; a missing reviewer or engine
forbids ALLOW there too (it returned ALLOW on zero findings before).
- Fixtures review-run-A/B said WARN over a BLOCKER; now BLOCK.

Operator rule (2026-10-05): standard is Opus 5.5 at reasoning effort High. Four retrieval agents (architecture-mapper, dependency-tracer, git-historian, task-finder) pinned effort: medium in frontmatter, which overrides the session level, so every /trekplan exploration ran part of its swarm below the floor. They came from fdd3ad8 on the premise that unpinned agents inherit high; e4e37f9 corrected that (unpinned inherits the session; Opus 5.5 defaults to medium), so the reason for medium was already gone. All four now pin high; the other agents keep following the session (operator's choice). docs/profiles.md (one bullet for the eight pinned agents), CLAUDE.md and README.md updated; the model-default sentence stays. Dated decision records and CHANGELOG untouched.
Red: 4 of 5 new tests fail on the pre-fix files - effort floor over all agents (offenders: the four), the four pin exactly high, no live doc says retrieval runs at medium (hits docs/profiles.md:76, CLAUDE.md:85, README.md:806), profiles.md names the four as high. Known-positive control green on arrival.
Verify 153/153 (agent-frontmatter, doc-consistency). SC9 probe: npm test green under a scratch HOME with CLAUDE_PLUGIN_DATA empty, nothing under its plugin data dir. Suite 1393 (1391/0/2).
The change is repo-true, not installed-true, until a release.
Voyage order 20261005T104513Z-1250514346, plan step 5/5.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Operator decision 27.09.2026: Voyage works the PM way, so nothing in it
starts Claude Code. Work that needs another session becomes an order plus
a starter line the user gives a session they open themselves; Voyage
never starts the session.
/trekexecute's multi-session path (Phase 2.6) now prepares one wave at a
time: worktree per session, then lib/util/session-orders.mjs writes one
order per session (worktree, steps, done-condition, starter) plus
orders/wave.json, and the run stops. Running the same command again reads
each session's progress file (status mode), merges sequentially with
push-before-cleanup as before, and prepares the next wave. Phase 2.5 skips
the destructive pre-flight while a wave is in flight. Phase 7.6 recovery is
handed over the same way. Hard rule 21: never start Claude Code.
Removed with it, because they existed only for launched children: the
ANTHROPIC_API_KEY billing check (and its dry-run line and Hard rule 1
exception), the hooks-fire version precheck, the safety preamble, the
scoped-tool/turn/budget flags, templates/headless-launch-template.md, the
decomposer's launch.sh (now orders.md), /trekplan's "launch", the README
billing and MCP tuning sections, and scripts/q3-cache-prefix-experiment.mjs
(an experiment that spawned the binary; its results stay in docs/).
/trekresearch's deep-research pre-gate no longer runs the binary for its
version; the post-hoc check was already the authoritative guard.
Chose orders + starters over in-session subagents because the order's done
criterion asks for exactly that, and because /trekexecute disallows the
Agent tool on purpose (Hard rule 10); reopening that is a separate change.
Gate: 54 -> 0 hit lines outside EXEMPT. npm test 1250 (1245 pass, 0 fail,
5 skipped: 3 are the retired-reference scan without its local list).
verify.sh 7/0. The fixture run (tests/lib/session-orders.test.mjs) writes
two orders and wave.json for wave 1 and leaves a trap binary untouched.

A denylist in front of /bin/sh is whack-a-mole. Measured 2026-09-18, end to
end through both screens: 5 of 11 named evasions ran with real effect - a
`command` prefix reached git, an escaped `rm` inside a shell fence deleted a
directory, `find -delete` deleted a file, `>|` and `tee` wrote outside the
working tree, a python one-liner deleted the whole tree - and 19 of 28 got
past the refusal list on its own. Every quoting, aliasing and indirection form
of the shell is another mole.
So the screen is now an ALLOWLIST. A criterion runs only when its first word
is a known test runner (npm test, npm run <script package.json declares>,
node --test, vitest, jest, pytest, python -m pytest, uv run pytest,
cargo test, go test, make test, bash <script under tests/>, a read-only git
subcommand) AND the command carries no shell operator and no newline.
Everything else is NOT RUN with the reason said out loud: never run, and never
reported as a failure either - an absent measurement is not a finding. That
also closes the smaller hole in the same file: a bare word a sentence merely
names (`whoami`, `login`, `package.json`) is no longer executed, because it is
not a runner.
REFUSED_BY_POLICY is gone with the list that produced it; a command outside
the allowlist is `unrunnable`, which in plan mode still fells the run and in
brief mode is reported to the reviewer as an absent measurement.
What the allowlist deliberately does NOT do, said in the file and in the
reviewer's rubric: it is not a sandbox. `npm test`, `npm run <script>` and
`make test` run whatever the repo's own package.json/Makefile says they run,
including a script that pushes - that is the repo's responsibility. And it
rejects honest commands too: an env prefix, a project's own binary, anything
piped. A check that needs one of those is declared through
`bash tests/<script>.sh`, the documented way in.
Red first: 6 of the new tests fail against the previous runner (measured with
an always-allow shim so the module still loads), including the end-to-end one
where the canary directory was deleted and files were written outside the
tree. The fixtures move from `true`/`false` to two allowlisted shell fixtures,
because `false` is no longer a runner - the fail case must still be a real
non-zero exit, not an unrun criterion.
Suite 1148 (1146/0/2).
Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>
/trekreview now runs the commands a BRIEF declares, and a brief is an artifact
that can arrive from outside the repo. Measured 2026-09-18 on 6cafb4c: the
executor denylist stopped a download piped into a shell, but the remote-writing
git subcommand and a recursive delete of a path both RAN. The denylist screens
catastrophe (root deletion, fork bombs, mkfs); it was never meant to screen an
artifact under review.
A second screen, in the runner and ahead of the denylist, refuses four classes:
- a remote-writing git subcommand. The subcommand is found by walking git's own
options (`-C`, `-c`, `--git-dir`, ... take a value), so `git status` and
`git log` still run and `git -C sub push` does not.
- a recursive delete: any `rm` carrying `-r`/`-rf`/`--recursive`. A plain
`rm build/artifact.txt` still runs.
- a download piped straight into a shell (also caught by the denylist; pinned
here so the runner does not depend on another file for it).
- a write outside the working tree. `/dev/null`-class devices are fine, and so
is anything under the working tree; `~/...`, an absolute path elsewhere, and
a target carrying an unexpanded `$VAR` are refused - the runner cannot know
where a variable points, and guessing is how a screen stops screening.
A refusal is its own outcome, REFUSED_BY_POLICY: the command never reaches a
shell, and `summary.ok` is false in both plan and brief mode. For the reviewer,
REFUSED is like NOT RUN - the absence of a measurement, never on its own a
finding - and the rubric and the evidence block both say so.
Chosen deliberately, and it is stricter than today's habit: writing scratch to
/tmp is refused too. The repo's own example plan does `> /tmp/out`. Verification
output belongs in the working tree; exempting the whole system temp dir would
have made the rule unstatable, since a working tree created under /tmp then
contains its own escape hatch.
NOT covered, stated rather than implied:
- other writing git subcommands (tag, remote, config, gc) - only push is listed
- writes through a wrapper: `sh -c '...'`, `xargs`, `find -exec`, a Makefile
target, a script the criterion invokes. The screen reads the command it is
given, not what that command goes on to do.
- `>` inside a quoted string reads as a redirect, so a criterion echoing a
literal `>` is refused. Fail-closed, on purpose.
- the whole surface still runs with the invoking process's permissions; this is
a refusal list, not a sandbox.
The denylist-layer test now uses a stand-in command with a screen double: the
refusal list catches a recursive delete first, so naming one there would have
stopped exercising the denylist layer at all.
Red first: the 6 new tests failed before this change (`refuseCommand` did not
exist), and the fixture brief's four writes ran.

"The first backtick span is the command" is right for a plan, whose template
puts the command first, and wrong for a brief, whose criterion usually opens
by NAMING the thing under discussion. Measured 2026-09-18 on the repo's own
example brief: 5 of 6 criteria FAILED, 3 of them parse artifacts - `--verbose`
run as a command gave exit 2 ("invalid option"), `tests/` gave exit 126 ("is a
directory"). The rubric reads a FAILED result as decisive, so each one became
a BROKEN_SUCCESS_CRITERION BLOCKER about prose.
looksLikeCommand() screens the span by SHAPE only - no filesystem lookup, so a
span parses the same everywhere. Refused: a leading flag, a directory, a token
carrying quotes/braces/prose, and a lone relative path with a slash (an
explicit ./, ../, / or ~/ still runs, as do env-var prefixes). A refused span
is `unrunnable` with reason `not-a-command` - its own outcome, never FAILED,
and it never reaches a shell.
It deliberately does NOT scan on to a later span. "The first span that LOOKS
like a command" invents commands out of prose: in that same example brief it
would have run `whoami` and `login`, two real binaries a sentence happens to
name. An absent measurement is honest; a guessed one is not.
The shape check applies to prose spans only. Inside a shell-tagged fence the
author has already declared shell, so `[ -f x ] || exit 1` still runs.
The rubric follows: a NOT RUN result is never on its own a finding. The
Partial row now describes half-built DELIVERED CODE, and the reviewer gets a
table of the three reason strings - no-command, placeholder, not-a-command -
with what each says about the sentence rather than about the code.
Not covered, stated for the record: a multi-token span whose first token is a
non-executable file (`tests/golden/login.stdout --check`) still runs, and a
criterion whose command is real but whose binary is absent still reports the
shell's exit 127 - that is a true measurement of a missing binary, not a
parse artifact.
Red first: 4 runner tests + 1 doc-consistency pin failed before this change.

The rubric required `brief-conformance-reviewer` to classify a Success
Criterion as Full only when "its verification command/test exists and passes".
Its tools are `Read`, `Glob`, `Grep`. It cannot run anything, so "passes" was
either guessed from the command's mere existence or quietly downgraded to
"exists" — a BLOCKER-tier rule key resting on an impression.
The reviewer stays read-only — a reviewer that executes the code it reviews is
not an independent reviewer. The command does the running instead:
- `/trekreview` Phase 4.5 runs the brief's `## Success Criteria` commands
through `lib/verification/criteria-runner.mjs --brief --evidence` and captures
the block as `sc_evidence_block`, pasted verbatim into the reviewer prompt in
Phase 5. The exit code does not stop the review — a failing criterion is
exactly what the review exists to find.
- `formatCriteriaEvidence` builds that block in code: one row per criterion with
the command, the exit code and the first output line. Chose a code-built block
over an orchestrator-written summary so the orchestrator cannot narrate a pass
that never happened.
- The rubric now judges the supplied result: `PASS` supports Full, `FAILED` /
`BLOCKED` is `Broken` with the exit code cited, and `NOT RUN` is the absence
of a measurement — never evidence in either direction.
- Phase 4.5 is skipped in `quick` mode: that mode does not launch the
conformance reviewer, so there is nobody to hand the result to.
Red first: seven tests in `tests/lib/criteria-runner.test.mjs` against a
committed brief fixture whose three criteria pass, fail, and are prose-only.
The two doc pins were verified red against the pre-fix files (rubric asked
"exists and passes"; no Phase 4.5; the block reached nobody).
Suite: 1117 (1115/0/2), up 9.

Claude Code 2.1.233 stopped offering the task/todo tools on Opus 4.8, Sonnet 5, Fable 5
and newer unless CLAUDE_CODE_ENABLE_TODO_TOOLS=1 is set. /trekplan still listed
TaskCreate and TaskUpdate in allowed-tools. So did the two orchestrator reference docs,
which mirror the command but are never spawned. No prose in any command calls either tool.
Runtime effect, measured BEFORE removal on CC 2.1.274, claude-opus-5, with
CLAUDE_CODE_ENABLE_TODO_TOOLS unset:
- Probe: a throwaway project command whose frontmatter is
`allowed-tools: Read, TaskCreate, TaskUpdate`, run headless
(`claude -p --output-format stream-json --verbose`).
- The session had 151 tools. The only Task* names among them were Task, TaskOutput and
TaskStop; neither TaskCreate nor TaskUpdate was there.
- The command loaded (listed in slash_commands) and completed with subtype=success,
is_error=false and 0 tool calls. The model reported the tool as not available.
- `claude plugin validate` does not flag the unknown names either: only the one
accepted warning appears, before and after.
Verdict: harmless dead weight, no error anywhere, so this is a correctness-of-docs fix
rather than a runtime fix.
End-state gate D-05: open -> closed (defects 5 -> 4 of 7). Suite 1059 (1057/0/2), run
on a clean export of the index.

Claude Code 2.1.178 removed the TeamCreate and TeamDelete tools, so /trekplan's
"execute with team" handoff could never create a team: it always ended in its own
fallback (sequential execution). The path is removed and the fallback is now the only
behaviour. The handoff no longer offers "execute with team"; if a user asks for it
anyway, /trekplan says there is no team path and executes sequentially. Parallel
implementation stays where it already was: --decompose plus /trekexecute's
multi-session waves. No new orchestration replaces it.
- commands/trekplan.md: TeamCreate/TeamDelete removed from allowed-tools; the offered
option and the team section are gone.
- agents/planning-orchestrator.md (reference doc): the offered option is gone.
- docs/cc-upgrade-2.1.181-decision-matrix.md still names the team path: a historical
record, left as written.
End-state gate D-01: open -> closed (defects 7 -> 6 of 7). Suite 1059 (1057/0/2), run

gemini-bridge is dropped by operator directive (three repetitions). The drop is
permanent, not a wait-for-SDK-upgrade state, so the change clears every LIVE
surface that could still steer a run toward the agent and leaves the historical
record alone.
Live surfaces cleared: agents/gemini-bridge.md deleted; trekresearch (bridge
launch block, --local help, high-effort always-on pair, stats record,
degradation list), trekplan, trekbrief, research-orchestrator (mode table,
agent table, prompting block, summary line), README (feature prose, mode table,
agent prose, mermaid EXTERNAL node, conditional legend, optional-MCP
requirement, --local section), CLAUDE.md, settings.json (the whole
trekresearch.geminiBridge block), both templates, architecture.md,
command-modes.md.
trekplan high-effort Adversarial Pass 2 now degrades EXPLICITLY: it emits its
section with status "unavailable, skipped" instead of failing or vanishing. A
high-effort plan carrying no Pass 2 marker is indistinguishable from one whose
Pass 2 crashed, which is the failure mode this wording exists to prevent.
gemini_used is deliberately KEPT as a vestigial trekresearch stats field pinned
to false. Removing it would break the observability export schema for existing
consumers, and the directive was about the agent, not the field.
Not touched: CHANGELOG history and the measurement/decision docs keep their
gemini references. They record what a past version did or what was measured
then; rewriting them is the same defect class as bumping a version string
inside a measurement doc.
Driven test-first. Five new pins in tests/lib/doc-consistency.test.mjs, verified
RED before the edits, including a KNOWN-POSITIVE CONTROL asserting the
historical records still DO carry gemini references — so the empty result on
live surfaces is a measurement and not a broken query (Verifiseringsloven
ansikt 4). Agent inventory 24 -> 23 (20 spawnable + 3 orchestrator reference
docs); the <example>-block floor moves 34 -> 32 because an agent legitimately
left the inventory, not because examples went missing from a surviving one.
Docs: docs/T1-cc26-delegated-orchestration.md §8 item 3 claimed both the §6
synthesis-agent PoC and the §5 bake-off were "designed but unbuilt". That was
written in S7 and falsified the same afternoon by S12, which ran the §6 PoC and
recorded Δ main-context (faithful flow) = 0.0%, NEGATIVE. The stale wording is
what caused the settled PoC to be re-ordered as new work on 2026-09-02, so it is
struck rather than deleted and §6 gained a RUN AND DECLINED status block. The
finding is structural, not stochastic: Phase 5 spawns the exploration swarm
foreground (trekplan.md:158,338-341), so the outputs are already resident in
main before Phase 7 — delegating only the Phase-7 digest evicts nothing.
Also measured 2026-09-03 (CC 2.1.259): claude -p --output-format stream-json
runs on subscription auth with no ANTHROPIC_API_KEY and now emits a
subagent_stats block, so S12's environment-block premise is half stale. Recorded
in §8 item 4. It lowers the cost of §5; it changes nothing about §6.
Suite 1041 (1039/0/2), up from 1036 by exactly the five tests added.

Follow-up defect in the reviewer accounting added by e2aec01, found by review
and confirmed by probe before fixing.
validateFindings only WARNS on a missing `reviewer` field, so a payload can
fail schema while carrying no name. ingest then records `reviewer: null`, and
runContract turned that null into the literal reviewer name "unnamed reviewer".
MEASURED before the fix:
runContract([{findings:[{file:'x.mjs',line:1,rule_key:'NOPE',severity:'MAJOR'}]}],
{expectedReviewers:['code-correctness-reviewer']})
-> missing_reviewers = ["unnamed reviewer", "code-correctness-reviewer"]
One failure, two entries, one of them an agent nobody launched. The
`reported.delete(s.reviewer)` line was also inert for that case, since a null
name was never in the set to begin with.
Fix: skipped payloads are split by whether they carry a name. Named ones go to
missing_reviewers as before; anonymous ones increment the new
`unattributable_payloads` count, which forbids ALLOW on its own - so stripping
a reviewer name from a payload cannot restore ALLOW, and the floor does not
depend on the caller passing expectedReviewers. `allow_blocked_by` reports the
two facts separately: `missing-reviewer:<name>` and `unattributable-payload (n)`.
The old behaviour never produced a false ALLOW - it failed in the safe
direction - but it named a reviewer that did not exist, which is the kind of
output an operator would chase.
Iron Law: two failing tests first (double entry; anonymous-payload-alone must
forbid ALLOW), then the fix.
Also verified in this pass, by temporarily adding a fake reason to
UNVERIFIED_REASONS: the prose-vocabulary pin does go red when a reason is
declared in the lib but missing from agents/review-coordinator.md. A pin that
cannot fail is not a pin.
Suite 1034 (1032/0/2) -> 1036 (1034/0/2), 0 failures.

computeVerdict counted only the findings handed to it (reasoned.kept), so a
finding removed by Pass 2 or Pass 3, and a reviewer whose payload was thrown
away or never arrived, were arithmetically identical to a finding that never
existed. All three pushed the verdict toward ALLOW.
Measured before the fix (probes, 2026-09-01):
- a BLOCKER with a 101-character title -> ALLOW (Pass 2 succinctness)
- a payload with one ad-hoc rule_key is skipped WHOLE at ingest, taking a
valid BLOCKER sibling with it -> ALLOW
- a reviewer that never reported -> ALLOW
Pass 3's own no-citation / unknown-rule_key branches turned out unreachable
through runContract (validateFindings rejects those payloads first), so the
reachable exposure was Pass 2 plus the skipped/absent reviewer.
THE OPEN DESIGN DECISION, and why it went against the order's default.
The order proposed: indeterminate file-existence YES, plain succinctness NO
("a too-long finding is not an uncertain finding"). I kept the first and
overrode the second, on one principle:
A removal is `dropped` only when the test REFUTED the finding as a claim
about this codebase. Every other removal is `unverified`.
Succinctness and actionability read a `.length`. They never examine the claim,
so they cannot establish the finding is unreal - and dropping a BLOCKER for a
101-character title is precisely the fail-open shape being fixed. Three things
settled it:
1. Under the order's default the fix would have been almost inert. Pass 3's
drop branches are unreachable via runContract, so leaving Pass 2 out would
have left the only reachable finding-level exposure open.
2. Cost asymmetry, priced rather than asserted: the verdict is not a gate.
Handover 6 feeds `findings` filtered to BLOCKER+MAJOR into /trekplan
(commands/trekplan.md:218); `verdict` is optional metadata
(docs/HANDOVER-CONTRACTS.md:353). Nothing loops or re-plans on WARN. So a
false `unverified` costs WARN plus a printed reason; a false drop costs a
silent ALLOW over a live BLOCKER.
3. unknown-rule_key joins them for the same reason: an ad-hoc key is a real
defect wearing the wrong label, and v5.1.1 high-effort mode already KEEPS
those, normalised to PLAN_EXECUTE_DRIFT. Refuting them at normal effort
while keeping them at high effort would be incoherent.
no-citation stays a drop: a finding whose file is empty or whose line is
negative names no location, so it makes no checkable claim at all - the one
deterministic refutation, and what the Pass 3 prose already said it was.
Iron Law: tests/lib/coordinator-contract.test.mjs first, red (missing export +
the three measured ALLOWs), then production code. Two existing assertions were
updated AFTER implementation as contract changes, not to make the red pass.
A known-positive control pins that ALLOW is still reachable - without it,
"no ALLOW" is not a fail-closed contract, only a broken one.
lib/review/coordinator-contract.mjs
+ classifySuppression / REFUTING_REASONS / UNVERIFIED_REASONS - one
vocabulary owned by the lib, including the tokens only the LLM
coordinator emits (accuracy:refuted, file-existence:refuted/indeterminate),
so prose and lib cannot drift. Unclassified reasons default to unverified:
the default fails closed.
~ judgeFilter / reasonablenessFilter return {kept, dropped, unverified}
~ computeVerdict(findings, {unverified, missingReviewers}) -> + allow_blocked_by.
Never raises a verdict, only withholds ALLOW. Unverified findings are NOT
counted into a severity tier: their severity was never substantiated, and
counting it would be invention.
~ runContract(payloads, {expectedReviewers}) -> + unverified,
missing_reviewers, allow_blocked_by. `suppressed` stays the union of
dropped + unverified, so existing consumers (gold-eval) keep their meaning.
agents/review-coordinator.md - Pass 2/3 tables gain a fate column, new
"Suppression is two-valued" section, Pass 4 threshold table gains the two
fail-closed rows, Executive Summary must state a withheld ALLOW, Suppressed
Findings tags each line [dropped]/[unverified]. Pass 3's unknown-rule_key
bullet explicitly says high-effort does not reach that branch, so the same
input never has two documented fates.
commands/trekreview.md - Phase 5 "Reviewer accounting": the expected set is
written down before the spawn, a silent reviewer gets one re-ask and then
STOP. That extends the pattern already in the file (schema failure -> 2
bounded re-asks -> "do not feed unvalidated findings to the coordinator") to
the other two ways a reviewer goes missing, rather than softening it to WARN.
The lib's missing_reviewers stays as belt-and-braces for direct callers.
docs/agent-return-channel-defect.md - the "inferred, not observed" caveat on
the unnamed arm above 66 lines is struck: akashic-intelligence S27
(f168630) measured 2/2 unnamed agents returning against a 4370-line plan,
30449 B and 10989 B, both valid JSON. Recorded with akashic's own two
caveats intact - the measurer owns the finding, and byte-identity between
the returned string and the file on disk was not proven. The separate S25
named-arm figures are left standing; these are two measurements, not a
correction of one by the other.
No release, no version bump, no tag, no catalogue ref, no Workflow port.
Suite 1025 (1023/0/2) -> 1034 (1032/0/2), 0 failures.

CLAUDE.md claimed both STORM phases go inert when the flag is unset. Only
Phase 5 did: the flag check lives in research-loop-cap.mjs, and Phase 4.5
never invokes the cap — it was gated on effort: high alone. At high effort
with the flag unset, discovery still mined Phase-4 output and mutated the
dimension list, so `dimensions` diverged from `dimensions_baseline` and the
decline branch was unreachable for half the mechanism.
The code was the deviator, so the guard is fixed rather than the claim: the
Phase 4.5 skip-guard now names both conditions, with the reason inline.
Three surfaces scoped the flag to "the loop" and are corrected with it
(README, docs/command-modes, docs/architecture), plus the orchestrator phase
map. CLAUDE.md's claim is now true, but its stated MECHANISM was not — Phase
4.5's inertness comes from its own guard, not from the cap module — so that
sentence is corrected too.
New doc-consistency pin: the flag must be documented as gating both phases on
all four reference surfaces. Review finding 00a3af1a.
