fix(accounting,gate): the conversion claim comes from the run's ledger
Chose the side channel over neutralising pointer-shaped document text,
because the second fix changes what every document SAYS in order to
defend a tool outside the build: a source quoting a bundle listing would
come out altered and existing bundles would move bytes. This reads a
file the run already writes.

`assets.conversion` names the pair, `DocumentAssets.conversions` carries
it out of the run, `DocumentAccount.conversions` books it, and the
accounting JSON states it per document. `_declared_conversions` reads it;
`_conversions` now believes a pair only when the RUN booked it AND a
pointer block confirms it for the asset it names. The confirmation can be
forged and the ledger cannot, which is why the ledger decides.

Measured through the real `okf build`: the three arms PM reproduced
(two `<p>`, one `<p>` with `<br>`, a markdown note beside the carrier)
go forged -> refused, 3 of 3, with the known-positive True in all three.
The text-level regression guard goes 3 arms to 13, the two new ones being
a perfectly written pointer block the run never booked.

R761, rebuilt: 25 BMP sources, 19 held, 19 of 19 conversions confirmed
against 19 declared, 50 assets (29 JPEG + 21 PNG, 0 BMP), SHY 71,
u = 0, d = 0, exit 0.

feat(extract): one normalisation door removes U+00AD before the guard
Operator decision 2026-09-18, and the whole of it: okf removes the soft
hyphen, the guard is untouched, and the number is BOOKED.

`normalise_extracted(text) -> (text, removed)` in `extract`, applied
once at the end of `extract_document`, so both entry points and both
sides of a plan's `text_sha256` see one string. `_pdf_units` measures
its page offsets through the same door, because that table is
CHARACTER offsets rebuilt from the pages while the text it indexes
comes back normalised -- two readings of one document, and a table
built against the other names the wrong page with full confidence.
Removing a character removes no newline, so every line-indexed rule
downstream is unmoved by construction.

`ExtractedDocument.soft_hyphens` carries the count out;
`InboxResult.normalised` carries it per document; `account_run` reads
it off the RUN rather than recounting the source, because a second
count would be a second reader. It reaches the accounting JSON as
`normalised_soft_hyphen` at both levels and `log.md` as a
`**Normalisation**` bullet naming the count, the documents touched and
that no other character is moved.

EXPOSURE, measured and not assumed: 0 of the 78 readable documents of
the K2 reference corpus carry U+00AD or any of the four real
zero-width characters (the 8 unreadable ones raise before extraction),
0 in the pinned K2 bundle's concept bodies, and 0 across
`tests/fixtures`, `examples`, `skills`, `docs`, `src`, README and
CHANGELOG. The door cannot have moved a byte anyone here has measured.

Suite 2179 passed, 1 skipped; ruff and `mypy --strict src/` clean.

864570b320 feat(accounting): okf build accounts for every source element
okf build --accounting PATH inventories every source document before
extraction, in the gate's per-format vocabulary, and after the run gives
each element one fate (carried / pointer / coded rejection), written as
JSON and summarised in log.md. "carried" is checked against the written
concept bodies, so a gate that drops a line is found (test). Exit 1 on
anything unaccounted or double-booked. Opt-in: +744 s (+19 %) on the
43-document reference corpus, and that corpus fails the check on 24 real
losses (22 images on text-less PDF pages, 2 docx Title paragraphs).

Changed without the flag:
- okf build exits 1 when it extracted documents and persisted none.
  Door B and corpus.measure are unchanged. One test relied on exit 0.
- An image file carried through a persisted document is its own K1b
  column, no longer also extractor_unknown. The set is what the resolver
  actually carried (ExtractedDocument.files), never a byte match.

tools/okf_accounting_gate.py (checks untouched) is green on all six rows,
R761 110 of 110 under both gates.

bc39e8091f feat(assets): a bundle carries the images its sources declare (0.10.0)
Until now no reader in this package fetched, named, described or copied a
single image. `<img>`'s attributes were never read, a NISO-STS `<graphic>`
was walked past, a PDF was opened for its text alone, the converter's
markdown writer dropped every picture, and the only writer into a bundle
took `content: str`. The two lossiness warnings said so on every run, which
made the loss honest and did not make it smaller.

Measured on R761 Prosesskoden:2025, published as a 701-page PDF and as a
NISO-STS delivery: the process text is carried in full while 12 `Tabell N-N`
and 9 `Figur N-N` captions stand over nothing, because that publisher ships
those tables as raster pictures in both. Process 84's "toleranseklasse ...
er gitt i tabell 84-2" points at empty space.

THE GATE WAS WRITTEN FIRST AND RED. `tests/test_asset_gate.py` reads its
denominator out of the source (`page.images`, `word/media/`, `ppt/media/`,
`<img`, `<graphic`), never from a constant here. Measured at 332961a, built
from `git archive` and not from the editable tree: carried 0 of 8 local
images across 5 documents (9 declared), and no `assets/` at all. After: 8 of
8, with the ninth a remote source carried as a pointer without a file.

FIVE READERS PLACE, ONE MODULE DECIDES. `assets.py` owns what an image is
(sniffed from the bytes, never from the claimed extension), what it is
called (`<sha256[:12]>-<the source's own basename>`) and how it is pointed
at (one two-line block, one regex). `.xlsx` is deliberately not a row: a
block inside its pipe tables would break the `source_rows` locator, and 0 of
4 K2 workbooks hold media.

A PDF stream that is already a file is carried VERBATIM (29 of R761's 50
objects are DCTDecode); raw samples are encoded to PNG with stdlib zlib, so
no new dependency. Rendering the page region was the alternative and was
felled on determinism: a rasterised crop's bytes, and therefore the asset's
content-addressed name and the bundle's digest, would depend on the
installed rasteriser. What the encoder cannot express exactly is refused
with a code and counted, never approximated.

NO SIZE FLOOR, and that is a measurement: over the 4 828 image objects of
the K2 corpus the size distribution is a broad spread with no gap, unlike
OCR_CID_SHARE's bimodal one, so a threshold would be a number we chose.

ON BY DEFAULT, AND THE CONTROL IS TWO WHOLE BUILDS. The 43-document
reference corpus at 332961a versus rebuilt at HEAD with `--no-assets`:
865 files on both sides, `diff -rq` reports ONE difference, the added
`Images: NOT CARRIED` line in log.md. Every concept byte-identical.
Against the default: 453 -> 454 concepts, 865 -> 867 md, 0 -> 2 964 assets
(2 964 carried of 3 145 found, 4 622 pointers), 4.7 MB -> 115 MB, 2 414 s ->
3 088 s, peak RSS 6.26 -> 8.74 GB, 422 of 865 md files differ. The one new
concept has a measured cause: the pointers are body text, so a section
holding 146 of that document's images grew from 19.0 % to 30.6 % of the
extracted text and crossed `--outline-gate`'s 0.20 share clause.

THE IMAGE BYTES ARE NOT SCREENED. The guard is text-only, the pointer block
passes the gate as body text, the picture beside it passes nothing, and
log.md says so on every run.

Also fixed, both found by measuring rather than by reading:

- a markdown image is no longer read as a cross-reference. `structure._LINK`
  never looked at the character in front of the bracket, so every pointer
  would have arrived in the index as an edge to a concept that cannot exist.
- Door C carries the assets its merged concepts point at. Before this,
  importing a bundle built with `--assets` merged 6 of 6 concepts and wrote
  no `assets/` at all, so every pointer named a missing file.

4f7bd61500 feat(consume): parent reaches the reader -- excerpt field, body link, checker rule
K3-21 A. `okf consume` resolves a concept's `parent:` pointer -- a
`segment_id`, unique only inside one document's plan -- among the concepts
sharing its `source_file` (`consume.link_parents`, one pass, no file opened
again) and an excerpt carries `parent: { concept_id, title }`. Conditional
like `req_number`: a concept with no `parent` key moves no byte. A pointer
that lands nowhere is named `parent_unresolved: true`, never dropped.

The door writes ONE line into a heading-only body whose entry has a parent:
`Enclosing section: [<title>](/<bundle-relative path>)` (SPEC SS 5.1 lineage
through links, SS 6.1 the recommended absolute form and the kind in the
prose). Only such a body, so the segmented goldens' declared parents -- bodies
holding text -- are untouched. Appended AFTER structure derivation and
screened on its own (`_screened`, the `description` rule): read as body text
the link was derived into a second, unresolved `references` edge, measured on
the fixture. `segmentation.heading_only` is the one predicate the proposer and
the door share.

`okf check` gains its seventeenth rule, `parent_unfollowable`: a `parent`
that is not a concept_id and title, names its own excerpt, or names a concept
in neither `excerpts` nor `withheld` (together every considered concept).
Contract SS 8 point 6 added, the figure carries `parent`, and "additional
members are not read by the checker" now says the checker reads only the
members SS 8 names. The template tells the reader what `parent` is and that
SS 2.2 lets it read that one concept; `skill.CONDITIONAL_FIELDS` gains
`parent`. README and CLAUDE.md say what consume now reads.

Moved on purpose, each named: the SS 7.4 known-positive IS the contract
document, so `budget.known_positive` moves in every payload (13 238 / 12 893
/ 345 -> 14 455 / 14 083 / 372); `skills/okf-consume/` regenerated from the
segmented golden, whose plan declares s1 and s2 under s0 -- its example
payload now carries both parents; `test_bundle_identity` 16 -> 17 rules;
`test_shell_parent`'s byte test also accounts for the link line.

ed0418f228 fix(frontmatter): write a value a YAML reader reads back, and read both forms
K3-22. SPEC SS 11 point 1: "Every non-reserved `.md` file in the tree
contains a parseable YAML frontmatter block." Measured with PyYAML 6.0.3,
okf's own default K2 bundle failed safe_load on 41 of 455 blocks and the
R761 build on 1 of 2 763, every one a block scalar written verbatim.

Block (the profile emitter, every key): a value the K3-19 rule refuses as
plain is written double-quoted, `\` and `"` escaped; every other value keeps
its bytes, and a flow collection or an empty value is written as it stands.
The rule, now `profiles.yaml_block_plain`, agrees with PyYAML on every
top-level value in eleven measured trees (0 refused that it reads verbatim,
0 kept that it does not). Double, never single: 0 values in those trees are
`"`-wrapped and 11 193 are `'`-wrapped.

Flow (`sources`, Door A and Door B, and a run-stated flow value): the pinned
guard refuses ANY quote in a flow mapping (1.3.0, measured), so a leaf PyYAML
needs quoted has no form both read. `yaml_flow_plain` refuses it instead:
`,[]{}`, `?`, a quote, ": ", " #", a trailing `:`, a leading indicator -- a
leading `-` before a non-space excepted, which both readers take. The file
name is checked too, because it is the entry's `title` when the document
declares none. Existing codes: inbox_source_file_unaddressable,
inbox_source_title_unaddressable, source_reference_unquotable,
run_frontmatter_invalid.

Readers: parse_frontmatter, profiles' and structure's copies, and both
read_sources branches unquote a `"`-wrapped value (`\"` and `\\` decoded,
nothing else); `'`-wrapped values are untouched, and structure keeps the
single-quote rule it already had. The flow-mapping split is quote-aware, so
`{ title: "a, b" }` is one pair. The generated SKILL.md header goes through
the same block rule.

TWO K3-19 TESTS MOVED, deliberately: test_run_frontmatter built with
`sources=[{ resource: ...?languageCode=nb, ... }]`, the exact form PyYAML
refused on 2 761 of 2 761 frontmatters of K3-19's flagged build. The two
build tests now write an address without `?`; the flag-grammar test keeps
the `?` address (it only splits), and a new test holds that the build
refuses it with exit 2 and writes nothing.

1753 passed, 1 skipped (OKF_HTML_CORPUS, known). No golden moved.

77e7caba2e fix(description): write a spec point only where a YAML reader reads it verbatim
K3-19 c, repaired before delivery on its own measurement. The first R761
build from de7849e wrote 2 024 descriptions, and PyYAML's safe_load refused
the frontmatter of 217 of those concepts -- every one carrying ": " -- where
the same document had parsed with 1 refusal of 2 761 before the key existed.

`inbox._yaml_plain` is checked where the description is written, so a direct
`render_inbox_concept` caller is held to it as well as `okf build`: no leading
YAML indicator, no ": ", no " #", no trailing ":", no tab or line break.
Decided by rule rather than by a parser, because the one runtime dependency is
the guard; over the 2 024 measured values the rule and PyYAML agree on every
one (217 refused, 0 refused that PyYAML reads, 0 kept that it does not).
Omitted rather than quoted or cleaned: a quoted value comes back from the
line-oriented readers here WITH its quotes, and a cleaned one is a sentence
the source does not carry.

de7849e35b feat(description): an STS section's description is its own first spec point
K3-19 c. The NISO-STS reader records, per titled <sec>, the FIRST <p> of its
FIRST direct-child <sec sec-type="spec"> as `OutlineMark.description`. The
plan entry carries it (`description`, only where the source has one, so every
other row's plan keeps its bytes), `parse_segmentation_plan` refuses an empty,
multi-line or non-string value, and the door writes it as the concept's
`description` after the gate has seen it: it is document text persisted
outside the body the gate screens, so it is kept only on the non-blocking
floor and as the sanitized text.

SPEC SS 4.1 makes `description` RECOMMENDED and sets no length, in SS 4.1,
SS 8 or SS 11. The limit is ours and structural -- one paragraph, whole --
because a cut inside it writes a sentence the source never wrote. Measured on
R761: 2 026 of 2 761 titled sections carry a direct-child spec point; the
first <p> runs 17 / 109 / 273 / 521 / 942 characters (min / median / p90 /
p99 / max). A section with none gets no key; nothing is derived from the
title. A stated `--frontmatter description=...` replaces it.

The extracted text does not move: the description is read beside it.

912b85026b feat(frontmatter): --frontmatter KEY=VALUE stamps a key on every concept of a run
K3-19 b. `okf build --frontmatter KEY=VALUE`, repeatable, split on the FIRST
'=' (`cli.frontmatter_from_flags`) because a publisher's address carries '='
itself. The value is written verbatim on ONE line: the block form of a
`sources` list is invisible to this package's line-oriented readers, so the
flow form is the only one that survives them. Also reachable as
`build(frontmatter=...)`, `measure(concept_frontmatter_values=...)`,
`process_inbox(concept_frontmatter_values=...)` and
`render_inbox_concept(concept_frontmatter_values=...)`, keyword-only with
defaults, so every existing call site is source-compatible.

Precedence: a stated value beats what the document declares, which beats the
file name. A run may ADD any key and REPLACE only `sources` and
`description` -- the two with a derived layer below the flag. Every other key
the door writes is refused by `inbox.validate_concept_frontmatter` before a
proposal is written (`run_frontmatter_invalid`): measured from the bytes, owned
by another argument, the ownership stamp a later run reads back (including
Door A's `ingest_manifest`, which would make that door claim a Door B file),
or a derived facet whose `derived` marker would go on naming a replaced value.
A value that would not read back as stated -- empty, multi-line, or padded,
since `parse_frontmatter` strips -- is refused too.

SPEC SS 4.1 "Extensions" lets a producer add any key and SS 11 forbids a
consumer to reject one. Without the flag nothing moves: a test holds the
flagged tree to the plain one minus exactly the stated line.

ee8d5b5776 feat(identity): an STS document's doc-number names its directory and its title the address
K3-19 a. `extract.declared_identity` reads what a NISO-STS document states
about itself -- exactly one <std-ident> (<doc-number>, <year>) and exactly one
<title-wrap> (<full>, else <main>) -- and returns None for every other row,
for XML that is not STS, for an unparseable file, and for a document that
states neither. A value stated more than once is not read: an adopted
standard carries one <std-ident> per issuing body, and picking one is a guess.

`okf build` names a document's directory from its <doc-number> through the id
grammar, replacing only the file's stem. A declared name another document in
the run also claims falls back to the file name for both, said on stderr: the
existing collision gate would refuse both with "rename one", and a name read
from inside a document is not one a rename can change.

`sources[0].title` becomes <doc-number> + <year>, then the <title-wrap>
title, then the file name -- the first that survives the gate and can be
written into the flow mapping verbatim. Measured on R761, <full> carries a
comma, which ends a flow mapping, so it is never the title there; it is never
cleaned up either. `resource` stays the inbox-relative file.

Every other row, and every profile without an address, is untouched: the
identity is asked for only where `sources` is written.

36c201cc8a chore(ruff): the acceptance was whatever the default happened to be [skip-docs]
`uv sync --frozen` resolved ruff 0.15.22 and the tree read clean. A loose
install resolves 0.16.6, under which the SAME untouched code reports 148
findings -- 4 more than round 9 counted, because this round added four files.
All of them are new rules rather than new defects: 0.16 widened the default
rule set to whole families (YTT, ASYNC, PL, ISC, C4, UP, B, SIM, FURB, ...).

(`[skip-docs]` is for CLAUDE.md, which a lint-configuration change does not
reach. README's developer section IS updated in this commit.)

THE DEFECT IS NOT THE 148, IT IS THAT NOBODY CHOSE THEM. `[tool.ruff]` set only
`line-length` and `target-version`, so the acceptance was ruff's default, and
the tree stayed green only as long as the lockfile froze an old ruff. `select`
is now written down: `E4`, `E7`, `E9`, `F` (the historical default), `I`
because this tree already keeps imports sorted, and `RUF100` so a `noqa` that
has stopped meaning anything is caught rather than left as decoration. Pin
`ruff>=0.9` -> `ruff>=0.16.6,<0.17`.

Per rule, before -> after: RUF100 50 -> 0, I001 20 -> 0, ISC004 19, PLW1510 8,
C408 8, EXE001 6, RUF007 5, PLE2515 4, UP031 3, B017 3, and fourteen more with
2 or fewer -- the families out of the declared set are 0 by selection, and 148
is the number to start from if they are adopted, which is a separate decision
and not one to take inside a version-pin commit. 57 were auto-fixed; one E402
was reintroduced by the import-sorting fix merging a block away from its
`noqa`, and got the directive back rather than a bare one.

`S` IS MEASURED OUT, NOT ASSUMED OUT: it reports 2657 `S101` on a suite whose
every assertion is an `assert`, and `S603` flags 19 subprocess calls of which
one was ever marked -- selecting it buys 18 suppressions and no defect. Two
`noqa` directives naming non-selected rules were dropped with that reason
recorded in the configuration instead.

THE TWO FILES 0.16 WOULD REFORMAT ARE MARKDOWN, NOT PYTHON: `README.md` and
`docs/2026-09-08-blindsone-below-k-k2.md`. 0.16 formats fenced Python inside
markdown, and both blocks are RECORDS -- the second is a quotation of
`COST_VOCABULARY` as it stood when that measurement was taken. Reformatting a
quotation makes it stop being one, so markdown is excluded from the formatter
and `ruff format --check .` stays in the acceptance over `.py`.

`tools/okf_consume_measure.py` is fenced by the order as run-not-edited, so its
three findings are exempted by path with the reason and the debt named, and its
bytes are untouched.

THE LOCKFILE TRAP IS CLOSED, NOT AVOIDED. `uv.lock` predated the `[ocr]` extra,
so any unlocked resolve wrote that extra's transitive tree back into it -- 681
insertions over 4 deletions, twice now, and round 9 recorded the cause as
`uv run` OUTSIDE the project when it is `uv run` without `--frozen` INSIDE it.
The relock is complete for every declared extra (703 insertions, 26 deletions),
and measured after it, an unfrozen `uv run` leaves the file alone.

`ruff check src tests tools`, `ruff format --check .` (0.16.6), `mypy src` over
21 files and 1535 tests, all green.

b01492b7f5 feat(propose,cli): typography as a reserve, and the two of our own numbers it took to measure it
K3 round 5. Three questions, three answers, and two of them correct a figure
this repository published.

RETRIEVAL FIRST, because it could have reversed a default. hit@8 over the six
questions on BOTH K2 bundles -- Arm B at 629 concepts and the shipped default
at 492 -- is 5 of 6 with ranks 1,1,1,1,1,- on each, so 0 of 6 rows lost. The
order's rule reverses `--unit-fold` at >= 2 of 6; it does not fire, and the
default stands. The gold sets shrink (49->26, 20->17, 43->36, 11->18) while
every rank holds at 1, which is the fold merging concepts rather than removing
a document from the top.

TWO PUBLISHED NUMBERS CORRECTED, both ours. The S7 candidate ranks 96 of 629
and 159 of 492 were measured with the cost vocabulary passed to
`concept_scores` and NOT to `document_scores`, while `build_payload` passes it
to both; scored the way the shipped payload scores it, the same concept is 10
of 629 and 19 of 492. And round 4 attributed its non-delivery to the default
move -- measured here, it is not delivered on the Arm B bundle either, for a
different reason (knapsack eviction at 68 046 bytes of a 120 000 budget, versus
`below_k`). That column had been inherited from round 3's own build, never
re-measured.

`--pdf-headings font-reserve`, OFF, and the hypothesis behind it is falsified
by its own condition rather than by a score: position 7, the one position the
flag exists for, has THREE outline runs, so the reserve is silent there at
every minimum. It changes 0 of 12 cells on the reference and reaches 4 of 39
corpus documents, none of them rated. Built anyway because it was authorised
and because the condition is now measured rather than assumed. The predicate
lives in one place (`propose.heading_reserve_applies`) and the door receives it
as a callable, like `gate`: a plan indexes the exact string it was proposed
against, so a reserve firing on one side only would make every document it
touches a coded rejection.

The `xlsx` re-reading is confirmed on the artifact -- 11 `rule:sheet-section`
units plus 1 `rule:table-block` ingress -- but the number alone makes the cell
worse (distance 1 -> 2), because the criterion counts that ingress as a table
that should have been merged. A hit needs both halves ratified, and the
reference is the operator's.

`--sheet-section-rows` as a default: three cells better and none worse on the
twelve positions, but the K2 control moves -- row 1's gold document splits 1 ->
12 concepts and its best concept ranks 2 instead of 1. Condition not met,
default not moved.

Default build byte-identical before and after (`diff -r`, 30 md files).
Suite 1441 -> 1449; three of the eight were red first.

53d5c74c96 feat(extract,cli): typography as a PDF heading source and OCR behind an optional group, both off
A PDF carries no notion of a heading -- a heading in a PDF is a typographic
fact -- so the text stream `pdfplumber` hands the segment proposer has already
thrown away the only evidence there was. The `docx` path never had that problem:
the converter emits ATX headings and `_ATX` cuts on them. Two readers close the
gap, and both are OFF.

`--pdf-headings font` infers a heading from the conjunction this repository
already measured (size above the document's character-weighted body median AND
a bold font name, recall 1.000 / precision 0.846) and emits it as ATX in the
SAME markdown the office path produces, so `_ATX` applies unchanged and no
PDF-only heading grammar exists.

It stays off BY MEASUREMENT, and the measurement is the point of the round:
against the operator's unit worksheet it takes `pdf` from 2 of 8 to 0 of 8,
losing two exact matches. The mechanism of the loss is stated rather than
guessed -- on those documents the outline rule already recovers the document's
own numbered chapters, so a second heading source can only add. Whole-corpus
screen: 25 of 32 `pdf` change, 0 of 5 `docx`, 0 of 2 `xlsx`. The default bundle
is byte-identical before and after this commit (`diff -r`, exit 0).

`--ocr` reads a page as an image when its own text never arrived: empty, or
`(cid:N)` placeholder codes at or above a threshold READ OFF a measured
distribution -- 834 pages over 32 files, 818 at exactly 0.0 and 16 at 0.93 or
above, nothing in between. On the one corpus document with the failure: 95.07 %
cid to 0 %, 44 to 2561 words of four or more letters, 17 to 18 pages with text.
Its engine is an optional dependency group and never a runtime dependency; a
packaging test pins both halves, and without the group every affected file is a
coded rejection (`extractor_ocr_group_missing`) rather than a crash.

Also corrects two stale published facts found while measuring: the README still
said two segmentation rules were on by default after `f6fea13` made it three,
and CLAUDE.md's K2 digest named the round-3 default. The current default is
492 concepts / 944 files, `bdefa679...`.

b6a8c8bd89 feat(inbox): point every concept at the document it came from, with a locator per format
A concept named its source file by basename and, when segmented, carried a
`source_offset` into the text THIS LIBRARY extracted. Following that pointer
needed the corpus directory, the extractor and its exact transitive version --
none of which the bundle carries. Hand-walked on a real K2 concept: six steps,
four of them requiring knowledge from outside the bundle, to learn that a
requirement sits on pages 12-13 of a 20-page document.

The address is spec's: `sources: [{ resource, title }]`, where `resource` is
the dropped file's inbox-relative path (SPEC v0.2 5.1:303-306 -- "an absolute
URL, a bundle-relative path, or a path into a `references/` subdirectory").
The locator is ours, and it has to be: 5.1 has no field for a place within a
resource, and the pinned guard (1.3.0) rejects every route to putting one
inside a `sources` entry -- a non-allowlisted key by name, a nested flow list
as "scalar leaves only", and quoting as an unsupported form. So the locator is
top-level keys shaped like `source_offset`, and a path carrying a flow
terminator is refused fail-fast rather than mangled.

The unit table is built AT EXTRACTION, where the extracted text and the
original's structure are known to agree: pdf -> `source_pages` from
pdfplumber's own page numbers (a page that yielded no text does not renumber
the ones after it), xlsx -> `source_sheet` + `source_rows`, everything else ->
`source_lines`. `source_offset` stays.

Two measurements changed the design before it shipped. A `paragraphs` key for
docx would name a number the document does not have: `<w:p>` counts of
108/27/65/176/57 against converted-markdown lines of 75/33/67/144/63, not one
pair agreeing -- so the key is `source_lines` and says what it indexes. And an
empty spreadsheet row renders exactly like a table separator: the content-based
rule ate 8 empty rows on the K2 price sheet and reported its last row as 92
against a workbook that says 100. The separator is now found by position, and
`tomrad.xlsx` keeps that red.

One profile moves. `provenance` is a policy object, `None` everywhere but
`SEGMENTED_OKF_V0_2`; the other five shipped profiles are byte-identical.

K2 rebuilt from a frozen src copy: 629 concepts, 1108 files, name set identical,
0 ids moved, 479 files byte-identical, 629 changed and 0 lines removed anywhere.
629/629 now carry an address and a locator. New ref
`sha256-tree:665563a2f74423fcbcc8e4f0b0954ee73b73985ac0418de4f6987bd162a1f7c8`;
`2f82fcfe...` is stale. The pre-pass payload does not grow by one byte
(209 092 B before and after, 18 changed lines: the ref and eight per-concept
digests) -- because an excerpt carries the body, not the frontmatter, which is
also why the consumer still cannot cite "file X page 12" from a payload alone.

4ce14ae5dd feat(cli): okf build, one installed command for folder in, bundle out
Until now "run the door over a folder" was a shell loop over two scripts
under `tools/`, with nine flags between them and a `--path-prefix` rule
that lived in a code block in a measurement report. Neither script was
packaged (`pyproject.toml` ships `src/llm_ingestion_okf` only), so the
path the published K1/K2 numbers were measured on was reachable from a
clone and nowhere else.

`okf build <folder> --bundle <dir>` is that path, packaged, declared as a
console script and installed with the wheel. It is orchestration only:
the proposer and the corpus harness MOVED into the package
(`llm_ingestion_okf.propose`, `llm_ingestion_okf.corpus`) and the two
`tools/` scripts became thin entry points to them, so the published
reproduction blocks still run and there is exactly one implementation of
each rule. Neither move adds a dependency or a model call.

Two decisions belong to this layer and are stated where they are made.
A document's proposed paths are scoped by its RELATIVE PATH minus the
extension, not its basename: the door walks recursively now, and two
documents named alike in different folders would otherwise collide on a
path Door B is supposed to make impossible rather than merely detect.
And omitted timestamps do not come from the clock -- `--ingested-at` and
`--proposed-at` default to one shared epoch constant, because a
wall-clock default would put a changing byte in the artifact and take
rebuild-equals-incremental away from every caller who did not pass them.

Arm C and Arm D stay off and are not exposed here.

Measured on the 43-file K2 corpus, one invocation against the two-script
bundle of 2026-09-03: N = 43 computed, merged 39/43, coded rejections
4/43 (`extractor_unknown` 3, `extractor_empty_pdf` 1), K1b 39 + 4 = 43,
exit 0, 779.43 s. 1107 of 1108 files byte-identical. The one that
differs is the root `index.md`, by exactly the `log.md` link a commit
fifteen hours younger than the stored artifact adds -- appending that
line to the stored file reproduces the new one byte for byte. Against
the two scripts at THIS commit the trees agree in full, which is what
the byte-identity test holds.

Suite 1127 passed after `git add` (1113 before), mypy --strict clean,
ruff clean.

aa87eb8818 feat(inbox): walk the drop directory recursively
Door B listed `inbox.iterdir()` and kept only top-level files. A file in a
subdirectory was neither ingested nor refused: it appeared in none of the
result's buckets, so a nested drop produced a bundle that was silently short
of what was dropped and no count said so. That broke the K1b identity for any
inbox with folders in it. Operator decision 2026-09-06.

- `walk_inbox` is the ONE walk rule, shared with `tools/okf_corpus_run.py`:
  the denominator N is now counted over exactly the set of files the door
  ingests, rather than over a second listing that happened to agree.
- Sorted on the whole relative path, not the basename, so the order is a
  function of the tree; that is what keeps rebuild-from-scratch byte-equal to
  an incremental update.
- A concept's `source_file` is the path relative to the inbox root,
  `/`-separated. The concept NAME still comes from the basename, so two
  folders holding one basename hit the existing §3 collision refusal instead
  of one silently claiming the other's concept.
- Dot-directories and a bundle directory inside the inbox are skipped with a
  CODE, in a new `InboxResult.skipped`. Recursion makes the door's own output
  reachable as its own input; a silent skip would be the same
  absence-without-a-denominator defect one level down.
- `--path-prefix` reduces per component and rejoins with `/`, so the caller
  driving a nested corpus can carry the relative directory. Reducing the whole
  string folded the separator into a `-` and flattened `sub/sub2`.

`tests/test_inbox_flow.py::test_subdirectories_are_not_walked` asserted the
opposite and is superseded in place, with the reason written down.

Measured on the K2 corpus (flat, N=43): 39/43 merged, 4 coded, K1b holds. The
bundle digest is
`1472e98aec8643c5beee540f4c42b5e437bd26e7c61d69a91bcff799f06a6d13` over 1108
files -- byte-identical to a run of the same corpus at 190086f WITHOUT this
change (`diff -r` exit 0), so recursion costs a flat inbox nothing. It differs
from the stored 2026-09-03 artifact by one line in `index.md`
(`- [Corpus run history](log.md)`), which 95eb271 added 15 hours after that
bundle was built.

Suite 1113 passed, `ruff` clean, `mypy --strict src/ tools/` clean.

55a09d6c8e feat(profiles): a profile may name a per-suffix renderer
Arm E, capability only. A profile MAY name a renderer per suffix; no
domain-aware renderer is written here, that stays a Non-Goal, and `_RENDERERS`
is empty on purpose so the emptiness reads as a decision rather than an
omission.

THE LAYERING IS THE DESIGN, not an implementation detail. `extract.py` is the
extraction registry and must not import the contract layer, or the dependency
runs backwards and the registry stops standing on its own. So `extract_text`
gains a keyword-only `renderer: Callable[[str], str] | None`, knowing nothing
about profiles, and `inbox.py` -- which already holds the profile at that call
site -- resolves a NAME to a function. A test asserts extract.py still contains
no reference to the profile layer, because that constraint is the whole reason
the parameter is shaped this way.

The renderer runs AFTER extraction, never instead of it, so it never has to
re-implement a reader and the two cannot drift. The default is identity, which
is what keeps the five byte-pinned goldens byte-pinned -- asserted per suffix
rather than once.

An unknown renderer NAME is refused rather than falling back to identity: a
silent fallback would produce a bundle that looks rendered and is not, which is
the failure mode this arm exists to make visible. That needed a registered code
(`unknown_renderer`) and its test -- slightly beyond the step's named files,
but the capability cannot ship without defining what an unknown name does.

`tests/test_profile.py`'s exact-field-set assertion went red, as the plan's risk
table predicted. Updated deliberately with the reason recorded: that assertion
exists so a field cannot arrive without someone deciding it should, and its red
run is the mechanism working.

Suite 917 -> 926. All five goldens byte-identical.

d2a8c43d77 feat(index): one ordering helper, called by both doors
An index ordering a profile names must be honoured wherever this library
writes an index. Door B and Door C have separate index writers, so an
ordering built on Door B's `_index_sort_key` seam alone would have been a
profile field Door B obeys and Door C ignores -- silently, because
nothing raises and both files still parse. That is
`IndexPolicy.per_directory` again: a field that reads as global and acts
on one path.

`IndexPolicy` gains `sort_key`, `sort_order` and `sort_missing`. Both
order fields draw from CLOSED sets, and `sort_order` is deliberately not
a caller-supplied callable: a callable cannot be serialised into the
bundle, reproduced from it, or audited by a reader, which is the whole of
what a deterministic bundle claims. A `sort_key` the facet policy does
not name is refused too -- every entry would be missing the key and the
ordering would silently do nothing, which is this row's own defect class.

`IndexPolicy.sort_entries` is the one helper. Four stable passes, so each
is the tie-break of the next: concept path, then the named key, then the
missing group partitioned to whichever end the policy says, then
navigation last. Passes 2 and 3 are separate on purpose -- folding them
into one reversible key tuple would flip the missing group along with the
order, so `sort_missing="last"` would mean "first" under `descending`.

The tie-break is the CONCEPT PATH, not the link target, and that is
measured rather than assumed: `notes-beta.md` precedes `notes/alpha.md`
by concept path and follows it by generated filename, so ordering Door C
on the target would have re-ordered every existing Door C bundle.
`IndexEntry` carries the path for that reason; `parse_entry` leaves it
`None` and the ordering falls back to the target, which costs nothing
because no caller sorts entries it read back off disk.

Door B's two reprojection writers and Door C's index emission all route
through the helper. Door B's unfaceted path is not routed and does not
need to be: `sort_key` requires a facet policy, and a faceted profile
never reaches that writer. Door C's guarantee is bounded and stated in
the code -- `link_in_index` appends what is absent and leaves what is
present, so the order holds within a run and never re-orders entries an
earlier run wrote.

Default ordering, unchanged and now stated: with no `sort_key`, concepts
before navigation, each group ascending by concept path.

TDD, and the red was watched twice. First behaviourally with the fields
inert (both doors emitted the exact reverse of the named order), then
again with Door B routed and Door C not -- the broken world reproduced,
where a Door-B-only test would have passed.

882 tests (868 before). The five byte-pinned goldens are untouched and
green; no shipped profile moved.

f65f5fc342 fix(inbox): a segment title is declared, never marked derived
A segment's title comes from the plan, so a human adjudicated it. But
structure derivation runs over the segment body, finds no title key and
no usable heading, falls back to a stem, and adds "title" to derived.
The concept then emitted a stated fact under an inferred marker, and a
consumer that distrusts derived fields would distrust exactly the thing
a human decided. An over-marked field is the same defect class as an
unmarked heuristic: the marker is only worth something if it is
accurate in both directions.

Scoped to title alone, and pinned that way by test: number stays in
derived on a segment, because nothing about segmentation makes an
inferred document number declared. Without a segment a derived title is
still marked, so only a plan makes a title declared.

The SEGMENTED_V1 golden moves, which is the intended consequence and
the only golden that may. The four existing goldens are byte-identical
to baseline 770d8d4, measured against the sha rather than inspected.

8f9b4c8cca fix(inbox): refuse a segmentation plan that matches no dropped file
A plan is selected by content hash, so a mistyped source_sha256 matched
nothing, every dropped file fell through to the one-concept rule, and
process_inbox returned an ordinary success over a flat bundle. The
operator asked for segmentation, got none, and had no error to read --
the silent skip this library refuses everywhere else. vegnormal-okf is
about to run an N500 corpus through this path, where a silent zero
would read as "the corpus has no concepts".

The refusal asks whether a covering plan was FOUND, not whether every
file was examined, so an unreadable drop cannot mask it; and coverage
is recorded at selection, not after path validation, so a matched plan
with a refused entry path still reports its own per-file code. The
first cut got that second question wrong and an existing collision test
caught it; the case is now pinned by its own test, verified red against
the earlier form.

New code segmentation_plan_unmatched, registered in the SegmentationError
docstring register in the same commit. Fail-fast before any disk
mutation. The four existing goldens are byte-identical to baseline.

894ccdf42f fix(inbox): retire the orphaned index when a directory's last segment goes
Retiring a directory's last concept left its index.md standing, so the
directory survived a rebuild that never creates it -- diff -r reports it as
'Only in ...'. Found by S7 once its fixture was made to actually retire a
segment: with round 2 a superset of round 1, S7 stayed green with retirement
disabled entirely.

7ce548c09b feat(inbox): Door B derives structure and reprojects the index additively
Door B takes a profile (keyword-only, DEFAULT) and, under a profile carrying
facets, derives each dropped document's title, number, hierarchy and
cross-references, writes them into the concept's own frontmatter, and projects
them into the index entry.

The additive requirement is answered by one decision rather than by an
algorithm: the index is a PROJECTION of the concept files, recomputed from the
whole bundle each round. Nothing is diffed, so the three invariants hold by
construction -- rebuild-from-scratch equals incremental byte for byte,
re-dropping a document replaces its entry instead of doubling it, and a
relation formed in round 3 UPDATES the round-1 entry it is about, which an
append-only index could never do.

An unresolved pointer is marked '?' in the entry rather than omitted: during
build-up, pointing at something not dropped yet is normal, and the dangerous
version is the one that leaves no trace. Facet values are validated per file
BEFORE the write, so a producer value that breaks the grammar fails that file
and not the run.

DEFAULT is byte-identical with and without the new parameter, and is asserted
so. Door B keeps writing the literal 'generated: true' rather than the
profile's ownership stamp -- routing it through the profile would move
DEFAULT's bytes and orphan every bundle this door has already written; that is
a separate question and answering it here would have answered it silently.

18 new tests; suite 677 -> 695.

e60f7261a1 build(deps): move the guard pin from >=0.3,<0.4 to >=1.2,<2.0
The operator's condition for revisiting the pin was met (v1.2.0 contains
the flow-mapping frontmatter fix, commit 5870483) and the operator has now
approved the move itself, dispatched as its own order because it changes
consumer-visible Requires-Dist and shifts golden-fixture admission.

Floor 1.2: this library needs the flow-mapping support (`generated: { by:
x, at: y }`) that landed there — without it Door C fail-secures every
concept carrying that stamp. Ceiling <2.0, not narrower: the guard's own
1.0.0 release freezes its exported surface until a 2.0.0, and explicitly
keeps calibration (severities, dispositions) free to move within 1.x, so a
tighter ceiling here would claim a stability guarantee neither side needs.

Re-measured through Door C against the guard's own default
(allow_reserved=True, matching how the earlier recommendation measured
it), over the 9 concept documents across all four Door-A goldens:
4/9 admitted at 0.3.4 -> 8/9 admitted at 1.2.0, confirming the number
already reported. The ninth (a `sources` block-list carrying `resource`)
stays refused by design (G30) and is not expected to move.

Failing-test-first: test_guard_adapter.py::test_guard_version_is_inside_the_pin
and test_packaging.py::test_the_only_runtime_dependency_is_the_security_boundary
were updated to the new pin first and confirmed red against the
still-installed 0.3.4, then `uv sync --extra extract` installed 1.2.0 and
both went green. Full suite (615), ruff and mypy --strict clean.

f536e1384d feat(guard): bump the pin to >=0.3,<0.4 and pin Door C's allow_reserved=False
Measure first, widen after. The 19-fixture guard-surface suite was re-run
against v0.3.4 in a scratch venv before the range moved, and reproduced the
three deltas measured against v0.3.3 exactly, with none added. v0.3.4 is the
tag pinned rather than v0.3.3 because it shipped first and repairs a quadratic
regex (okf._MD_LINK_RE) that sits on Door C's own call path.

Door C now passes allow_reserved=False explicitly. The guard added the keyword
in the 0.3 line and defaults it True for received bundles, which would merge a
sender's index.md / log.md instead of rejecting them. The override keeps the
unconditional reserved-name refusal committed to before the keyword existed,
and the reason is structural rather than a second opinion on the guard's scan:
Door C generates the merged bundle's index.md from what it merged and writes
every merged concept verbatim, so a sender's index.md would be a second and
irreconcilable claim on one path.

This is not a behaviour change for anyone on the previous pin: under v0.2.0
the keyword did not exist and reserved names were refused by construction.

The floor is >=0.3 and not >=0.2 for a measured reason. allow_reserved is
absent in v0.2.0 and present from v0.3.0 onward, checked across all five tags:
a >=0.2 floor would admit a version that raises TypeError on every Door C
import. That measurement also corrects a recorded premise -- the plan said the
keyword "shipped in v0.3.3", which read the first version we ran the suite
against as the version it was introduced in. The conclusion held; the reason
did not, and the reason is what a future bump would have relied on.

test_door_c_pins_allow_reserved_false_against_the_guards_default locks both
halves: that the guard still defaults True, without which the override is a
no-op that would pass forever over nothing, and that Door C overrides it.

586 tests, mypy --strict clean, goldens byte-identical.

6f42c10608 refactor(phase-3): the bundle contract becomes a profile object
Phase 3 step 1. What Phases 1 and 2 hard-coded about a valid bundle now
lives on one frozen `BundleProfile`, and `DEFAULT` states exactly the
ingest-spec v1 + Phase 2 contract. Nothing observable changes: the 425
existing tests are unmodified and green, and `git diff --stat examples/`
is empty, which is assumption C1's whole proof. Both oracles are real
rather than nominal — the golden suite compares `read_bytes()`, and Door
B pins its frontmatter block as an exact string.

Moved onto the profile, each one previously a constant with a reader:
the index name and its managed-link shape (template plus pattern, kept
honest by a round-trip test), the three filename namespaces
(`ingest-`/`inbox-`/`import-` and `.md`), the reserved `verdict` layer
(duplicated in `manifest` and `inbox` before this), the frontmatter key
order, and the `source_query` whitespace collapse.

Two details are worth naming. The DEFAULT key order spans both doors:
Door A's seven keys and Door B's six are subsequences of one nine-key
order, so a single canonical order reproduces both doors byte-for-byte.
And emission follows commons decision D1 — ordered prefix, then any
remaining keys sorted — which is the mechanism the proving consumer's
hash registry needs; under DEFAULT the tail is always empty.

`TypePolicy` refuses the reserved layer at CONSTRUCTION (assumption C3),
so a profile admitting `verdict` cannot be built, let alone passed to a
door. It reports refusals rather than raising them, because Door A
refuses with `ManifestError` and Door B with `MaterializationError` for
the same type — the wording and the stable code come from the policy,
the exception class stays each door's own.

Deliberately NOT moved, each for a stated reason: the required and
allowlisted key sets the proving consumer needs (no code reads them
until the STRICT_V1 validator exists, and a field nothing reads is a
claim nothing tests), the id grammar (a pattern the slugger derives a
separator class from, not a flat name), `NAME_MAX_BYTES` (a filesystem
fact, not a contract choice), and every disposition/origin/channel
vocabulary (guard territory, always). The profile is also not exported
from the package root yet — the optional `profile` argument on the
flows is step 3's plumbing change.

f10fc60de2 feat(import): Door C flow against an injected import gate (Phase 2 step 5)
Reads an external OKF bundle as {bundle-relative path -> document text},
hands it WHOLE to an injected gate over the guard's okf.import_bundle (a
bundle-level call: it resolves the cross-link graph across concepts), and
merges only concepts clearing the non-blocking floor. Same injection pattern
as Door B, so the core stays dependency-free while the CI channel for the
real guard is settled.

Two constraints shaped the design and are pinned by tests:

- A merged concept is written VERBATIM. Stamping provenance into it would
  require round-tripping its frontmatter through this library's line-oriented
  parser, which cannot represent the block lists the guard's parser accepts --
  silent data loss -- and would persist bytes the guard never screened.
- Ownership is therefore proven by content identity: identical bytes at the
  target name are a no-op re-merge (re-import of an unchanged bundle is
  idempotent), and anything else at the name is refused. Curated content and
  an updated concept are refused alike; refusing is what never destroys.

The floor is fail-closed beyond the plan's "no error" wording: an error, an
unrecognised disposition, and a concept the gate returned no verdict for are
all refusals. quarantine_review stays its own bucket, as at Door B.
origin/channel are validated against the guard's pinned vocabulary -- it
derives trust from origin by enum identity, so an unrecognised string would be
silently downgraded rather than caught.

Three primitives promoted for reuse rather than duplicated:
reduce_to_id_grammar and check_filename_length to materialize.py, and
extract.decode_text. Door C slugs the WHOLE concept path, so tables/users.md
and views/users.md stay distinct. Concept discovery folds case explicitly
rather than globbing *.md, whose case-sensitivity follows the filesystem and
would import the same bundle differently on APFS and ext4.

README's "what is gated today" section corrected: it claimed nothing is gated,
which is no longer true, but the honest statement is narrower than "the doors
are gated" -- the library cannot verify that an injected adapter is a real
guard, and a permissive stub is believed.

405 tests green; ruff, ruff format and mypy --strict clean.

d812a839be feat(inbox): Door B flow against an injected guard gate (Phase 2 step 3)
`process_inbox(inbox_dir, bundle_dir, ingested_at, *, okf_type, gate)`:
per dropped file, bytes -> extract_text -> guard gate -> render -> collision
gate -> write -> index link. Returns an InboxResult whose four buckets
(persisted / quarantined / rejected / failed) are disjoint and complete, so a
file that vanished shows up as a missing entry rather than as nothing.

The guard is INJECTED rather than imported. The library calls no guard
function and makes no security decision: the Gate adapter returns a
GateDecision carrying the guard's own disposition value and the sanitized
text, and the flow only branches on it. That keeps the core dependency-free
while B2 (the guard's CI channel) is still open, and lets the persist and
refuse branches run deterministically against a test double.

Decisions taken with the operator this session:

- What is persisted is the gate's SANITIZED text, not the extracted text.
  Screening one string and writing another would make the verdict a statement
  about bytes nobody kept. `sanitize` is exported by the guard precisely for
  callers composing the checklist themselves, so this is sanctioned API, not
  a reimplementation. Door B has no model call, so the fenced text the
  bookends produce for a transform is never persisted.
- Quarantine is reported apart from rejection. QUARANTINE_REVIEW means "hold
  for human review" — an operator queue — where FAIL_SECURE is a decision.
  No quarantine directory in v1; that stays an extension point.

Fails closed by construction: only the guard's non-blocking floor (`warn`)
persists, so a renamed member, a future disposition or an adapter typo lands
in `rejected` rather than being guessed safe.

One bad file never aborts the run. Only three conditions fail the whole run,
and each is wrong for every file at once: an invalid `ingested_at`, a reserved
`okf_type`, and a missing inbox directory.

New code `inbox_slug_collision`: two dropped files reducing to one generated
name are BOTH refused. Persisting one would let iteration order decide the
winner, and overwriting would lose the other's content.

Phase 1 primitives are reused, never duplicated, so four helpers become
package-internal names (write_bytes, link_in_index, parse_frontmatter,
INDEX_NAME) and link_in_index learns to append to an empty index — Door A
always seeds its index with bundle_summary, but Door B has no summary to
invent and must not open with a blank line.

b7f5ce3800 feat(inbox): fail-fast on filenames over the 255-byte limit
The slug inherited the whole filename stem with no length bound, so a long
dropped name produced a filename the filesystem cannot hold — surfacing as an
untyped OSError at the write, with an errno that differs per platform (63 on
macOS, 36 on Linux). The inbox contract promises a typed per-file outcome, so
the gate belongs in the pure function that forms the name.

Refusal, not truncation: truncating is lossy AND collision-prone — two long
names sharing a prefix would reduce to one filename and the second write would
silently claim the first file. Same posture as `inbox_slug_empty`: the library
never invents a filename the operator did not give it. The message carries the
actual size and the limit, since the fix is to rename the dropped file.

Limit verified empirically on APFS 2026-07-25: a 255-byte component writes, a
258-byte one raises errno 63. ext4 and NTFS cap at the same 255.

New MaterializationError code `inbox_slug_too_long`, registered in the errors
docstring and in the error-code conformance suite.

6b9b21c602 feat(inbox): Door B provenance render + filename slug (Phase 2 step 2)
Second guard-independent step of Phase 2: the pure functions that turn a
dropped file's name and extracted text into an OKF concept file, with the
§7-analogous honesty marker. No runtime dependency, no guard call, no model
call — the persist gate is still steps 4–5.

- `inbox_slug(filename)` reduces a name to the Phase 1 id grammar
  (`[a-z0-9][a-z0-9-]*`): drop the final extension, lowercase, collapse every
  run of non-grammar characters to one `-`, strip the ends. A name that
  reduces to nothing fails fast rather than inventing an `untitled` fallback
  that would silently collide across unrelated files.
- `inbox_filename(slug)` namespaces it `inbox-{slug}.md`, disjoint from
  `index.md`, Door A's `ingest-*`, and `promoted-verdict-*` for every slug the
  grammar admits (asserted over a hostile-name table, not one example).
- `render_inbox_concept(...)` emits the six-key layer in fixed order — `type`,
  `title`, `source_file`, `source_sha256`, `ingested_at`, `generated: true` —
  with the digest taken over the ORIGINAL dropped bytes, never the extracted
  text, so provenance stays re-verifiable against the operator's file. Body is
  normalised to LF-only with exactly one trailing newline (dropped files
  legitimately arrive with CRLF; Door A can validate instead because it renders
  its own bodies).

Slugging normalises to NFC first. macOS (APFS) hands filenames over
decomposed, so `é` arrives as `e` + combining acute: without normalising, the
combining mark alone becomes a separator and the base letter survives
(`cafe`), where the composed form is one non-grammar character (`caf`) — one
visual filename, two slugs, depending on where the string came from. The test
pins both forms with explicit escapes so it cannot depend on the test file's
own encoding. No transliteration, deliberately: mapping non-ASCII to ASCII is
a semantic claim the slugger cannot make (Norwegian `møte`/meeting would
become `mote`/fashion). The readable name survives verbatim in `title`.

Fail-fast gates, all typed: reserved `verdict` layer refused at this door too
(the promotion gate stays the only path in); a title that is multi-line or
contains `[`/`]` refused, matching Door A's manifest rule, because it renders
verbatim into `- [title](target)`; a multi-line `source_file` refused because
line-oriented frontmatter would take the injected lines. Four new stable
MaterializationError codes, each with its entry in the test_error_codes.py
registry: `inbox_slug_empty`, `inbox_title_invalid`,
`inbox_source_file_invalid`, `okf_type_reserved`.

`ingested_at` validation moves to a shared `validate_ingested_at` in
materialize.py and is called by both doors — one rule in one place is what
keeps the determinism contract from drifting apart per door.

TDD: tests/test_inbox.py precedes the implementation. 325 tests green;
mypy --strict, ruff check/format, and the sanitize|quarantine|lexicon
boundary grep-gate clean; runtime dependencies still exactly none; the Phase 1
golden suite still passes byte-for-byte.
